A plasmid-loaded biomimetic nanoplatform targeting the PAD2-METs-OPN axis against recurrent herpes simplex keratitis.

The high recurrence rate of herpes simplex keratitis (HSK) is primarily attributed to the reactivation of herpes simplex virus type 1 (HSV-1) within the trigeminal ganglion (TG), a latent viral reservoir cannot be eradicated by current antiviral or anti-inflammatory therapies. This study aims to elucidate the novel immune mechanisms regulating viral reactivation within the TG and to develop a targeted therapeutic strategy based on this mechanism. We found that macrophages are the key effector cells inhibiting viral reactivation in recurrent HSK models, exerting their core antiviral function th

Targeting host deoxycytidine kinase mitigates Staphylococcus aureus abscess formation.

Host-directed therapy (HDT) is an emerging approach to overcome antimicrobial resistance in pathogenic microorganisms. Specifically, HDT targets host-encoded factors required for pathogen replication and survival without interfering with microbial growth or metabolism, thereby eliminating the risk of resistance development. By applying HDT and a drug repurposing approach, we demonstrate that (R)-DI-87, a clinical-stage anticancer drug and potent inhibitor of mammalian deoxycytidine kinase (dCK), mitigates Staphylococcus aureus abscess formation in organ tissues upon invasive bloodstream infect…

Pulmonary toxoplasmosis.

With the advent of the human immunodeficiency virus (HIV) epidemic, the worldwide incidence of infections caused by Toxoplasma gondii has been rising. In this article I discuss the pathophysiology, diagnosis, and treatment of toxoplasma pneumonia. Basic research is now directed at the relationship of this organism to its host cells and how pharmacological or immunologic manipulation of that relationship may treat or prevent primary or recurrent infection. In addition to the standard diagnostic methods for T. gondii infection, newer methods using the tools of molecular genetics and immune compl

panTB-HM: a multi-arm clinical trial of a pan-TB regimen targeting both host and microbe.

BACKGROUND: Newer oxazolidinones will be required to advance regimens in pan-TB indications. The addition of host-directed agents may help promote the recovery of lung function during TB treatment and prevent post-TB lung disease. METHODS: The panTB-HM trial assesses the capacity of three novel regimens containing the oxazolidinone sutezolid and the antioxidant N-acetylcysteine (NAC) to meet the target criteria proposed by WHO for pan-TB indications (including use without rifamycin susceptibility testing and of 4 months or less duration) in a phase 2C trial. DISCUSSION: This trial is ground-

Emerging evidence for anti-PD-1 and IFN-γ as adjunctive immunotherapy in invasive mold infections.

UNLABELLED: Invasive mold diseases (IMDs) such as mucormycosis and aspergillosis carry high mortality despite optimal antifungal therapy. Adjunctive immunomodulation, including anti-PD-1 antibodies and interferon-γ, may help restore antifungal immunity in severely ill patients. We implemented multidisciplinary team meetings in France to assess and validate the use of adjunctive anti-PD-1 monoclonal antibodies and interferon-γ in combination with antifungal therapy for patients with life-threatening IMDs. Here, we report the characteristics and outcomes of the treated patients. Twelve cases wer

Cell death mechanisms during Mycobacterium tuberculosis infection: A perspective from a host-pathogen interface.

Tuberculosis, caused by Mycobacterium tuberculosis (Mtb), is a life threatening disease, which accounts for millions of lives annually. Mtb is an intracellular bacterium, has coevolved with humans to premeditate its machinery to surpass the immunity mounted against it in order to persist for long durations in the system. Cell death is a fundamental process required not only for tissue homeostasis but also for providing protection against intracellular pathogens. Various forms of cell death processes are known including apoptosis, pyroptosis, necroptosis, autophagy etc., that have been shown to

Innate immunity-mediated inflammation and immunopathogenesis in HSV-1-infected encephalitis.

HSV-1 encephalitis (HSE) remains a devastating viral infection of the central nervous system despite effective antiviral therapy. Increasing evidence indicates that disease outcome is not determined by viral replication alone but by how innate immunity is calibrated within the brain. Studies of host susceptibility in childhood HSE have identified brain-intrinsic antiviral mechanisms, referring to central nervous system (CNS)-localized innate immune pathways that enable resident brain cells to sense and restrict HSV-1 infection, particularly TLR-dependent interferon responses, neuron-associated

Host-mediated Leishmania donovani treatment using AR-12 encapsulated in acetalated dextran microparticles.

Leishmaniasis is a disease caused by parasites of Leishmania sp., which effects nearly 12 million people worldwide and is associated with treatment complications due to widespread parasite resistance toward pathogen-directed therapeutics. The current treatments for visceral leishmaniasis (VL), the systemic form of the disease, involve pathogen-mediated drugs and have long treatment regimens, increasing the risk of forming resistant strains. One way to limit emergence of resistant pathogens is through the use of host-mediated therapeutics. The host-mediated therapeutic AR-12, which is FDA IND-a

[Molecular mechanisms of chronic gastric mucosal inflammation induced by Helicobacter pylori via immune evasion].

