Diagnosing hypersensitivity reactions to cephalosporins in children.

OBJECTIVES: The goals were to evaluate the usefulness of skin tests, patch tests, serum specific IgE assays, and challenges in diagnosing hypersensitivity reactions to cephalosporins and to clarify the pathogenic mechanism of such reactions. METHODS: Children with immediate manifestations (within 1 hour) underwent immediate-reading skin tests with penicillin reagents and any suspect cephalosporins, serum specific IgE assays, and challenges; some children underwent reevaluations. Children with nonimmediate manifestations (after >1 hour) were assessed with patch tests, delayed-reading skin test

Evaluation of diagnostic approach and clinical outcomes in children with suspected beta-lactam allergy.

BACKGROUND: Beta-lactams (BLs) are commonly implicated in hypersensitivity reactions among children. However, majority of self-reported BL allergies may be viral exanthems and antibiotics side effects. International guidelines have recommended de-labelling BL allergies in children with delayed mild hypersensitivity reactions via direct drug provocation tests (DPTs) without prior skin testing. However, studies on the safety of direct DPTs in Asia remain limited. OBJECTIVE: Our study aims to describe the diagnostic approach and clinical outcomes in children with suspected BL allergies and evalu

Anaphylaxis to the 23-valent pneumococcal vaccine: a second explored case by means of immediate-reading skin tests with pneumococcal vaccines.

Anaphylaxis to pneumococcal vaccines is rare. In the only one child with anaphylaxis to a first injection of the 23-valent pneumococcal vaccine that has been explored, skin tests and specific IgE determination diagnosed immediate-type hypersensitivity to pneumococcal antigens. We report the case of a child who tolerated three injections of the 7-valent pneumococcal vaccine, but experienced anaphylaxis to a fourth injection of the 23-valent vaccine. Immediate responses in skin tests diagnosed immediate-type hypersensitivity to the two vaccines. Immunizations with the 7-valent pneumococcal vacci

Quinolone hypersensitivity.

PURPOSE OF REVIEW: Quinolones are potent antibacterial agents that can cause drug hypersensitivity reactions affecting different organs. A better understanding of the underlying mechanism and the level of crossreactivity within different quinolones is needed to handle and prevent these diseases. RECENT FINDINGS: The adverse side-effects caused by quinolones are the result of different immunological mechanisms and cause quite different diseases. The development of an assay detecting quinolone-specific IgE revealed specific antibodies in more than 50% of patients with immediate-type reactions,

[Allergic reactions to local anesthetics].

In this article type I and type IV allergic reactions evoked by local anaesthetics are discussed. Contact dermatitis (type IV) reactions are mainly caused by local anaesthetics of the ester-type like procaine. Both contact dermatitis and anaphylactic reactions (type I) can be caused by anaesthetics of the amide-type (e.g. lidocaine), but these reactions are rare. Intra- and epicutaneous tests can be useful to prove allergy, but sometimes provocation tests are necessary. Many reactions are not caused by an allergic mechanism.

The hypersensitivity response to the adult worm proteinase, SMw32, in Schistosoma mansoni infected mice.

A cysteine proteinase (SMw32) from the digestive tract of an adult Schistosoma mansoni worm has previously been purified and characterized. During the course of infection with this parasite, a strong immune response to the enzyme occurs. We now have confirmed the presence of anti-proteinase IgE and IgG in S. mansoni infected mice and have investigated the in vivo cellular response to proteinase in infected and uninfected mice. Immediate and delayed type hypersensitivities were detected in uninfected mice sensitized by multiple injections of proteinase. In S. mansoni infected mice, immediate hy

Paracetamol Allergy: A Case of a 9-Year-Old Female with a History of Atopy.

Background: We describe a rare case involving paracetamol, a commonly used drug worldwide. Increased paracetamol consumption increases the risk of adverse drug reactions. Case Presentation: This is a case of a 9-year-old girl who visited the emergency department due to sudden onset sneezing, nasal itching, urticaria, and angioedema after paracetamol ingestion. The diagnostic and etiologic studies revealed an immunoglobulin E (IgE)-mediated hypersensitivity mechanism to paracetamol. Conclusion: Few cases of this phenomenon have been reported in previous literature. As confirmed in this study, a

[Atopy and drug allergy].

