Crusted Scabies leading to osteomyelitis and bilateral below-the-knee amputations - a case report.

Crusted scabies is a rare, highly contagious dermatologic condition associated with severe mite infestation, primarily affecting immunocompromised or socioeconomically vulnerable individuals. We report the case of a 38-year-old man with type two diabetes, neuropathy, and housing instability who was misdiagnosed with a hyperkeratotic skin disorder and subsequently developed bilateral foot osteomyelitis. Delayed recognition of crusted scabies led to progressive infection and ultimately necessitated bilateral below-the-knee amputations. Diagnosis was confirmed by bedside potassium hydroxide prepa…

Models of housing and support to reduce risks of COVID-19 infection and homelessness: the moving on pilot randomised controlled trial.

BACKGROUND: The UK government 'Everyone In' initiative in response to COVID-19 in England saw an unprecedented number of individuals experiencing homelessness moved into temporary accommodation (TA). A limited supply of settled housing meant swift access to settled accommodation (SA) would not be possible for all. This pilot RCT pursued a unique opportunity to examine the feasibility and acceptability of randomising people experiencing homelessness (PEH) to SA or TA and the impact on COVID-19 infection and housing instability. METHODS: A pilot RCT, with embedded process and health economic ev…

Less than One Meal Per Day: Food Insecurity and Deprivation Among Indigenous Oaxacan Farmworkers in California.

Food insecurity and lack of wellbeing among farmworker populations, particularly during the COVID-19 pandemic, are well documented. However, Indigenous migrant farmworker communities remain an understudied group, despite comprising over 40% of the current migrant farmworker labor workforce in California. This mixed-methods study assessed wellbeing and food insecurity among 40 Oaxacan Indigenous migrant farmworker households in the California Central Valley from 2022 to 2023. This study is the first of its kind utilizing the common Self-Reliance Index (SRI) among Indigenous migrant farmworkers.

Structural vulnerability as a conceptual framework for transgender health research: findings from a community needs assessment of transgender women of colour in Detroit.

The concept of structural vulnerability explains how systems of oppression drive health inequities by reducing access to survival resources (e.g. food, housing) for marginalised populations. Indicators of structural vulnerability such as housing instability, violent victimisation and poverty are often interconnected and result from intersectional oppression. We sought to demonstrate the utility of the structural vulnerability framework for transgender health research by examining patterns of structural vulnerability indicators among transgender women of colour in Detroit. We conducted latent c…

Understanding the impact of a residential housing programme for people living with HIV/AIDS: a realist evaluation protocol.

INTRODUCTION: Housing instability and homelessness are significant barriers to medical treatment for people living with HIV/AIDS. For these individuals, lack of stable housing and stigma is associated with insufficient access to care, poor adherence to medication and higher cost burdens to the healthcare system. This protocol reports on the efforts to evaluate Sanctum V.1.0, a hospice and transitional care home for adults with HIV/AIDS in Saskatoon, Saskatchewan, Canada. The current project was developed out of a need to identify how Sanctum V.1.0 produces varying programme outcomes to assist …

Use of digital health tools to promote equitable and efficient health-related social needs screening and response.

Unmet health-related social needs (HRSNs), such as food insecurity, housing instability, transportation barriers, and caregiver stress, are associated with increased healthcare utilization and poorer health outcomes. Screening for HRSNs and addressing unmet needs in pediatric healthcare is crucial to improving child health across the life course. As HRSN screening becomes more widespread, digital health tools offer an opportunity to expand equitable screening and resource referral programs. These technologies can improve workflow efficiency, adapt to different clinical settings, and facilitate…

Preventing Congenital Syphilis Through Prenatal Care: Qualitative Findings from a Pilot Clinical Program.

BACKGROUND: Congenital syphilis (CS) is highly preventable through prenatal screening and treatment. However, populations disproportionately impacted by prenatal syphilis often face barriers to access, in part due to housing instability and substance use. This study assessed Pregnancy Connections, a pilot program designed to prevent CS by offering comprehensive, patient-centered prenatal care, with added support and comprehensive case management services, for patients experiencing unstable housing and/or substance use. METHODS: This qualitative study involved interviews with 3 groups: program…

Identifying underlying constructs of childhood adversity in a low-income population.

BACKGROUND: Extensive research has documented the association between adverse childhood experiences (ACEs) and poor outcomes later in life, as well as the high prevalence of ACEs in the American population. Studies consistently find that over half of American adults have experienced at least one ACE. Despite this, research on the long-term impacts of ACEs is challenging due to the complex nature of adversity. OBJECTIVE: Our study aimed to define underlying constructs of adversity, and explore how they changed throughout childhood, in a low-income population. PARTICIPANTS AND SETTING: We fiel

State-Based Eviction Moratoria and Child Maltreatment During the COVID-19 Pandemic.

OBJECTIVES: The COVID-19 pandemic and associated economic crisis increased housing difficulties for families in the United States, putting children at increased risk of eviction and Child Protective Services involvement. For the first time at the national level, policymakers issued eviction moratoria with implementation approaches varying across states. This study examined whether state-based eviction moratoria were associated with changes in child maltreatment report rates. METHODS: This study used child maltreatment report data from 318 counties in 17 states from January 1, 2019 to mid-Augu

Risk of delivery complications among pregnant people experiencing housing insecurity.

