Cervical diffuse skeletal hyperostosis as a rare cause of dysphagia: Two surgical cases.

BACKGROUND: Cervical diffuse idiopathic skeletal hyperostosis (DISH), also known as Forestier's disease, is an uncommon but potentially reversible cause of dysphagia due to mechanical compression of the hypopharynx or upper esophagus. CASE DESCRIPTION: Two male patients presented with progressive dysphagia caused by anterior cervical DISH. Radiological evaluation demonstrated extensive anterior osteophyte formation resulting in significant hypopharyngeal and esophageal compression. Both patients underwent anterior cervical osteophytectomy, which led to immediate and complete resolution of dys…

Fundus albipunctatus-like retinopathy in hypovitaminosis A-related optic neuropathy with skull hyperostosis.

A young man presented with progressive night blindness, optic neuropathy and bilateral retinal flecks, initially suggestive of an inherited retinal dystrophy. Genetic testing identified a heterozygous pathogenic variant in USH2A alongside multiple variants of uncertain significance, none of which fully explained the phenotype. Further investigation revealed severe vitamin A deficiency and cranial hyperostosis with narrowing of the optic canals. Retinal findings improved following vitamin A supplementation. This case illustrates the diagnostic complexity of retinal dystrophies, emphasises the n…

Neuro-ophthalmic complications of endosteal hyperostosis, Worth type: the importance of ophthalmic monitoring.

Disorders of bone formation or resorption affecting the cranium can lead to ophthalmic complications. We describe a family with Worth endosteal hyperostosis (WEH), an autosomal dominant condition characterized by mandibular overgrowth and cortical thickening of long bones. Although traditionally considered benign, we report significant neuro-ophthalmic complications in this kindred. This retrospective case series examines a two-generation family of three siblings and their mother, all with clinical features of WEH and a confirmed lipoprotein-related protein 5 (LRP5) gene mutation. A literature …

Thoracolumbar Spine Trauma Outcomes: Effect of Ankylosing Spondylitis and Diffuse Idiopathic Skeletal Hyperostosis.

INTRODUCTION: Ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH) are chronic conditions associated with altered spine architecture that can potentially influence the outcomes of spine trauma. This study aims to compare demographic characteristics, clinical outcomes, and surgical interventions among thoracolumbar spine trauma patients with a concomitant diagnosis of AS or DISH with those with no underlying ankylosing spinal conditions. Our aim is to better understand how these diseases may affect the prognosis and treatment in the context of thoracolumbar spine trau…

Progressive dysphagia secondary to cervical osteophytosis due to diffuse idiopathic skeletal hyperostosis (DISH).

Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory systemic disease characterised by ossification of ligaments and entheses. Although it is usually asymptomatic, in its cervical localisation it can cause dysphagia secondary to oesophageal compression or functional limitation due to ankylosis. We present the case of a 58-year-old male with progressive dysphagia of three years' evolution, with an EAT-10 score of 34/40. Surgical resection of the impressive osteophyte formation was performed by an anterior cervical spine approach and precision drilling. Within a few weeks, there…

Atypical vertebral body stress fracture in the setting of diffuse idiopathic skeletal hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory arthritis characterized by bridging ossification along the anterolateral spine. Bridging osteophytes in DISH can result in fusion, altering the biomechanics and increasing the risk of unstable spinal fractures. This report details two cases of unusual vertebral body stress fractures in the setting of DISH: a T6 fracture in a 76-year-old male presenting without definite precedent trauma, and an L2 fracture in an 82-year-old female associated with 1 month of prodromal lower back pain radiating to the groin. CT and MR imaging f…

Pneumothorax by Penetrating Endplate Screw for Diffuse Idiopathic Skeletal Hyperostosis-Related Thoracolumbar Fracture.

