Protective effect of vitamin B therapy on bone and cardiovascular disease.

Vitamin B deficiency causes many diseases, which may be improved by vitamin B supplementation. Homocysteine, a sulphur amino acid, is frequently increased in patients with vitamin B deficiency, chronic renal failure and metabolism disorders. Bone and cardiovascular disease is often detected in patients with renal disease or homocystinuria. This review shows the most important discoveries, including recent patents, about vitamin B therapy not only in hyperhomocysteinemic patients but also in the general population. Vitamin B supplementation may be useful to reduce bone and cardiovascular events…

Cystathionine β-synthase deficiency: Of mice and men.

Cystathionine β-synthase (CBS) deficiency (Online Mendelian Inheritance in Man [OMIM] 236,200) is an autosomal recessive disorder that is caused by mutations in the CBS gene. It is the most common inborn error of sulfur metabolism and is the cause of classical homocystinuria, a condition characterized by very high levels of plasma total homocysteine and methionine. Although recognized as an inborn error of metabolism over 60years ago, these is still much we do not understand related to how this specific metabolic defect gives rise to its distinct phenotypes. To try and answer these questions, …

Clinical manifestations in thrombotic children with factor V Leiden mutation.

Sixty-three children with thrombosis were screened for factor V Leiden (FVL) and the mutation was found in 20 patients, 5 of whom were homozygous for FVL. The clinical features of 15 heterozygous and 5 homozygous patients with factor V Leiden mutation were analyzed. Additional risk factors were found in 19 of these 20 patients (95%) with FVL mutation. The most frequent predisposing exogenous factor was infection. The majority of children with FVL mutation had had several associated disorders prior to their vascular occlusion, such as homocystinuria, Behçet's disease, collagen tissue disorders,…

Ocular findings in several metabolic diseases.

Changes in ocular findings have been noted in association with several metabolic diseases. In homocystinuria the crystalline lens in the majority of cases is subluxated inferiorly, while in Marfan's syndrome the dislocation was upward. In cystinosis, slit-lamp examination reveals numerous gold crystal-like cystine deposits in both the cornea and bulbar conjunctiva. Patients with galactosemia have cataracts of the "oil drop" type, which usually can be seen with an ophthalmoscope even though the opacity is not dense. Eight patients with Lowe's syndrome who were observed had cataracts, and four o…

[Hereditary ectopia lentis].

If not due to trauma, ectopia lentis is usually caused genetically. It is a main symptom of several syndromal disorders such as Marfan syndrome or homocystinuria. Also other connective tissue disorders convey an elevated risk for ectopia lentis. Isolated ectopia lentis is frequently caused by genetic alterations as well, most commonly due to mutations in ADAMTSL4. Depending on the molecular basis, the consequences for the management of patients may differ significantly: On the one hand, possible accompanying symptoms may require a specific surveillance and treatment. Also, the risk for other f…

Overview of homocysteine and folate metabolism. With special references to cardiovascular disease and neural tube defects.

This overview addresses homocysteine and folate metabolism. Its functions and complexity are described, leading to explanations why disturbed homocysteine and folate metabolism is implicated in many different diseases, including congenital birth defects like congenital heart disease, cleft lip and palate, late pregnancy complications, different kinds of neurodegenerative and psychiatric diseases, osteoporosis and cancer. In addition, the inborn errors leading to hyperhomocysteinemia and homocystinuria are described. These extreme human hyperhomocysteinemia models provide knowledge about which …

[Homocysteine metabolism].

Homocysteine, a sulfur amino acid, is an intermediate metabolite of methionine. In 1969, McCully reported autopsy evidence of extensive arterial thrombosis and atherosclerosis in children with elevated plasma homocysteine concentrations and homocystinuria. On the basis of this observation, he proposed that elevated plasma homocysteine (hyperhomocysteinemia) can cause atherosclerotic vascular disease. Hyperhomocysteinemia is now well established as an independent risk factor for atherosclerotic vascular disease. Mild hyperhomocysteinemia is quite prevalent in the general population. It can be c…

[From gene to disease; from homocysteine to hyperhomocysteinemia].

Homocysteine is a sulfhydryl containing amino acid which is produced as an intermediate product in the metabolism of the essential amino acid methionine. Apart from environmental factors such as the intake of folate and other B vitamins, the level of homocysteine in the blood is determined by the genetically based activities of several enzymes involved in the methionine or folate cycle. The well-known congenital defect homocystinuria is due to homozygosity for mutated cystathionine beta synthase. It is characterized by severe hyperhomocysteinemia, which leads to arterial and venous disease at …

Practical approach to the diagnosis and treatment of apnea of infancy.

Twenty-three infants aged 6 days to 9 months, sequentially referred for apnea or apparent life-threatening events (ALTE), were studied. A se … cribed for four patients. In three nonmonitored subjects apnea recurred. Two infants died, one with homocystinuria complicated by brain hemorrhage and another with a fulminant viral infection. These investigative results and their outcome indicate that in cases of apnea or ALTE in apparently healthy infants, in-patient monitoring and evaluation is of much value. …

[Mathematical approach to the differential diagnosis of phenotypically similar hereditary diseases].

The paper, which is based on the report made at XIV International Genetical Congress, concerns the problems of applying mathematical methods in differential diagnosis of relatively rare hereditary diseases (Marfan's disease, homocystinuria, mucopolysaccharidosis, epiphyseal dysplasias) and in analysis of phenotypical differences in dominant and recessive type of inheritance. Results of algorithmic mechanisms of detection, which provide a diagnosis during establishment of phenotypically homogeneous subgroups of patients within existing nosologic entities, are discussed. With such an approach, e…

Reduction of the false-positive rate in newborn screening by implementation of MS/MS-based second-tier tests: the Mayo Clinic experience (2004-2007).

The continued expansion of newborn screening programmes to include additional conditions increases the responsibility of newborn screening l … screening for congenital adrenal hyperplasia (CAH), tyrosinaemia type I, methylmalonic acidaemias, homocystinuria, and maple syrup urine disease (MSUD). Beginning in 2004, this approach was applied to Mayo's newborn screening programme and resulted in a false-positive rate of 0.09%, a positive predictive value of 41%, and a positive detection rate of 1 affected …

Cryoextraction of dislocated lenses.

Twenty-two dislocated lenses were removed using cryoextraction. They occurred in a consecutive series of 1,800 planned intracapsulars. Included in the complete series were 16 dislocated lenses in 3 groups: Marfan's syndrome, homocystinuria and congenital types of dislocations. None of these were removed since the patient's vision was adequate in each case. These patients are still under observation. In one case, we used the Hagler-Calhoun double pronged needle. We think this procedure is entirely too traumatic. In 21 cases, the dislocated lenses were removed intracapsularly. In one instance, t…

Homocysteine and the pathogenesis of atherosclerosis.

The homocysteine theory of arteriosclerosis was discovered by study of arteriosclerotic plaques occurring in homocystinuria, a disease caused by deficiencies of cystathionine synthase, methionine synthase or methylenetetrahydrofolate reductase. According to the homocysteine theory, metabolic and nutritional abnormalities leading to elevation of plasma homocysteine cause atherosclerosis in the general population without these rare enzymatic abnormalities. Through studies of metabolism of homocysteine thiolactone, the anhydride of homocysteine, in cell cultures from homocystinuric children, the …
