The distinction of area postrema syndrome between MOGAD and NMOSD.

BACKGROUND AND OBJECTIVES: Both myelin oligodendrocyte glycoprotein-IgG associated disorders (MOGAD) and neuromyelitis optica spectrum disor … acks while 31 had APS at disease onset. In patients with MOGAD, 2 had nausea, 3 had vomiting, 5 had hiccups, and 1 patient presented with all three symptoms above. In patients with NMOSD, 70.2 % had nausea, vomiting and hiccups at the same time during APS attacks. Apart from the medulla oblongata, other subtentorial regions were also affected in 6/7 MOGA …

A trembling detour to the diagnosis of encephalitis: a neuroborreliosis case report.

In this Clinical Rounds, we present a case of neuroborreliosis in a man aged 59 years who initially complained of headache, tremor, and hiccups. Diagnosis was delayed, partly due to an atypical presentation but mainly because subtle MRI findings were overlooked. There is emerging recognition that radial and patchy hyperintensities in cerebral peduncles, mesencephalon, and pons is a distinct MRI feature of neuroborreliosis. Dissemination of this knowledge could contribute to more timely and efficient diagnoses in future patients.

When gut and nerves collide: A case of Neuromyelitis Optica Spectrum disorder after golimumab for ulcerative colitis.

Anti-TNF alpha agents, such as golimumab, are effective treatment options for moderate to severe inflammatory bowel disease. However, demyelinating syndromes have been reported as rare adverse events. We present the first documented case of Neuromyelitis Optica Spectrum Disorder with positive aquaporin-4 antibodies following golimumab therapy for ulcerative colitis, manifesting as intractable hiccups secondary to Area Postrema Syndrome. The patient was treated with plasmapheresis and rituximab, with partial improvement.

Post-oesophagectomy hiatal hernia.

This video tutorial presents the laparoscopic repair of a post-oesophagectomy hiatal hernia in a 69-year-old patient who had an oesophagectomy 5 years previously for distal oesophageal cancer. He presented acutely with epigastric pain, nausea and hiccups. During the operation, the herniated small bowel was reduced and found to be viable. The defect was sealed by suturing the conduit to the diaphragm. At a follow-up examination 23 months after the repair, the patient is asymptomatic and there is no evidence of recurrence.

Baclofen-Induced Encephalopathy in End Stage Renal Disease.

Baclofen is a highly used centrally acting GABA agonist that continues to be an effective therapy for spasticity and chronic hiccups. The renally dependent excretion determines the circulating concentrations and guides effective dosing to decrease adverse reactions. Caution should be considered in administering baclofen to patients with decreased renal function. We present a patient with end stage renal disease on hemodialysis with recent baclofen ingestion who presented with toxic encephalopathy that was resolved with additional dialysis sessions.

[Paracoccidioidomycosis in cerebral hemisphere and brainstem: case report].

We report on a 36 years-old man that had been at the Amazon forest four years before. Six months before the admission he had developed a progressive quadriparesis, gait ataxia, dysphagia, dysarthria, difficulty in breathing and hiccup. The gadolinium-enhanced T1-weighted MRI showed a lesion into the right parietoccipital area and another into the medulla, that was the largest. There was any evidence of tuberculosis or AIDS. The patient was submitted to microsurgical approach to the medulla. Pathological examination revealed paracoccidioidomycosis. Treatment with anphotericin B till 2100 mg was…

Orthostatic hypotension in a case with multiple sclerosis.

A 37-year-old woman with a 5-year history of multiple sclerosis is reported. She began having recurrent syncope even in the sitting position; other neurological features included hiccup, faciooro-lingual flushing and clumsiness of the hands. She had alternating Horner's syndrome, mild hypoalgesia of the right face, exaggerated deep tendon reflexes of the upper extremities, decreased deep sensation and ataxia of the upper extremities, and incomplete transverse myelopathy with a T4 sensory level. Head-up tilt testing confirmed orthostatic hypotension with relative preservation of the heart rate …

Novel Approach to Medullary Oblongata Cavernous Hemangioma.