BACKGROUND: Housing insecurity is increasingly being recognized as an important social determinant of health. Pregnant individuals experiencing housing insecurity may represent a particularly vulnerable subset of this population, but few studies have examined this population nationally. In particular, racial and ethnic minority individuals may be at risk for poor outcomes within this group because of structural racism and discrimination. The introduction of the International Classification of Diseases, Tenth Revision, Clinical Modification diagnosis codes related to social determinants of heal

Protocol: A mixed methods evaluation of an IPS program to increase employment and well-being for people with long-term experience of complex barriers in Vancouver's downtown and DTES.

BACKGROUND: Employment improves mental health and well-being by providing financial security, daily structure, a sense of identity and purpose, and social engagement. However, securing and sustaining employment is exceptionally challenging for vulnerable populations who experience persistent and multiple barriers, such as mental illness, homelessness, food and housing insecurity, and marginalization. Evidence-based supported employment programs, most notably individual placement and support (IPS) are becoming a more common approach for addressing the needs of these high-risk individuals. The a

Transitioning from Face to Face to the Digital Space: Best Practices and Lessons Learned Leveraging Technology to Conduct HIV-Focused Interventions.

Introduction: Coronavirus disease 2019 (COVID-19) has impacted researchers' ability to continue to deliver HIV prevention and treatment interventions face to face. Although telehealth has been an important strategy to maintain research operations during the current pandemic, participants at increased risk of or living with HIV are often at higher risk of also experiencing poverty, housing instability, and other challenges that may present obstacles to successful remote delivery. Methods: We provide descriptions of remote adaptations to two randomized controlled efficacy trials of behavioral in…

Treating homeless opioid dependent patients with buprenorphine in an office-based setting.

CONTEXT: Although office-based opioid treatment with buprenorphine (OBOT-B) has been successfully implemented in primary care settings in the US, its use has not been reported in homeless patients. OBJECTIVE: To characterize the feasibility of OBOT-B in homeless relative to housed patients. DESIGN: A retrospective record review examining treatment failure, drug use, utilization of substance abuse treatment services, and intensity of clinical support by a nurse care manager (NCM) among homeless and housed patients in an OBOT-B program between August 2003 and October 2004. COVID-19 Emergency Reforms in Massachusetts to Support Behavioral Health Care and Reduce Mortality of People With Serious Mental Illness.

People with serious mental illness are at disproportionate risk of COVID-19 morbidity and mortality because of high rates of risk factors that directly parallel those related to poor coronavirus outcomes, including smoking, chronic obstructive pulmonary disease, cardiovascular disease, and diabetes, along with housing instability, homelessness, food insecurity, and poverty. Community-based behavioral health organizations are also at risk of adverse outcomes because of dramatic declines in revenues and a diminished workforce. The State of Massachusetts has responded to this crisis by rapidly im…

A prospective 15-year follow-up study of kwashiorkor patients. Part II. Social circumstances, educational attainment and social adjustment.

Of 221 infants serially admitted to hospital with kwashiorkor and continuously observed thereafter, 116 were available for study after 15 years. Eighty-nine siblings formed the comparison group. This report is a final assessment of their social circumstances, attainment in education and adjustment to society. Economic circumstances had improved considerably, housing adequacy had deteriorated and family instability had become relatively less marked. Achievement in education resembled that of the population from which the sample was drawn, and there was no difference between that of ex-patients

Development and assessment of a natural language processing model to identify residential instability in electronic health records' unstructured data: a comparison of 3 integrated healthcare delivery systems.

OBJECTIVE: To evaluate whether a natural language processing (NLP) algorithm could be adapted to extract, with acceptable validity, markers of residential instability (ie, homelessness and housing insecurity) from electronic health records (EHRs) of 3 healthcare systems. MATERIALS AND METHODS: We included patients 18 years and older who received care at 1 of 3 healthcare systems from 2016 through 2020 and had at least 1 free-text note in the EHR during this period. We conducted the study independently; the NLP algorithm logic and method of validity assessment were identical across sites. The

Practice Assessment Tool for the Care of Patients With Borderline Personality Disorder.

Borderline personality disorder (BPD) is characterized by a long-term pattern of instability of interpersonal relationships, unstable self-image, marked impulsivity, and/or affective instability. In addition, these features can be evidenced by efforts to avoid real or feared abandonment, chronic feelings of emptiness, mood reactivity, recurrent self-injurious or suicidal behavior, other impulsive behaviors with potential for self-damaging effects, intense anger or difficulty with anger control, and transient paranoid ideation or stress-related dissociative symptoms. BPD is associated with subs

"We have to be uncomfortable and creative": Reflections on the impacts of the COVID-19 pandemic on overdose prevention, harm reduction & homelessness advocacy in Philadelphia.

The COVID-19 pandemic and ensuing service delivery interruptions had serious impacts on people who use drugs (PWUD) and people experiencing homelessness, including instability in the drug supply, decreased access to substance use disorder (SUD) treatment and harm reduction supplies, increased substance use and relapse due to stress and isolation, inability to properly isolate and quarantine without stable housing, and risk of COVID-19 spread in congregate living spaces. At the same time, many have noted a potential opportunity for rapid change in health, housing, and drug policy despite previo

Multidimensional Social Adversity and Mortality in People With HIV Infection and Heart Failure: Insights From NYC Health + Hospitals HIV-Heart Failure Cohort.

BACKGROUND: Heart failure is an increasingly common comorbidity among people with HIV infection, complicating care and heightening the vulnerability of this population to social adversity (SA). However, the impact of different SA domains on outcomes in this population remains poorly understood. METHODS: We analyzed data on people with HIV infection and heart failure from the NYC 4H (NYC Health + Hospitals HIV-Heart Failure) cohort. Baseline multidimensional SA was assessed by licensed clinical social workers using standardized evaluations and grouped into 5 domains: economic hardship, health