Helicobacter pylori (H. pylori) persistently colonizes the gastric mucosa and induces sustained inflammation, making it a major driver of the development and progression of chronic gastritis, peptic ulcer disease, and gastric cancer. Its pathogenicity does not rely on direct tissue damage inflicted by a single virulence factor, but rather on the coordinated actions of multiple factors, including cytotoxin-associated gene A (CagA), vacuolating cytotoxin A (VacA), lipopolysaccharide, and urease that disrupt the gastric epithelial barrier and perturb mucosal immune homeostasis. Concurrently, H. p

Guanilated Quinolones with Dual Antitubercular and Anti-Inflammatory Activities.

Host-directed therapy (HDT) for tuberculosis (TB) generally involves the addition of adjuvant(s) to the standard TB regimen of at least four drugs with the hope of reducing treatment duration. Potential clinical challenges with the addition of adjuvant (s) to the TB regimen are increased pill load, which may lead to nonadherence, and consequently the precipitation of drug resistance; as well as the issue of drug-drug interactions. Molecules that can concurrently act as HDT and antitubercular agent thus have the potential to reduce TB treatment duration, and the pill burden, which may ultimatel…

An Artificial Peptide-Based Bifunctional HIV-1 Entry Inhibitor That Interferes with Viral Glycoprotein-41 Six-Helix Bundle Formation and Antagonizes CCR5 on the Host Cell Membrane.

Human immunodeficiency virus type 1 (HIV-1) is characterized by high variability and drug resistance. This has necessitated the development of antivirals with a new chemotype and therapy. We previously identified an artificial peptide with non-native protein sequence, AP3, with the potential to inhibit HIV-1 fusion through targeting hydrophobic grooves on the N-terminal heptad repeat trimer of viral glycoprotein gp41. Here, a small-molecule HIV-1 inhibitor targeting chemokine coreceptor CCR5 on the host cell was integrated into the AP3 peptide, producing a novel dual-target inhibitor with impr

Current and future therapies for triple-negative breast cancer.

Triple-negative breast cancer remains an aggressive and biologically heterogeneous breast cancer subtype. Although the therapeutic landscape has expanded, durable disease control remains clinically challenging in many settings. Existing reviews often organize TNBC therapy by drug class or molecular subtype, which can obscure how treatment response is shaped by interacting biological layers. Here, we review current and emerging therapeutic strategies through a three-layer framework: tumor-cell-intrinsic vulnerabilities, the local immune and stromal microenvironment, and host-level systemic modi

Shared and organism-specific host responses to childhood diarrheal diseases revealed by whole blood transcript profiling.

Globally, diarrheal diseases are a leading cause of death in children under five and disproportionately affect children in developing countries. Children who contract diarrheal diseases are rarely screened to identify the etiologic agent due to time and cost considerations associated with pathogen-specific screening and hence pathogen-directed therapy is uncommon. The development of biomarkers to rapidly identify underlying pathogens could improve treatment options and clinical outcomes in childhood diarrheal diseases. Here, we perform RNA sequencing on blood samples collected from children ev

Host-directed therapeutics against antimicrobial-resistant bacterial infections: emerging roles of multi-omics and systems biology.

The rapid emergence and global dissemination of antimicrobial resistance (AMR), coupled with the limited development of new antimicrobial agents, have intensified concerns regarding a major global public health crisis. Although antibiotic resistance is primarily driven by microbial mechanisms, host immune and inflammatory responses also play critical roles in determining infection outcomes, pathogen persistence, and treatment efficacy. In this context, host-directed therapies (HDTs) have emerged as a promising complementary strategy by modulating host pathways to enhance antimicrobial immunity

Oxidative stress drives potent bactericidal activity of pyrazinamide against Mycobacterium tuberculosis.

Pyrazinamide (PZA) is a critical component of tuberculosis first-line therapy due to its ability to kill both growing and non-replicating drug-tolerant populations of Mycobacterium tuberculosis within the host. Recent evidence indicates that PZA acts through disruption of coenzyme A synthesis under conditions that promote cellular stress. In contrast to its bactericidal action in vivo, PZA shows weak bacteriostatic activity against M. tuberculosis in axenic culture. While the basis for this striking difference between in vivo and in vitro PZA activity has yet to be resolved, recent studies hav

Immune checkpoint receptors: homeostatic regulators of immunity.

Alcoholic liver disease (ALD) is an escalating global problem accounting for more than 3 million deaths annually. Bacterial infections are diagnosed in 25-47% of hospitalized patients with cirrhosis and represent the most important trigger for acute decompensation, multi-organ failure, septic shock and death. Current guidelines recommend intensive antibiotic therapy, but this has led to the emergence of multi-drug resistant bacteria, which are associated with increased morbidity and mortality rates. As such, there is a pressing need to explore new paradigms for anti-infective therapy and host-
