In the United Kingdom and United States of America, fulminant viral hepatitis is due mainly to sporadic (non-parenteral) non-A, non-B hepatitis and hepatitis B whereas that caused by hepatitis A virus is very uncommon and by the herpes viruses remains rare. Recent advances in fulminant non-A, non-B hepatitis have come with the identification and cloning of a virus (27-34 nm) in the enteral variety (hepatitis E) which is prevalent in the Indian sub-continent, North Africa and elsewhere, especially in pregnant women. Virus-like particles (50-70 nm) with ultrastructural features similar to the to…

A case of severe alcoholic hepatitis was observed in a man aged 30 years. Attention is called to possible diagnostic and therapeutic difficulties. Despite abstinence lasting 9 months the results of biochemical tests failed to return to normal values.

Drug-induced hepatitis still arouse many practical problems, as their pathogenesis has not been fully elucidated yet, given the absence of specific criteria. Drug-induced hepatitis are acute and chronic. Cytolytic hepatitis, cholestatic hepatitis and mixed hepatitis belong to the former category. Drug-induced hepatitis show various clinical and biological pictures, generally similar to those of viral hepatitis. In the most cases, the prognosis is good and their evolution favourable. Cytolytic hepatitis--the result of a wider hepatocytic necrosis--have a more severe prognosis. The most severe f…

A previously well young woman presented with an acute hepatitis resembling viral hepatitis and a liver biopsy after 5 weeks showed features of acute hepatitis. Infection with identifiable viruses or other organisms known to cause hepatitis was excluded. Evidence for autoimmune chronic active hepatitis ab initio included prolonged fever, lymphadenopathy, urticaria, arthralgia, Coombs' positive hemolytic anemia, lymphopenia, a markedly raised level of immunoglobulin G and a positive antinuclear antibody test. Liver biopsies after 4 and 28 months showed typical histologic features of autoimmune c…

The high rate of infection with hepatitis B virus in certain defined populations in industralized countries and among the general population in many non-industrialized countries stresses the need for hepatitis B vaccines. Hepatitis B, one of at least six different forms of viral hepatitis, may progress to chronic liver disease, including chronic persistent hepatitis, chronic active hepatitis, cirrhosis and primary liver cancer. Primary liver cancer is one of the ten most common cancers in the world today. Immunization against hepatitis B is required therefore for groups at high risk of infecti…

Three patients with granulomatous hepatitis due to cytomegalovirus are described. They are compared to the three previously described patients with this disease, and their clinical and serologic characteristics are discussed. Similarities and differences between infectious mononucleosis (Epstein-Barr virus) and cytomegalovirus infections are adduced. That cytomegalovirus may be a cause of granulomatous hepatitis in the adult is stressed.

Acute alcoholic hepatitis is an anatomical (fatty liver with sclerosing hyaline necrosis) and a clinical (hepatomegaly with a variety of symptoms of hepatic failure) entity arising out of chronic alcoholism, and of a typically 'pre-cirrhotic' state. Although fatal in 25% of acute cases due to failure of homeostasis, it often leaves a centrilobular scarring necrosis which in more than 60% of cases progresses to nodular cirrhosis. Continued alcoholism worsens the prognosis. Alcoholic hepatitis may be confused with acute abdominal catastrophes or with a hepatoma. The characteristic Mallory bodies…

Three cases are described in which a chronic relapsing febrile illness was associated with granulomas in the liver. None of the known causes of hepatic granulomas was present in these patients. In two cases there was clinical improvement after corticosteroid treatment and this was associated with resolution of the histological changes on liver biopsy. These three cases support the concept that chronic granulomatous hepatitis of unknown aetiology is a distinct entity.