Pneumothorax is a rare surgical complication in spinal surgery with thoracic pedicle screws. The penetrating endplate screw (PES) technique has been developed as a strong alternative spinal anchor to conventional pedicle screws for diffuse idiopathic skeletal hyperostosis (DISH). We present an intraoperative pneumothorax without deviation to the thoracic during the maneuver of the PES. A 56-year-old male who presented with non-union of DISH-related T12 vertebral fracture underwent T12 kyphoplasty and T10-L2 posterior fixation using the PES technique. The left pneumothorax was developed postope…

Young thoracic vertebra diffuse idiopathic skeletal hyperostosis with Scheuermann disease: A case report.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a disorder characterised by the calcification and ossification of ligaments and entheses. It is a frequent occurrence in elderly males, but rarely encountered in younger individuals. CASE SUMMARY: A 24-year-old male was admitted to the hospital due to low back pain accompanied with numbness in both lower limbs for 10 d. Upon clinical examination and imaging tests, the patient was diagnosed with DISH with Scheuermann disease and thoracic spinal stenosis. Before the operation and medical treatment, the patient had hypoesthesia of th…

Dysphagia revealing diffuse idiopathic skeletal hyperostosis: report of two cases and literature review.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) also known as Forestier's disease, is a musculoskeletal disorder characterized by the calcification of ligaments essentially the vertebral longitudinal anterior ligament. Men are generally affected. It is often asymptomatic. The most common extra-spinal clinical manifestation of this disease presents as dysphagia followed by respiratory disturbances such as dyspnea and sleep apnea. In this paper we discuss two cases where the patients have experienced progressive dysphagia. Radiological findings were compatible with DISH. The management was based…

Outcomes of lumbar decompression surgery in patients with diffuse idiopathic skeletal hyperostosis (DISH).

BACKGROUND: Only a few studies have described the effect of diffuse idiopathic skeletal hyperostosis (DISH) on the clinical results after lumbar surgery. The aim of the study is to clarify the associations between DISH and the clinical results after lumbar decompression surgery. METHODS: The outcomes of 328 consecutive patients who underwent primary lumbar decompression surgery for treatment of lumbar canal stenosis with or without grade I spondylolisthesis were analysed retrospectively. The major outcome measures were surgery-free survival and the need for further surgery because of same-seg…

Spinal fractures in diffuse idiopathic skeletal hyperostosis: Advantages of percutaneous pedicle screw fixation.

PURPOSE: To validate the effectiveness of percutaneous pedicle screw (PPS) fixation for spinal fractures associated with diffuse idiopathic skeletal hyperostosis (DISH) by comparing surgical outcomes for PPS fixation and conventional open posterior fixation. Patients with DISH are vulnerable to unstable spinal fractures caused by trivial trauma, and these fractures have high rates of delayed paralysis, postoperative complications, and mortality. METHODS: This retrospective study assessed surgical outcomes for 16 patients with DISH (12 men; mean age 76.1 ± 9.4 years) who underwent PPS fixation…

Simultaneous occurrence of ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis: a systematic review.

OBJECTIVES: AS and DISH are both spinal ankylosing conditions with a 4-fold increased risk of spinal fractures. The most commonly used criteria for DISH were designed to exclude radiographic signs of spondyloarthritis. However, case reports describing the presence of both conditions exist. In this study, the co-occurrence of AS and DISH were reviewed in the literature to explore the potential need to revise the criteria for DISH. METHODS: A search was conducted in Pubmed, Embase, Web of Science and the Cochrane library using the terms 'spondyloarthritis' and 'DISH' and their matching synonyms

Spondylosis deformans and diffuse idiopathic skeletal hyperostosis (dish) resulting in adjacent segment disease.

Spondylosis deformans and diffuse idiopathic skeletal hyperostosis (DISH) are usually incidental findings and in most dogs are either asymptomatic or associated with mild clinical signs. Severe spondylosis deformans and DISH can result in complete bony fusion of consecutive vertebral segments. One of the recognised complications following vertebral fusion in human patients is the development of adjacent segment disease, which is defined as degenerative changes, most commonly degenerative intervertebral disc disease, in the mobile vertebral segment neighboring a region of complete vertebral fus…

Wiskott-Aldrich syndrome presenting with early onset recurrent acute hemorrhagic edema and hyperostosis.

We report two unusual presenting manifestations of Wiskott-Aldrich syndrome (WAS), recurrent acute hemorrhagic edema of infancy (AHEI); a form of cutaneous vasculitis and hyperostosis of the tibia. Though cutaneous vasculitis is known to occur in WAS, presentation in early infancy and as AHEI is extremely uncommon. Hyperostosis is not a well-recognized association in WAS; only three patients with this association have been previously reported. In our patient these two unusual manifestations preceded the onset of recurrent infections. Recognition of this rare presentation led us to an early dia…
