Three-dimensional transesophageal echocardiography in diagnosis of intermediate atrioventricular septal defect in the adult: case report and literature review.

BACKGROUND: Intermediate type atrioventricular septal defect is less frequent than complete or partial atrioventricular septal defect, and is rarely encountered in the elderly and the utility of three dimensional transesophageal echocardiography in the diagnosis has not been reported to date. CASE PRESENTATION: In this case report, we described a rare case of an intermediate atrioventricular septal defect in an adult patient and we showed the valuable utility of real time 3D transesophageal echocardiography in the diagnosis and future surgical planning. The patient was referred to a tertiary

Intraoperative device closure of perimembranous ventricular septal defects without cardiopulmonary bypass: preliminary results with the perventricular technique.

OBJECTIVE: In infants undergoing closure of perimembranous ventricular septal defects, cardiopulmonary bypass remains one of the factors that prolongs hospital stay and morbidity. A new technique was used to close the defects under echocardiographic guidance without cardiopulmonary bypass to prevent the deleterious effects of bypass. METHODS: Recently, the Amplatzer membranous ventricular septal defect device (AGA Medical Corp, Golden Valley, Minn) was introduced. The device has a double-disc design with a short connecting waist. The left ventricular disc has an eccentric design to prevent en

Mitral and tricuspid valve repair in an operated case of complete atrioventricular septal defect: A surgical challenge case report!

Atrioventricular canal defects constitute about 3%-5% of total congenital heart defects, with a prevalence of 0.3-0.4/1000 live births. Mitral valve defects are associated with atrioventricular canal (septal) defects in most cases. Our case is of a 37-year-old male who underwent double patch closure of atrial and ventricular septal defects at the age of 24 years, now presenting with NYHA-III symptoms of severe mitral regurgitation and severe tricuspid regurgitation. The mitral valve was of atrio-ventricular canal type. We repaired both mitral and tricuspid valves using soft rings. Because of a

Infective Endocarditis With a Giant Vegetation on the Tricuspid Valve With a Congenital Ventricular Septal Defect in a 63-Year-Old Patient.

Infective endocarditis is an infection of the endocardial tissue of the heart, primarily affecting the cardiac valves. Of various causes, untreated or undiagnosed congenital heart defects are known contributors. This report presents the case of a 63-year-old man with infective endocarditis of the tricuspid valve associated with an uncorrected ventricular septal defect. We describe our elderly patient who underwent successful surgical repair, including tricuspid valve reconstruction and closure of the ventricular septal defect, after a long life uncomplicated by severe pulmonary hypertension.

Slit-Robo signalling in heart development.

The Slit ligands and their Robo receptors are well-known for their roles during axon guidance in the central nervous system but are still relatively unknown in the cardiac field. However, data from different animal models suggest a broad involvement of the pathway in many aspects of heart development, from cardiac cell migration and alignment, lumen formation, chamber formation, to the formation of the ventricular septum, semilunar and atrioventricular valves, caval veins, and pericardium. Absence of one or more of the genes in the pathway results in defects ranging from bicuspid aortic valves

[Corrected transposition of the great arteries in patients over 65].

Three cases with corrected transposition of the great arteries in patients older than 65 years are described. Two had atrial situs solitus and one atrial situs inversus. One had pulmonary valvular stenosis with valvular calcification and a small ventricular septal defect in association with ischemic heart disease. This patient died due to cardiac failure at the age of 80 years. The second case was associated with ventricular septal defect, atrial septal defect and pulmonary hypertension. The third patient presented with mild tricuspid regurgitation. Although this congenital heart malformation

Congenital heart defects in Sotos sequence.

Of 10 patients with typical Sotos sequence, 5 had various congenital heart defects. They included 2 patients with secundum atrial septal defect, and one patient each with patent ductus arteriosus with mitral valve regurgitation, tricuspid atresia plus pulmonary atresia, and ventricular septal defect. Increases of head circumference and weight gain were less accelerated in the patients with congenital heart defects than in those without heart defects, while growth in length was comparable between the 2 groups. In view of these findings, it is suggested that the rate of congenital heart defects

Distinguishing Septal Heart Defects from the Valvular Regurgitation Using Intelligent Phonocardiography.

This paper presents an original machine learning method for extracting diagnostic medical information from heart sound recordings. The method is proposed to be integrated with an intelligent phonocardiography in order to enhance diagnostic value of this technology. The method is tailored to diagnose children with heart septal defects, the pathological condition which can bring irreversible and sometimes fatal consequences to the children. The study includes 115 children referrals to an university hospital, consisting of 6 groups of the individuals: atrial septal defects (10), healthy children

Contribution to the development of the heart, Part II: Morphogenesis of congenital heart diseases.

A concept for the explanation of the formal genesis of congenital cardiac malformations is introduced, based on the observation that 1. the septa in the outflow-tract of the heart are formed in spatial relation to changes in form of the external cardiac wall and always along the longer sides of the ovally deformed pathway of flow, 2. the muscular septa (ventricular and atrial septa) arise as protrusions due to different directions of ventricular growth. Accordingly, no septa develop when normal deformation fails to occur (e.g. truncus communis); septa are incompletely developed when the deform

Trends of mortality rate in patients with congenital heart defects in Germany-analysis of nationwide data of the Federal Statistical Office of Germany.

BACKGROUND: Congenital heart defects (CHD) are still associated with an increased morbidity and mortality. The aim of this study was to analyze trends of mortality rates in patients with CHD between 1998 and 2018 in Germany. METHODS: Data of registered deaths with an underlying diagnosis of CHD were used to evaluate annual mortality between 1998 and 2018. Polynomial regressions were performed to assess annual changes in CHD-associated mortality rates by age groups. RESULTS: During the 21-year study period, a total of 11,314 deaths were attributed to CHD with 50.9% of deaths in infants (age <

Prevalence and Pattern of Congenital Heart Diseases in School Children in Dongguan, China.

BACKGROUND: To determine the prevalence and pattern of congenital heart disease (CHD) among elementary school children in Dongguan, China. METHODS: Between November 2011 and November 2012, 540,574 school children from 449 elementary schools were screened for CHD by trained doctors in Dongguan. The school children who were suspected to have CHD were referred to a pediatric cardiologist and/or an echocardiographer for a complete evaluation. RESULTS: The total prevalence of CHD was 2.14 per 1,000 school children (1,157/540,574). The most common form of CHD was isolated ventricular septal defect

Systematic Mapping of Conduction Tissue Regions During Congenital Heart Surgery.

PURPOSE: Damage to the cardiac conduction system is a major risk of congenital cardiac surgery. Localization of the conduction system is commonly based on anatomic landmarks, which are variable in congenital heart diseases. We introduce a novel technique for identification of conduction tissue regions based on real-time fiberoptic confocal microscopy. DESCRIPTION: We developed a fiberoptic confocal microscopy-based technique to document conduction tissue regions and deployed it in pediatric patients undergoing repair of common congenital heart defects. The technique applies clockface schemati

Pulmonary hypertension after shunt closure in patients with simple congenital heart defects.

BACKGROUND: Patients with simple shunt lesions, such as atrial septal defect (ASD), ventricular septal defect (VSD) and persistent arterial duct (PDA) remain at risk of developing pulmonary hypertension (PH) even after correction of their cardiac defect. We aimed to assess the contemporary prevalence of PH in a well characterized nationwide group of patients based on the German National Register for Congenital Heart Defects. METHODS AND RESULTS: We included all patients >16 years of age with an isolated diagnosis of ASD, VSD or PDA. Only patients with previous surgical or interventional closu