UNLABELLED: The aim of our study is to show possible correlation between atopy and drug allergy, and the clinical manifestations of drug allergy that may have predictive value. There were 150 patients with positive history of drug allergy, ages 18-73 years. Cutaneous anr/or dose provocative testing was done, depending on the drug. Drug allergy was found in 58 patients (40%). There were 18 patients (12%) with atopic diathesis of which 9 (50%) were found to have drug allergy. Of 17 patients diagnosed as anaphylaxis, 13 (76%) had drug allergy, penicillin being the most common offending drug (7 pa

[The reaction to trichophytin in dermatophytoses].

The authors investigated the specific immunological competence of 31 patients with dermatophytosis using tricophytin antigen. Among them, 54.8% showed reaction to the delay phase (48 h) in the following proportions: tinea inguinale, 75%; tinea pedis, 61.5%; tinea unguium, 50% and tinea corporis, 20%. Other 62.5% showed positive result to the early phase (30 m). The association between these reactions revealed that, although the majority of cases with early positive reaction showed negativity to the delayed reaction, 20.8% presented positively to both phases of the reaction. Out of the non-reac

Occupational dermatitis with simultaneous immediate and delayed allergy to chrysanthemum.

A 42-year-old woman had recurrent dermatitis on the hands, arms, face and trunk for 5 years. She was allergic to chrysanthemum, with positive patch tests. She also had contact urticaria to chrysanthemum, with a wheal-and-flare reaction 15 min after application of chrysanthemum juice to a scratch test site. This was not seen in control subjects. The co-existence of allergic contact dermatitis and contact urticaria from chrysanthemum might play a rôle in the development of dermatitis, especially severe chronic dermatitis of her hands.

Skin-test reactivity to egg protein--exposure by inhalation compared with ingestion.

Of two hundred and sixty-seven subjects exposed to egg by ingestion only, forty-eight were skin (prick)-test positive to egg material. All forty-eight subjects also reacted to other allergens on skin testing, forty-seven (98%) to allergens associated with respiratory allergy. Of thirteen subjects exposed to egg by inhalation in their workplace (and ingestion) two were skin-test-positive to egg, both also reacting to respirable allergens. Thus, inhalation of egg antigen does not greatly influence skin-test reactivity to eggs as determined by comparing the ratio of egg skin-test-positives to tot

Failure to induce hypersensitivity reactions to opaque contrast media analogs in guinea pigs.

In a further exploration of idiosyncratic reactions to injections of contrast media, a systematic study of immediate or delayed hypersensitivity in guinea pigs injected with contrast media analogs was performed. No evidence of immediate or delayed hypersensitivity could be ascertained to either contrast media analogs alone, or analogs bound to protein carriers. This does not represent conclusive evidence against the notion of antibody formation as a factor in the production of some adverse reactions, but makes such a pathogenesis less likely.

IgE mediated hypersensitivity to pancreatic extract (PE) in parents of cystic fibrosis (CF) children.

On exposure to pancreatic extract (PE), four parents of cystic fibrosis (CF) children developed immediate hypersensitivity-like symptoms: i.e. rhinoconjunctivitis, asthma, and/or anaphylaxis. IgE to PE was demonstrated in the subjects by skin testing, leucocyte histamine release and radioallergosorbent test (RAST). No serum precipitating antibodies were found. Bronchial challenge caused an immediate asthmatic response. No delayed asthmatic response or hypersensitivity pneumonitis-like reaction occurred. The responsible antigen for PE induced asthma is unknown; trypsin failed to inhibit PE-RAST

Enhanced IL-4 but normal interferon-gamma production in children with isolated IgE mediated food hypersensitivity.

Atopic disorders such as atopic dermatitis and asthma have been characterised by an imbalance in interferon-gamma (INF-gamma) and IL-4. Whether similar imbalances are found in atopic disorders with different clinical manifestations, such as IgE mediated immediate food hypersensitivity, is not clear. We have examined the in vitro production of INF-gamma and IL-4 in peripheral blood mononuclear cells (PBMC) following phytohaemagglutinin stimulation from children with isolated immediate IgE mediated food hypersensitivity (egg, milk, "nut"), children with moderate and severe atopic dermatitis, and

Cutaneous adverse reactions after intra-articular injection of triamcinolone acetonide.

A patient is described with a disseminated morbilliform and partially persistent urticarial dermatitis following intra-articular injections of triamcinolone acetonide. A delayed-type hypersensitivity to triamcinolone acetonide was observed after patch and intradermal testing. However, an immediate-type hypersensitivity to this drug was not observed. A delayed-type sensitization to betamethasone, dexamethasone and prednisolone, but not to hydrocortisone was also observed after patch testing. Intradermal tests with these representatives of corticosteroids were all negative. Although little is kn
