Modified rush pin technique for two- or three-part proximal humeral fractures.

PURPOSE: To report the outcomes of modified Rush pin fixation for proximal humeral fractures. METHODS: 42 men and 20 women aged 19 to 94 (mean, 64) years with 2- or 3-part proximal humeral fractures underwent reduction and fixation using the modified Rush pin technique. 11 patients died from reasons unrelated to the surgery. RESULTS: Of 40 (out of 51) patients completing a subjective functional assessment using an Oxford Shoulder Questionnaire, 28 (70%) had 2-part and 10 (25%) had 3-part displaced fractures, and the remaining 2 (5%) had fracture-dislocations (one being 2-part and one 3-part)

Extended Paratricipital Approach for Intra-articular Fractures of the Distal Humerus.

This article describes an extensile surgical exposure to the distal humerus that is suitable for complex fractures involving the articular surface and extending into the humeral diaphysis proximal to the radial nerve. This method combines 2 approaches: olecranon osteotomy and the lateral paratricipital approach. This combination allows an appropriate exposure of both the articular surface and the humeral diaphysis up to the level of the deltoid tuberosity, while maintaining the extensor mechanism unharmed.

Total Humeral Endoprosthesis Replacement to Salvage Periprosthetic Fractures in Rheumatoid Arthritis.

The authors report their experience in the management of a 53-year-old woman with rheumatoid arthritis who presented with bilateral asynchronous traumatic periprosthetic fractures of the humerus after bilateral elbow replacements. One side was treated with a long-stem revision and internal fixation with bone graft, while the other side was treated with a long-stem distal humeral replacement. She sustained pathological periprosthetic fractures on top of the long-stemmed implants. Total humeral endoprosthesis replacements were performed bilaterally as salvage procedures to provide a stable platf

The "Bag of Bones" Treatment of Comminuted Intra-articular Distal Humerus Fractures in the Elderly.

BACKGROUND: Extensively comminuted intra-articular distal humerus fractures in the elderly present a challenging therapeutic dilemma. The purpose of this study was to investigate the results of nonoperative treatment of these fractures in a select subset of patients. METHODS: Patients treated with nonoperative management for a comminuted intra-articular distal humerus fracture between 2007 and 2018 were reviewed. Patients were administered 3 elbow-specific functional outcomes instruments. RESULTS: A total of 8 patients (2 men, 6 women) were treated with brief immobilization followed by early

Long bone asymmetry and hemihypertrophy as a skeletal marker of child abuse.

Diaphyseal overgrowth is a well-documented complication of long bone fracture in orthopedic studies, but it is not a condition commonly mentioned in the forensic literature as a possible indicator of child abuse. Here we present an occurrence of humeral hypertrophy associated with a physeal fracture to the left distal humerus in a case of infant child abuse. Humeral overgrowth was present in this infant along with superficial bruising, other humeral fractures, rib fractures, and vertebral injuries consistent with battered child syndrome. In 2002, the humeri, lower ribs, and vertebrae of this i

Treatment of humeral shaft delayed unions and nonunions with humeral locked nails.

OBJECTIVE: To report experience with use of humeral locked nails in treating humeral delayed unions and nonunions. The following techniques yielded encouragingly good results: static locking, short-to-long segment nailing, bone grafting, fracture compression, and minimal surgical trauma. DESIGN AND METHODS: A total of 41 consecutive patients with 13 delayed unions and 28 nonunions were treated with humeral locked nails. Delay from trauma to surgery averaged 4.2 months for delayed union and 15.5 months for nonunion. The average age of patients was 50.2 years; average follow-up time was 23.2 mo

Minimally Invasive Plate Osteosynthesis of Humeral Shaft Fractures: Current State of the Art.

Most closed humeral shaft fractures can be successfully managed nonsurgically. However, fractures for which closed treatment is unsuccessful are stabilized using either plates or intramedullary nails. There are shortcomings associated with each technique, including the potential complications of nonunion, infection, shoulder pain, and radial nerve injury. Minimally invasive plate osteosynthesis (MIPO), an innovative alternative treatment, is gaining in popularity. This technique is based on the anterior humeral shaft providing a relatively safe surface for plate application, and limited open e

Dual-time points (18)F-FDG PET/CT imaging in detection of bone metastases from hepatocellular carcinoma.

