D-penicillamine prevents the development of hepatitis in Long-Evans Cinnamon rats with abnormal copper metabolism.

The Long-Evans Cinnamon rat is a mutant strain that contracts hereditary hepatitis and, eventually, spontaneous hepatocellular carcinoma. Because we found a corresponding gross copper accumulation in the liver of the rats, we examined whether the development of hepatitis in our rat system could be prevented by administration of D-penicillamine. D-Penicillamine is a copper-chelating agent and one of the drugs effective for human Wilson's disease, in which abnormal copper metabolism is also observed. The results show that D-penicillamine treatment inhibited the elevation of serum transaminases,

The hepatic expression of vitamin D receptor is inversely associated with the severity of liver damage in genotype 1 chronic hepatitis C patients.

BACKGROUND/AIMS: Low 25-hydroxyvitamin D serum levels have been associated with the severity of liver fibrosis in genotype 1 chronic hepatitis C patients (G1CHC), and experimental evidence suggested a hepatoprotective role of vitamin D via interaction with hepatic vitamin D receptor (VDR). We assessed the hepatic expression of VDR protein and its association with liver disease severity. METHODS: Ninety-one consecutive patients with biopsy-proven G1CHC and available frozen liver tissue were evaluated. Ten subjects without chronic liver diseases and nine patients with autoimmune hepatitis serve

Low 25-hydroxyvitamin D serum levels correlate with the presence of extra-hepatic manifestations in chronic hepatitis C virus infection.

OBJECTIVE: Chronic HCV infection is associated with extra-hepatic manifestations. Recent studies have suggested an immunomodulatory role for vitamin D during HCV infection. We investigated the association between serum vitamin D status and the presence of HCV extra-hepatic manifestations. METHODS: 25(OH)D serum levels were assessed in 94 HCV(+)RNA(+) patients [including 48 patients with mixed cryoglobulinaemia (MC) vasculitis]. Correlations between serum 25(OH)D levels and the presence of extra-hepatic manifestations of HCV infection were analysed. RESULTS: Overall, 84 of 94 patients (89%) h

Dissolution of copper-rich granules in hepatic lysosomes by D-penicillamine prevents the development of fulminant hepatitis in Long-Evans cinnamon rats.

BACKGROUND/AIM: The Long-Evans cinnamon rat has a mutation homologous to the human Wilson disease gene, leading to gross copper accumulation and the development of hepatitis. D-penicillamine, a copper-chelating drug widely and efficiently used in treating Wilson disease, has also been shown to prevent hepatitis in Long-Evans cinnamon rats. The objectives of this study were: i) to investigate the effectiveness of D-penicillamine when administered to the already affected animals, and ii) to elucidate the mechanism of action of the drug. METHODS: Long-Evans cinnamon rats were divided into groups

Changes in hepatic fibrosis and vitamin D levels after viral hepatitis C eradication using direct-acting antiviral therapy.

BACKGROUND: Vitamin D (VD) is important in hepatic fibrogenesis in animal models and human studies. VD deficiency is associated with liver fibrosis progression. Metabolic dysfunction of the liver, as an intermediate organ for VD metabolism, contributes partly to this deficiency. We hypothesized that improving hepatic fibrosis and inflammation in chronic hepatitis C (CHC) patients after eradication with direct-acting antivirals (DAA) would increase 25-hydroxyVD [25(OH)VD] levels. METHODS: Eighty CHC patients (17 chronic hepatitis, and 63 cirrhosis) were enrolled. Baseline characteristics, hepa

Burden and Clinical Impact of Hepatitis D Virus Co-Infection Among HBsAg-Positive Patients in Mauritania.

BACKGROUND: Hepatitis B virus (HBV) infection remains highly endemic in sub-Saharan Africa, where hepatitis delta virus (HDV) co-infection substantially worsens liver disease outcomes. Mauritania has long been suspected to be a high-burden setting for HBV-HDV co-infection, yet contemporary data describing its clinical and virological impact remain limited. METHODS: We conducted a hospital-based cross-sectional study at the National Institute of Hepato-Virology (INHV) in Nouakchott, including 401 HBsAg-positive patients. Demographic, clinical, biological, and virological data were collected. H…

Receptor recognition of hepatitis B and D viruses: Structural insight and its application to drug discovery.