We report the case of a 31-year-old male patient who presented with complaints of left upper and lower limb weakness with giddiness, imbalance while walking, hiccups, nasal regurgitation, and history of difficulty in swallowing. He was evaluated and diagnosed to have a space-occupying lesion in the left medulla oblongata-cavernous hemangioma. He underwent suboccipital craniotomy and excision of the lesion. Postoperatively, he regained strength and balance. He is asymptomatic during follow-up.

Ipsilateral Hemiparesis in Lateral Medullary Syndrome: A Case Report on Opalski's Syndrome.

Lateral medullary syndrome (LMS) is a neurological disorder usually presenting as loss of pain and thermal sensation over the ipsilateral face and contralateral half of the body, ipsilateral limb ataxia, Horner's syndrome, dysphagia, nystagmus, hiccups among other symptoms but never with limb weakness. In the present case, the patient presented with ipsilateral hemiparesis, which can be attributed to the extension of the infarct caudally beyond the pyramidal decussation, affecting the corticospinal fibers in the upper cervical cord, a variant of LMS, known as Opalski syndrome (OS).

Atypical presentation of gangrenous cholecystitis: A case series.

Gangrenous cholecystitis (GC) is a serious complication of acute cholecystitis that has been associated with increased morbidity. Patient with GC can present with a wide variety of non-specific clinical, laboratory, and imaging characteristics, making the diagnosis challenging. This disease requires emergent treatment, which is why a quick and reliable diagnosis is essential for the wellbeing of the patient. The authors herein present a case of GC in a patient whose initial complaint was intractable hiccups, and provide a thorough review of the literature of cases of GC with atypical presentat…

Intractable nausea and vomiting from autoantibodies against a brain water channel.

BACKGROUND & AIMS: Antibodies against the water channel protein aquaporin (AQP)-4 cause a spectrum of inflammatory, demyelinating, central nervous system disorders called neuromyelitis optica spectrum disorders (NMOSDs); these primarily affect the optic nerves and spinal cord but also the brain. Symptoms of intractable nausea, vomiting, and hiccups reflect involvement of AQP4 in the brainstem area postrema and account for gastroenterological presentations. We investigated the frequency of intractable nausea, vomiting, or hiccups in patients with NMOSD who tested positive for immunoglobulin G a…

Cardioneuroablation eliminating cardiac asystole associated with area postrema syndrome: a case report and literature review.

BACKGROUND: There have been few instances of symptomatic bradycardia-arrhythmia in the context of area postrema syndrome (APS), and some of them have been implanted permanent pacemakers. Cardioneuroablation (CNA) has emerged as a viable therapy for the treatment of syncope induced by neutrally mediated bradycardia or atrioventricular block. METHODS: We report a young patient with recurrent cardiac asystole and syncope following persistent hiccups caused by neuromyelitis optica spectrum disorder (NMOSD), who successfully completed CNA treatment and avoided permanent pacemaker placement. We als…

[Successful treatment of neuromyelitis optica spectrum disorder by early initiation of plasma exchange].

A 39-year-old woman initially developed vomiting and intractable hiccup, followed by progressive dysphagia, dysarthria and hypoglossal nerve palsy. She was admitted to our department on the 30th day of illness. MRI-FLAIR images of the brain revealed a hyperintense lesion in the dorsal medulla. A diagnosis of neuromyelitis optica spectrum disorder (NMOSD) was entertained according to the clinical course and the MRI images. The dysphagia was intractable to methylprednisolone pulse therapy, and so a course of plasma exchange therapy was initiated on the 32nd day of illness. After the third plasma…

Characteristics of breathing abnormality in Leigh and its overlap syndromes.

In this report we describe the respiratory patterns of six patients with Leigh syndrome, including two individual cases with accompanying clinical phenotypes of Alpers disease and mitochondrial encephalopathy with ragged red fibers. In five cases where sleep apnea was monitored, each one showed isolated or post-sigh central apnea, hiccup, apneusis-like breathing and obstructive apnea in various combinations. The remaining patient with Alpers/Leigh overlap syndrome showed an apneusis-like pattern of dyspnea. The sleep structure was examined in three patients. Two patients with brainstem lesions…