Bone scan is a well-established imaging modality in detection of osseous metastasis. However, its sensitivity decreases at evaluating lytic osseous metastasis in such entity as hepatocellular carcinoma (HCC). Increasingly, Fluorine-18 fluorodeoxyglucose positron emission tomography ((18)F-FDG PET) with dual-time points imaging is utilized in detection of osseous metastasis from hepatocellular carcinoma. We report a case of HCC presenting with a focal increased activity on bone scan corresponding to left distal humeral fracture seen on an X-ray. Subsequent dual-time points (18)F FDG PET/CT scan

Retrospective analysis of extra-articular distal humerus shaft fractures treated with the use of pre-contoured lateral column metaphyseal LCP by triceps-sparing posterolateral approach.

Management of extra-articular distal humerus fractures presents a challenge to the treating surgeon due to the complex anatomy of the distal part of the humerus and complicated fracture morphology. Although surgical treatment has shown to provide a more stable reduction and alignment and predictable return to function, it has been associated with complications like iatrogenic radial nerve palsy, infection, non-union and Implant failure. We in the present series retrospectively analysed 20 patients with extra-articular distal humerus shaft fractures surgically treated using the extra-articular

[Distal humerus fracture-extensile approaches].

The majority of dislocated, intra-articular fractures are treated with an open reduction and internal fixation. In this paper we describe a variety of dorsal approaches to the distal humerus. Beside the dorsal approach through an olecranon osteotomy we also discuss the alternative dorsal approaches without osteotomy and their advantages and drawbacks. Moreover we discuss the preoperative planning and operative procedure. Early functional rehabilitation, without weight bearing, is important to achieve an optimal outcome. Finally we present the results of the last 6 years of patients treated ope

Comparison of efficacy between Kirschner wire and cannulated screw internal fixation after pediatric supracondylar humeral fracture osteotomy.

BACKGROUND: In post-traumatic cubitus varus osteotomy, conventional internal fixation methods typically involve the use of Kirschner wires (K-wires). Nevertheless, with advancements in medical technology and a growing demand for robust internal fixation, surgeons are increasingly opting for cannulated screws. Despite this trend, controversy remains regarding whether cannulated screws are superior to K-wires, particularly in pediatric patients with open epiphyses. This study aimed to compare the clinical treatment outcomes and complications of K-wires and cannulated screws as internal fixation

Cortical bone thickness predicts the quantitative bone mineral density of the proximal humerus.

UNLABELLED: Cortical thickness determined at the humerus can serve as an easy and reliable screening tool to predict the local bone status when quantitative bone mineral density (BMD) measurements are not available. It can therefore serve as a rapid screening tool in fragility fractures to identify patients requiring further diagnostic or osteoporosis treatment. INTRODUCTION: Quantitative bone mineral density (BMD) of the humerus is difficult to determine but relevant for osteoporosis and fracture treatment. Dual-energy X-ray absorptiometry (DXA) of the femur and lumbar spine overestimates th

Humeral shaft fractures: union outcomes in a large cohort.

BACKGROUND: Humeral shaft fractures can be managed conservatively or operatively. Fracture characteristics were analyzed to identify patients who would benefit from early operative fixation. METHODS: We performed a retrospective cohort study of 126 consecutive humeral shaft fractures (2008-2015). Fractures were classified according to fracture type, location, separation, and comminution. RESULTS: Of 126 patients, 96 were managed conservatively. In 54%, union occurred before 26 weeks, and 13% had delayed union after 26 weeks, whereas 33% did not achieve union. Of 30 patients managed surgicall

Increasing complexity and plateauing outcomes in AO type C distal humerus fractures: a 50-year institutional case series.

BACKGROUND: Distal humerus fractures in adults are rare but complex injuries, particularly AO type C3 fractures, often involving comminution, instability, and osteoporotic bone. Demographic trends indicate rising incidence, especially among elderly patients, with increasing functional demands challenging modern osteosynthesis. Open reduction and internal fixation (ORIF) with bicolumnar plating remains the preferred reconstructive approach, although complications remain substantial. This study aimed to evaluate trends in fracture complexity, patient demographics, and functional outcomes over fi