Hepatitis B and D viruses (HBV, HDV) infect host hepatocytes through interaction of its N-terminal preS1 domain of the viral surface protein to the host entry receptor, sodium taurocholate cotransporting polypeptide (NTCP). The successful development of bulevirtide as the first clinically-approved anti-HDV drug has highlighted the NTCP-mediated entry process as an attractive antiviral target. Recent structural studies of NTCP, including its complex with preS1 peptide, have revealed a unique mechanism by which HBV and HDV recognize their receptor. These structural information have also provided

Chronic hepatitis D infection is associated with distinguishing microbial and functional features in the gut microbiome.

BACKGROUND: The microbiome of patients with hepatitis D virus (HDV) has yet to be characterized. This study aims to (1) characterize gut microbial composition in HDV, (2) determine its functional profile, (3) identify microbial species that contribute to changes in pathway expression, and (4) correlate the changes in the gut microbiome with clinical markers of disease severity. METHODS: Cross-sectional analyses of 35 HDV-infected patients and 32 healthy controls (HCs) were performed. DNA and RNA were isolated from stool and sequenced by shotgun-sequencing. Microbial and functional profiles we…

Correlation of Bile Acid Dynamics to Bulevirtide Response and Disease Severity in Patients With Hepatitis D.

BACKGROUND: Bulevirtide (BLV) blocks hepatitis D virus (HDV) entry by targeting the sodium taurocholate co-transporting polypeptide (NTCP). This study assessed the relationship between bile acid (BA) levels and antiviral response to BLV. METHODS: Serum BA levels were monitored in HDV-infected patients pre-, under, and post BLV treatment. Virologic response (VR) was defined by ≥ 2log10 decline in HDV-RNA and discriminated versus virologic non-response (VNR). Delta BA levels (ΔBA) were calculated and analysed in relation to HDV-RNA dynamics. Finally, the effect of BLV treatment cessation on BA …

The causal relationship between vitamin D deficiency and chronic hepatitis: A 2-sample Mendelian randomization approach.

Observational studies have suggested an association between vitamin D (Vit D) and chronic hepatitis (CH). However, the nature of this relationship remains uncertain. Therefore, the aim of this study is to investigate the potential causal links and specific directions between Vit D deficiency and CH. Gaining insights into the potential cause-and-effect association between Vit D deficiency and CH is crucial for understanding the disease progression and developing innovative strategies for the prevention and management of CH, as well as its associated complications. We employed a bidirectional 2-

Understanding the Natural History of Chronic Hepatitis D: Proposal of a Model for Cost-Effectiveness Studies.

BACKGROUND: As new therapeutic options become available, better understanding the potential impact of emerging therapies on clinical outcomes of hepatits D virus (HDV) is critical. OBJECTIVE: The aim of this study was to develop a natural history model for patients with hepatitis D virus. METHODS: We developed a model (decision tree followed by a Markov cohort model) in adults with chronic HDV infection to assess the natural history and impact of novel treatments on disease progression versus best supportive care (BSC). The model time horizon was over a lifetime (up to 100 years of age); sta…

Long-Term Treatment with Bulevirtide in Patients with Chronic Hepatitis D and Advanced Chronic Liver Disease.

Bulevirtide (BLV) is approved for the treatment of chronic hepatitis D (CHD). Because only limited long-term experience has been reported, we aimed to evaluate the efficacy and safety of BLV treatment in patients with advanced chronic liver disease (ACLD). We performed a retrospective analysis of patients with CHD who received BLV 2 mg/day for >12 months at a tertiary center. Virological response (VR) was defined as a reduction in hepatitis delta virus-ribonucleic acid (HDV-RNA) ≥2 log10 from baseline or HDV-RNA negativity and biochemical response (BR) as gender-specific normalization of trans…

Hepatitis-D Virus Infection Is Not Impaired by Innate Immunity but Increases Cytotoxic T-Cell Activity.

Approximately 70 million humans worldwide are affected by chronic hepatitis D, which rapidly leads to liver cirrhosis and hepatocellular carcinoma due to chronic inflammation. The triggers and consequences of this chronic inflammation, induced by co-infection with the hepatitis D virus (HDV) and the hepatitis B virus (HBV), are poorly understood. Using CRISPR technology, we characterized the recognition of HDV mono- and co-infection by intracellular innate immunity and determined its influence on the viral life cycle and effector T-cell responses using different HBV and HDV permissive hepatoma…
