Wavelet-based analysis of heart-rate-dependent ECG features.

BACKGROUND: Wavelet-based methods of analyzing ECG signals have been used to identify specific features in cardiac arrhythmias. Since some of these features are rate dependent, it is a requirement that they are examined across a range of physiological heart rates. The wavelet transform is a signal analysis tool that can elucidate spectral and temporal information simultaneously from complex signals, including the ECG. The aim of this study was to identify the local frequency characteristics of the ECG using a real-time wavelet scalogram and to study the rate dependence of these features. METH…

Can nutrition influence circadian rhythm and heart rate variability?

Recent studies indicate that there is an interaction between biorhythms, the biological clock and triggers, which may be important in the pathogenesis of altered heart rate variability (HRV) and blood pressure variability (BPV). Circadian rhythms are under the influence of, and physiological variables are mediated by the activation of the adrenals, sympathetic/parasympathetic, hypothalamic and pituitary activity. Emotional stress, physical exertion, sleep deprivation and large fatty meals are major triggers of myocardial ischemia, angina, infarction, sudden cardiac death (SCD) and stroke. Thes…

Fetal heart rate observations in the brain-damaged infant.

The objective of this study was to describe the fetal heart rate patterns and underlying pathophysiologic changes in the brain-damaged fetus. Fetuses with brain damage from hypoxic ischemic encephalopathy do not manifest uniform fetal heart rate patterns. However, these fetuses do show distinct fetal heart rate patterns that permit categorization based on their admission heart rate, subsequent changes in their baseline rate; and neonatal findings. Based on the observations of infants brain damaged in utero because of hypoxic ischemic encephalopathy, the intrapartum fetal management will depend…

Perioperative assessment of fetal heart rate and uterine activity.

Improvements in surgical techniques and anesthesia allow women the option to schedule needed surgery during pregnancy. However, perioperative monitoring of the fetus and uterine activity remains a matter of controversy. Monitoring may allow rapid improvement of the fetal status or uterine activity when early compromise or contractions are detected. The reassurance and decreased medicolegal risks provided by perioperative monitoring may offset the cost of a perinatal nurse and use of monitoring equipment even though the drug and anesthetic effect on the fetal heart limit the benefits of monitor

Increases in heart rate during an air pollution episode.

This paper assesses whether air pollution increases resting heart rates in 2,681 men and women aged 25-64 years who participated in the MONICA (monitoring of trends and determinants in cardiovascular disease) Augsburg cohort. Increases in heart rate were observed during the air pollution episode in January 1985 compared with non-episode days adjusted for cardiovascular risk factors and meteorologic parameters. Consistently, heart rates were also elevated at high concentrations of sulfur dioxide, total suspended particulates, or carbon monoxide. Acceleration in heart rates indicates an altered …

[Rate of prenatal detection of congenital right heart defects].

Congenital right heart lesions (including tetralogy of Fallot, pulmonary valve stenosis, pulmonary atresia with intact ventricular septum, Ebstein's anomaly and dysplastic tricuspid valve) account for about 19% of congenital cardiac anomalies. We performed a retrospective study in order to assess the percentage of patients with significant right heart lesions (requiring therapy in the first year of life), which is detected prenatally and referred to a centre for perinatal treatment. From 1/1990 until 12/1997 congenital right heart lesions were diagnosed in 21 fetuses and 190 infants (211 patie

Autonomic balance revisited: panic anxiety and heart rate variability.

The analysis of heart rate variability (HRV) is becoming widely used in clinical research to provide a window into autonomic control of HR. This technique has been valuable in elucidating the autonomic underpinnings of panic disorder (PD), a condition that is marked by reports of heart palpitations. A body of research has emerged that implicates a relative reduction in HRV and cardiac vagal tone in PD, as indicated by various HRV measures. These data are consistent with the cardiac symptoms of panic attacks, as well as with developmental evidence that links high vagal tone with enhanced attent…

Diurnal variation in baseline heart rate of anencephalic fetuses.

