False negative apraclonidine test in two patients with Horner syndrome.

BACKGROUND: Because of denervation supersensitivity, a miotic pupil in a sympathetically-denervated eye dilates in response to a dilute or weak alpha-1-agonist drug. A reversal of anisocoria after topical apraclonidine is considered as a positive test result that diagnoses a unilateral Horner syndrome. HISTORY AND SIGNS: Two women aged 34 and 46 years with a cocaine-confirmed oculosympathetic defect (Horner syndrome) were tested with 1 % topical apraclonidine on separate days. THERAPY AND OUTCOME: In one patient, her miotic Horner pupil dilated marginally but not enough to reverse the baseli…

[Three cases of abducens nerve palsy accompanied by Horner syndrome].

PURPOSE: To report three cases of nasopharyngeal carcinoma, exhibiting abducens nerve palsy and Horner syndrome during the medical treatment of the tumor, whose invasive cavernous sinus lesions could be detected. SUBJECTS: The patients were two men and one woman who were referred to our department with complaints of diplopia during the clinical course of nasopharyngeal carcinoma. RESULTS: Right abducens nerve palsy, Horner syndrome, and impairment of the 1st division of the trigeminal nerve were recognized in all patients. Magnetic resonance imaging (MRI) revealed invasive lesions in the rig…

Horner syndrome: a rare complication of internal jugular vein cannulation.

Percutaneous cannulation of the internal jugular vein is a widely used and accepted method for central venous cannulation, used for parenteral alimentation, fluid administration, and measurement of pressures. A multitude of complications associated with this procedure have been described. Horner syndrome as a rare complication of internal jugular vein cannulation is presented.

Raeder's Syndrome: An Uncommon Cause of Partial Horner Syndrome Secondary to Petromastoid Inflammation.

We report the case of a 58-year-old woman who initially presented with left-sided headache, facial discomfort, and a partial Horner syndrome affecting her left eye. Imaging revealed inflammatory changes within the petromastoid air cells, consistent with petrous apicitis. The clinical and radiological features were aligned with Raeder's syndrome, the rare cause of painful partial Horner syndrome. This case highlights the importance of early recognition of this uncommon but treatable medical condition.

[Benign superior vena cava syndrome associated with Horner syndrome--report of a case].

Superior vena cava syndrome is commonly caused by malignant diseases such as lung cancer, malignant lymphoma and so forth. It is uncommon that benign diseases obstruct the superior vena cava or its major tributaries. But there are some documented cases of benign superior vena cava syndrome induced by mediastinitis, benign mediastinal tumors, and uncommon lesions having their primary origin in the mediastinum. A 41-year-old male patient had recognized his ptotic palpebra two years ago and chest roentgenographic examination disclosed a tumor shadow occupying the apex of the right thorax but he w

Ophthalmic Vein Thrombosis, Abducens Nerve Palsy, and Horner Syndrome Secondary to Lemierre Syndrome.

Lemierre syndrome is a rare, life-threatening condition characterized by internal jugular vein thrombophlebitis and septic embolization. Ophthalmic complications are uncommon but clinically significant. This report describes a case of Lemierre syndrome presenting with ophthalmic vein thrombosis, abducens nerve palsy, and Horner syndrome. A 19-year-old man initially misdiagnosed with immune thrombocytopenic purpura developed neurological deficits, including aphasia and diplopia. Imaging revealed internal carotid artery dissection, cavernous sinus thrombosis, and superior ophthalmic vein occlusi…

[Horner's syndrome following epidural analgesia for labour].

Horner's syndrome is a rare complication of epidural analgesia, likely due to the cephalad spread of anaesthetics. We report a 30-year-old woman who developed Horner's syndrome 25 minutes after receiving an epidural, showing unilateral arm weakness, ptosis, miosis, and conjunctival hyperaemia. The symptoms resolved in 2.5 hours with no post-partum neurological deficits. While persistent symptoms may require further investigation, unnecessary imaging and treatment can be avoided. This case emphasizes the importance of recognizing this rare complication to obstetric anaesthesia.

Horner's syndrome in thyroidectomy patient: Case report.

A 30-year-old female patient with no previous chronic illness underwent hemithyroidectomy unusually complaint of ptosis on first postoperative day. On examination, she also had miosis. Thus, diagnosis of Horner's syndrome was made on ground of ruling out other complication. As it is caused by nerve insult, with a prolonged observation under close follow-up, the patient is fully recovered. Horner's syndrome after thyroidectomy is a rare entity. In textbook also, it is not mentioned as complication after thyroidectomy.

Segmental Zoster Paresis Accompanied by Horner's Syndrome.

We herein report a 90-year-old immunocompromised woman who developed right upper limb weakness and right ptosis with a miotic pupil 1 week after oral therapy for zoster on the right T2 dermatome. The right pupil was dilated with instillation of 1% apraclonidine, indicating Horner's syndrome. The patient was treated with intravenous acyclovir and methylprednisolone. Focal weakness related to zoster, generally known as segmental zoster paresis, improved over five months, but Horner's syndrome remained. We suggest that aggressive intravenous treatment should be considered for rare cases of zoster

Benign thyroid swelling presenting as Horner's syndrome.

Horner's syndrome is a rare neurological condition seen in association with the disruption in the sympathetic nerve supply. Thyroid swelling is a common condition but rarely causes cervical sympathetic chain compression. We describe a case of a 54-year-old man who presented with Horner's syndrome secondary to a benign thyroid nodule with pressure effect on the sympathetic chain. An association between thyroid pathologies and Horner's syndrome has been mentioned previously, however, to our knowledge, this is the first case of Horner's syndrome being the initial presentation for an underlying be

The role of apraclonidine in Horner’s syndrome – A case report.

INTRODUCTION: Horner’s syndrome is an interruption of the sympathetic nervous system at any point along its course between the hypothalamus and the orbit. Horner’s syndrome is classically presented as an ipsilateral miosis, subtle ptosis, and facial anhidrosis. Pharmacologic testing is very useful in the diagnosis of Horner’s syndrome as it could help to localize the lesioned neuron in the sympathetic pathway, suggesting an etiology. CASE OUTLINE: We present a case report of a 41-year-old woman who reported right eyelid drooping immediately after operation of sympathetic chain schwannoma. We

First Rib Fracture Resulting in Horner's Syndrome.

BACKGROUND: First rib fractures and traumatic Horner's syndrome are both quite rare, which can make it difficult to properly diagnose the combination of these 2 conditions in the emergency department. These conditions may be associated with severe medical emergencies, such as ongoing carotid dissection. CASE REPORT: We present the case of a 33-year-old man who sustained fractures to his right second, third, and fourth ribs and a delay in the diagnosis of left Horner's syndrome after he was involved in a traffic accident. Left Horner's syndrome was caused by a left transverse fracture of the f

Horner's syndrome in the prone position--a case report.

We report the case of a patient who developed a post-operative Homer's syndrome after non-cervical surgery. Part of the surgery was in the prone position. A hypothesis incriminating the prone position as being a possible cause of this syndrome is suggested.
