Serum concentration of 1 alpha,25-dihydroxyvitamin D [1,25(OH)2D] and intestinal absorption were measured before and after parathyroidectomy in 11 patients with primary hyperparathyroidism. Serum 1,25(OH)2D was high preoperatively (P less than 0.001) and normal postoperatively. Ca absorption was elevated preoperatively (P less than 0.001) and decreased significantly after parathyroidectomy (P less than 0.001). In normal volunteers infusion of parathyroid extract raised plasma 1,25-dihydroxyvitamin D (1,25-[OH]2D) levels by 80% (peak 8--40 h after infusion) without producing any significant change in plasma calcium, phosphate, or magnesium. An oral calcium load suppressed plasma 1,25-(OH)2D to 63% of control values within 48--72 h. These responses will be useful in the diagnosis and management of disturbances of calcium and mineral metabolism in which vitamin D metabolism is abnormal.

The response of serum 1,25-dihydroxycholecalciferol (1,25-OH2CC) concentration to the administration of parathyroid extract (PTE) was studied in a patient with pseudohypoparathyroidism (PHP) type 1, 3 days after withdrawal of dihydrotachysterol (DHT) treatment. The patient had had a normal serum calcium (Ca), phosphorus (P) and immunoreactive parathyroid hormone (iPTH) level on DHT for 6 years. After PTE administration no rise of the 1,25-OH2CC concentration and no response of urinary cAMP and P were seen.

Serum levels of calcium, phosphorus, alkaline phosphatase, and 25 hydroxy-vitamin D (25-OH-D3) were measured in normal and malnourished children with and without rickets.
