Autophor cementless total hip arthroplasty for osteoarthrosis secondary to congenital hip dysplasia.

The authors present a consecutive series of 38 arthroplasties for osteoarthrosis secondary to hip dysplasia. All arthroplasties were performed by a single surgeon using Mittelmeier II (Autophor, Osteo AG, Selzach, Switzerland) ceramic cementless components. Crowe grades of subluxation were 60% grade I, 16% grade II, 12% grade III, and 12% grade IV. In all cases, the acetabular component was seated in the true acetabulum. The mean follow-up period was 75 months (range, 40-122 months). By the time of review, six hips (16%) had required revision for aseptic loosening, mainly of the femoral compon

Cementless total hip arthroplasty with femoral head bone grafting for hip dysplasia.

Femoral head bone grafting was required to augment acetabular bone stock in 19 cases of hip dysplasia treated with cementless total hip arthroplasty. All acetabular grafts provided mechanical support for the cementless acetabular component. Radiographic evaluation of the fixation of the femoral components at an average of 3 years after surgery revealed an optimum appearance in all cases. All porous-coated acetabular components remained stable, but only one of six (17%) nonporous threaded acetabular components maintained stability. One threaded acetabular component has been revised for symptoma

The prone hip extension test: a method of measuring hip flexion deformity.

The "prone hip extension test" is described as a simple, convenient, and reliable method of measuring hip flexion contracture. The test may be performed on a padded examining table or bed, it is applicable to the spastic patient, and as the patient is examined prone, it provides a method of flattening the lumbar spine under visual control to avoid obtaining either a falsely high or low value. It, therefore, circumvents the problems commonly associated with the traditional "Thomas Test".

Anatomic restoration of congenital hip dysplasia in adulthood by total hip displacement.

Congenital dysplasia, treated or untreated, produces a hip joint difficult to reconstruct and is even more difficult when coxarthrosis supervenes producing significant disability. Total hip replacement can be dramatically successful in these patients, and equals those with coxarthrosis without congenital dislocation. The acetabulum must be totally reconstructed and relocated as near as possible to its original orientation. Usually a small straight stem femoral component must be placed into a generally constricted femoral canal. A thoughtful preoperative plan including X-ray templates is absolu

Expert Consensus on Trendelenburg Position Postless Hip Distraction Technique in Hip Arthroscopy.

Postless hip distraction technique is a novel traction method for hip arthroscopy developed in recent years. Studies have shown that, compared with the traditional post hip distraction technique, it can effectively solve perineal complications, lower the technical threshold of hip arthroscopy, and improve patients' postoperative satisfaction. The standardization of this technique is expected to promote the advancement and popularization of hip arthroscopy. To this end, this Consensus identified 17 questions of the greatest clinical concern that were selected through systematic literature retri

Acute total hip arthroplasty for anterior hip fracture-dislocation: a case report.

Hip fracture-dislocations accompanied by femoral head or acetabular fractures remain challenging, and the optimal treatment strategy is still controversial. Anterior hip fracture-dislocations are particularly rare, and reports describing acute total hip arthroplasty (THA) for this injury pattern are limited. We report the case of a 74-year-old active woman who sustained an anterior hip fracture-dislocation with femoral head and acetabular anterior wall fractures after a low-energy fall. The injury was classified as Pipkin type IV and Brumback type 4B. Following closed reduction, acute THA was

Traction arthrography for quantitative assessment of hip distractibility in early hip disorders.

PURPOSE: Dynamic hip distractibility is an intraoperative marker of hip microinstability; however, a quantitative assessment in the outpatient setting has not been established. This study aimed to quantify hip distractibility using traction arthrography in patients with early hip disorders and to identify clinical and radiographic factors associated with increased distractibility. METHODS: We retrospectively reviewed 84 patients (84 hips) with early hip disorders who underwent traction arthrography, including frank dysplasia (30 hips), borderline hip dysplasia (21 hips), labral tear (14 hips)

Trajectory of urinary incontinence symptoms following total hip arthroplasty for hip osteoarthritis.

INTRODUCTION: Urinary incontinence (UI), a prevalent quality-of-life-limiting condition, frequently coexists with hip osteoarthritis, potentially through pelvic floor dysfunction related to obturator internus muscle involvement. Several reports suggest improvement after total hip arthroplasty (THA), although postoperative time-course changes remain unclear. This study aimed to clarify the temporal trends in UI after THA and examine association of postoperative UI trajectories with functional recovery. MATERIALS AND METHODS: This retrospective study included 118 female patients who underwent T

Ipsilateral Lower-Limb Alignment after Total Hip Arthroplasty for Dysplastic Hip Osteoarthritis.