The diurnal change in baseline fetal heart rate (FHR) of four anencephalic fetuses at 20, 23, 24 and 30 weeks of gestation were examined. The mean baseline FHR in 00.00-06.00 h, 06.00-12.00 h, 12.00-18.00 h and 18.00-24.00 h were compared by one-factor ANOVA and Scheffe's test in each case. The diurnal variations in baseline FHR were recognized in all subjects (P < 0.01). In 3/4 subjects, the lowest values were at 00.00-06.00 h. The diurnal variation in baseline FHR might be caused by maternal factors because it was present even in the anencephalic fetuses that had no central nervous system ha…

Heart rate variability in workers exposed to carbon disulfide.

It is assumed that the cardiovascular impairments resulting from CS2 exposure may be associated with some functional disturbances within the autonomic nervous system. We adopted the heart rate variability (HRV) analysis to investigate the sympathetic and parasympathetic functions of the autonomic nervous system in workers exposed to carbon disulfide. The studies were performed on 152 workers, aged 24-66, with the period of exposure ranging from 5-38 years and 93 age-matched, non-exposed, healthy individuals as the control group. The HRV analysis concerned time-domain (AVG R-R, SD R-R, modal, m…

[The significance of heart rate analysis in psychiatric questions].

Heart rate analyses have become an important diagnostic tool for the assessment of autonomic nervous system function. Measurements of heart rate variability (HRV) have been frequently used to establish criteria for a cardiovascular autonomic neuropathy (CAN). Methods of HRV have also been applied to unmedicated patients with psychiatric disorders and to investigate the impact of psychotropic drugs on autonomic cardiac functions. In one of our studies 20% of patients with alcohol dependence (n = 60) fulfilled the criteria of CAN as a result of vagal neuropathy. In other psychiatric disorders cl…

Pseudo-sinusoidal heart rate associated with major cardiac anomalies.

Pseudo-sinusoidal fetal heart rate (pseudo-SHR) pattern, characterised by intermittent episodes of sinusoidal heart rate and normal fetal heart rate (FHR), is considered benign although its pathophysiologic mechanism remains obscure. We present a case of pseudo-SHR episodes throughout all stages of labor associated with multiple major cardiac anomalies. We suggest that when confronted with repeated episodes of pseudo-SHR pattern, the possibility of an underlying cardiac anomaly should be considered.

Heart rate and late mortality in cardiac transplant recipients.

There are currently 104 patients at this centre who have survived at least 3 months after orthotopic cardiac transplantation. Seven of these long-term survivors have subsequently died and in three cases death was sudden and unexpected. All three of these patients had been noted to have inappropriately high resting heart rates (> 130 b.min-1). The rhythm was sinus tachycardia, supraventricular tachycardia or both intermittently. The heart rates of all 104 long-term survivors were recorded from ECGs taken at routine follow-up visits every 3 months for one year and annually thereafter. The overal…

Effect of imipramine treatment on heart rate variability measures.

Recently, heart rate (HR) variability has received considerable attention, and a decreased HR variability has been linked to a significant risk of cardiovascular illness. We have previously reported such a decreased variability in panic disorder patients. In this study, we report on HR variability in 12 depressed and 6 panic disorder patients at baseline and 1 and 3 weeks of treatment with imipramine as measured by the standard deviation, mean consecutive difference and the standard deviation of the mean consecutive difference of the R-R intervals in supine, supine deep breathing and standing …

Electronic fetal heart rate monitoring with intrauterine fetal demise.

Management of patients during labor is often based upon interpretation of electronic fetal heart rate tracings. Misdiagnoses of these records can lead to inappropriate actions, including cesarean delivery for presumed fetal distress. This case study, using a heart rate tracing from a patient with intrauterine fetal death, describes the findings, reviews the origin of the electronic signal, and outlines the means to distinguish the fetal and maternal cardiac impulses.

Fetal heart rate patterns associated with amniotic fluid embolism.

Two cases with recorded fetal heart rate response during maternal amniotic fluid embolus are reported. Fetal heart rate abnormalities can be associated with the development of amniotic fluid embolus or occur during the acute events of amniotic fluid embolus. Attention to the management of amniotic fluid embolus in the mother must not neglect the fetus. Improvement of maternal oxygenation and perfusion can effect intrauterine resuscitation; however, when this is not successful early delivery of the fetus may be necessary for its survival.
