Idiopathic normal pressure hydrocephalus (NPH) remains a mysterious entity, with several gaps in our understanding, despite intensive research. The current theory of its etiology is diminished vascular compliance with redistribution of vascular pulsations. The manifestations of NPH can be explained by the compression of parenchyma and vessels and the accumulation of toxic metabolites in the cerebrospinal fluid (CSF) and in the brain parenchyma. The author believes that NPH is caused by repetitive disruption of the normal propagation of ventricular systole, due to chronic derangements of cerebr

To clarify the relation between the drainage pathway of cerebrospinal fluid and the development of idiopathic normal pressure hydrocephalus (iNPH), we examined flow patterns of internal jugular veins in 20 patients with iNPH and 13 control patients using air-contrast ultrasound venography during the Valsalva maneuver. The iNPH group had a significantly greater frequency of retrograde jugular venous flow (19/20, 95%) than the control group (3/13, 23%) (chi(2) test, p < 0.001). Our results suggest that retrograde jugular venous flow may be associated with the development of iNPH; therefore, the

The origin of gait disturbance in normal pressure hydrocephalus (NPH) is poorly understood. We assessed the frequency of frontal and sub-cortical hypokinetic gait clinical features and changes in gait variables after lumbar puncture (LP), in 15 patients with idiopathic NPH. Hypokinesia and disequilibrium were the main features, suggesting dysfunction of the basal ganglia (BG) and the frontal lobe (FL), respectively. Disequilibrium responded worse to LP and frontal signs were more frequent in patients that did not improve after LP. This could be related to earlier disturbance of frontal gait co

Spinocerebellar ataxia type 6 (SCA6) is a rare disease that is characterized by cerebellar dysfunction. Patients have progressive motor coordination impairment, and postmortem brain tissue reveals degeneration of cerebellar Purkinje cells and a reduced level of cerebellar brain-derived neurotrophic factor (BDNF). However, the pathophysiological changes underlying SCA6 are not fully understood. We carried out RNA-sequencing of cerebellar vermis tissue in a mouse model of SCA6, which revealed widespread dysregulation of genes associated with the endo-lysosomal system. Since disruption to endosom

OBJECTIVE: Evaluate long-term changes in cognition after ventriculoperitoneal shunt insertion in patients with idiopathic normal pressure hydrocephalus.
BACKGROUND: Although studies have indicated that treatment of normal pressure hydrocephalus with ventriculoperitoneal shunt improves gait, effects of treatment on cognition have been inconsistent. Several studies have reported no significant improvement on neuropsychological tests in patients with idiopathic normal pressure hydrocephalus. Selection of candidates for shunt surgery using a trial of controlled spinal fluid drainage via lumbar ca

Normal pressure hydrocephalus (NPH) is associated with periventricular white matter lesions and demyelination. The aim of the present study was to examine the cerebrospinal fluid (CSF) levels of tumor necrosis factor-alpha (TNF-alpha), a proinflammatory cytokine mediating myelin damage, in patients with NPH. TNF-alpha levels were analyzed by ELISA and measured before and after shunt operation in 35 patients with NPH. The levels of this cytokine were related to the symptomatology and to magnetic resonance imaging (MRI) verified white matter lesions. They were also related to intrathecal levels

Normal pressure hydrocephalus (NPH) is characterized by insidious onset and gradual development of the triad of gait disturbance, dementia, and urinary incontinence. Nausea, vomiting, and signs of increased intracranial pressure do not occur. A 71-year-old male patient was scheduled for total knee replacement due to osteoarthritis of right knee joint. No neurological symptoms and signs except mild forgetfulness were detected during physical examination following admission. Due to operational mistakes, the anesthesiologist was informed that the surgery was cancelled just after completion of ind

Symptoms of overdrainage, a frequent complication following shunting procedures especially in patients with normal-pressure hydrocephalus (NPH), are presumed to be caused by siphoning: gravity-enhanced flow of cerebro spinal fluid (CSF) through shunts in upright position. A critical reappraisal of siphoning and overdrainage (OD) is presented considering the occurrence of subdural hygromas (SDH) encountered in a group of patients shunted for NPH. The patients participated in the multicentre Dutch NPH study: 101 NPH patients were randomised for treatment with shunts with low or medium-high worki

Twenty-five patients with normal pressure hydrocephalus (NPH) and 16 patients with cerebral atrophy were investigated by quantitative cisternography using anterior images of the head 3, 6 and 24 h after a lumbar injection of 200 MBq 99mTc DTPA (diethylene-triamine-penta-acetic acid). The activity in the lateral ventricles, basal cisterns, hemispheric convexities and total intracranial region was measured. Ratios between these regions/quantities were calculated. Fifteen of the NPH patients improved while 10 were unchanged after ventriculo-peritoneal shunt surgery. The ratio between ventricular

A total of 21 patients with normal-pressure hydrocephalus were examined. Cerebrospinal fluid (CSF) was collected before and after operation with a ventriculoperitoneal shunt. A slight plasma-like protein pattern indicating a blood-brain barrier (BBB) dysfunction was seen in 38% of the patients before operation. No characteristic changes could be found in the "CSF-specific" protein fraction. After the shunt operation 65% of the patients had a BBB dysfunction to macromolecules and 70% had two additional acidic protein bands in the CSF-specific fraction. Ventricular CSF protein content was 73% of

In 1973 we reported results of ventricular shunting in 28 patients with idiopathic normal pressure hydrocephalus. The present report consists of a three year follow-up of the latter (series 1), and an additional 45 patients (series 2) are presented for further insight into the conclusions drawn from the original experiences.

The aim of this study was to explore how the lumbar cerebrospinal fluid pressure (CSFP) waves recorded during lumbar infusion compared with the intracranial pressure (ICP) waves recorded, either during lumbar infusion or during long-term, overnight monitoring. For this purpose, we assessed 27 simultaneous lumbar CSFP/ICP recordings made during lumbar infusion and 27 long-term, overnight ICP recordings in 27 consecutive idiopathic normal pressure hydrocephalus (iNPH) patients. Pressure waves during lumbar infusion were explored by computing pulse pressure amplitude and mean single wave pressure