BACKGROUND: Total hip arthroplasty (THA) is an effective treatment for dysplastic hip osteoarthritis (DHOA); however, factors associated with lower limb alignment changes after THA remain unclear. This study investigated factors associated with these changes in patients undergoing THA for DHOA. METHODS: We analyzed 121 patients undergoing THA for unilateral DHOA between 2018 and 2022. Based on the hip-knee-ankle angle (HKAA) changes, patients were divided into neutral (HKAA change <3°), varus (HKAA change ≥3° varus), and valgus (HKAA change ≥3° valgus) change groups. We compared their backgro

Total hip arthroplasty in patient with hip fracture and congenital pubic diastasis.

Congenital pelvic deformities, such as pubic diastasis, significantly complicate the surgical planning and execution of total hip arthroplasty (THA), the gold standard for treating degenerative hip disease unresponsive to conservative measures. These challenges are exacerbated in the context of trauma, because of disrupted anatomical landmarks and soft tissue imbalances. THA becomes particularly demanding in cases involving traumatic femoral neck fractures in patients with congenital anomalies of the pelvis. We present a unique case of a 62-year-old man with an acute, post-traumatic left-sided

Outcomes of Total Hip Arthroplasty for Congenital Hip Dislocation: A Retrospective Study.

Introduction Congenital hip dislocation (CHD) represents a long-standing public health issue in Morocco, with a late diagnosis often leading to advanced joint degeneration. While periacetabular osteotomy is preferred in young adults with preserved joint space, total hip arthroplasty (THA) becomes necessary in end-stage cases. However, THA poses challenges due to anatomical deformities and the increased likelihood of revision surgeries. This study evaluates the clinical, radiological, and functional outcomes of THA performed for CHD and compares them with existing literature. Materials and meth

Hip Pain Due to Leiomyoma After Total Hip Arthroplasty: A Case Report.

Total hip arthroplasty (THA) is a widely recognized treatment for various hip disorders, significantly improving pain and function. Despite advances in surgical techniques, a proportion of patients experience postoperative pain, leading to a necessity for revision. This report outlines a unique case of chronic hip pain post-THA associated with the unexpected identification of a large leiomyoma, an occurrence not yet reported in the literature. A 75-year-old female with a history of right THA presented with persistent left hip pain two years after the operation. After left hip replacement, she

Rare intrapelvic migration of a sliding hip screw after hip fracture fixation.

Sliding hip screws (SHSs) are commonly used for the operative treatment of extracapsular neck of femur fractures. We report 2 rare cases of intrapelvic migration of an SHS, resultant of non-unions, threatening regional major vessels. Only a handful of such cases have previously been reported in the literature. We provide an up-to-date review of the literature, propose rationale for how a lag screw may migrate, and suggest how this complication can be prevented.

Residual Hip Dysplasia in Children With Unilateral Hip Dislocation-Does Side Matter?

BACKGROUND: Developmental dysplasia of the hip (DDH) is the most common musculoskeletal abnormality in infants and young children. Despite the fact that the left hip is most frequently affected, literature and clinical experience suggest a poorer outcome for right-sided hip dislocation in DDH. On the basis of this hypothesis, we studied the outcomes (residual dysplasia, number of surgeries, and complication rate) of right-sided unilateral hip dislocation in comparison with left-sided unilateral hip dislocation in DDH up to 8 years follow-up. METHODS: We identified all patients with unilateral

Watchful waiting for the undisplaced hip dysplasia when undergoing contralateral hip reconstruction.

PURPOSE: Patients undergoing surgical reduction of the dislocated hip secondary to developmental dysplasia may have a contralateral undisplaced dysplastic hip. This study aimed to compare the observation of the contralateral undisplaced dysplastic hip with the treatment by acetabuloplasty in terms of persistent dysplasia to determine the need for bilateral surgery. METHODS: Acetabular dysplasia was defined by an acetabular index (AI) of more than 30 degrees plus a center edge angle (CEA) of less than 20 degrees. A surgical database was queried retrospectively for a cohort between 2008 and 201

Rapidly Destructive Hip Disease Following Intra-Articular Corticosteroid Injection of the Hip.

BACKGROUND: While recent reports have suggested that hip corticosteroid injections can hasten joint degeneration, there are few published data on the topic. The purpose of the present study was to evaluate for an association between corticosteroid injection and rapidly destructive hip disease (RDHD) and to determine the rate of, and risk factors for, occurrence. METHODS: This study was conducted in 2 parts. First, to assess for a potential association between hip corticosteroid injection and RDHD, a case-control analysis was performed. Patients who developed RDHD between 2013 and 2016 served
