Risk factors of the low anterior resection syndrome (LARS) after ileostomy reversal in rectal cancer patient.

This study is aimed at identifying risk factors of Low Anterior Resection Syndrome following ileostomy reversal in rectal cancer patients who had undergone preventive ileostomy. This retrospective analysis was conducted on a cohort of 605 patients treated at the Fourth Hospital of Hebei Medical University between January 2018 and December 2021. These patients were grouped based on LARS score, and Clinical and follow-up data were collected to conduct univariate analyses of potential factors influencing LARS occurrence based on variable type. Variables with statistical significance were included

ILEOSTIM trial: a study protocol to evaluate the effectiveness of efferent loop stimulation before ileostomy reversal.

PURPOSE: A protective loop ileostomy is the most useful method to reduce sequelae in the event of an anastomotic leakage (AL) after rectal cancer surgery. However, it requires an additional stoma reversal surgery with its own potential complications. Postoperative ileus (POI) remains the most common complication after ileostomy reversal, which leads to an increase in morbidity, length of hospital stay (LOS) and overall healthcare costs. Several retrospective studies carried out in this field have concluded that there are insufficient evidence-based recommendations about the routine application

Early Postoperative Complications and Surgical Anatomy After Ileostomy Reversal Among the Population of Khyber Pakhtunkhwa, Pakistan.

Objective To assess the frequency of early postoperative complications and surgical anatomy after ileostomy reversal among the population of Khyber Pakhtunkhwa, Pakistan. Materials and methods In the current study, a total of 241 patients were assessed. Sufficient urine output, usual serum electrolytes and urea were indicators of a sufficient recovery. All patients remained in the ward for a minimum of seven days after surgery to detect early postoperative complications like surgical site infection (SSI), wound dehiscence, small bowel obstruction, and anastomotic leak. Results In the present s

Protective loop ileostomy increases the risk for prolonged postoperative paralytic ileus after open oncologic rectal resection.

PURPOSE: Postoperative gut dysmotility is a physiologic and frequent temporary reaction after major abdominal surgery. If paralysis merges into a prolonged ileus state, it causes significant morbidity and subsequently worse outcome and discomfort for the patients. Pathophysiology of pathologic prolonged postoperative paralytic ileus remains multifactorial. METHODS: We present a retrospective single-center analysis of patients, who underwent a primary open oncologic anterior rectal resection with primary anastomosis with or without defunctioning loop ileostomy during a 43-month period of obser

Successful local excision of ileostomy adenocarcinoma after colectomy for familial adenomatous polyposis: report of a case.

Primary adenocarcinoma rarely develops at the site of an ileostomy performed for ulcerative colitis (UC), familial adenomatous polyposis (FAP), or Crohn's disease. We describe a case of ileostomy cancer found 14 years after proctocolectomy for FAP with cancer of both the sigmoid colon and rectum. Resection of the ileal mucosa around the stoma was performed three times. To our knowledge, only 35 other such cases have ever been reported. Our review of these cases indicates that routine examination of the stoma by a physician, or even by the patient, may lead to earlier detection of this rare com

Magnetic Side-to-Side Duodeno-Ileostomy with the MagDI System: A Technical Description of the Initial Experience in Argentina.

BACKGROUND: Anastomotic complications remain the principal source of morbidity in metabolic and bariatric surgery. Bleeding, leakage, and stricture are most often related to the anastomosis itself and to the permanent hardware left behind by staplers and sutures. Magnetic compression anastomosis has been proposed as an incisionless, sutureless alternative that avoids retained foreign material. OBJECTIVE: To describe the surgical technique for the creation of a side-to-side duodeno-ileal anastomosis using the Magnet Anastomosis System (MagDI) and to summarize early clinical outcomes. TECHNIQU

Ileostomy creation for rectal cancer in England: An observational study of variation, and correlates using administrative data.

AIMS: The decision to create an ileostomy following anterior resection is a clinical one based on the judgement of the surgical team. However, there are few data to support decision-making. This study aimed to explore variation in, and factors associated with, ileostomy creation rates following surgery for rectal cancer in England. METHODS: This exploratory, retrospective analysis used observational data from England's Hospital Episode Statistics (HES) dataset. All patients aged ≥17 years undergoing an elective anterior resection with ileostomy creation from 1 April 2016 to 31 March 2024 were

Comparison of complication rates after early and late closure of loop ileostomies: A retrospective cohort study.

BACKGROUND: In the treatment of rectal cancer, a temporary loop ileostomy (TLI) is created after low anterior resection to protect bowel function in the postoperative period. Dehydration and kidney failure are significant potential complications in loop ileostomies. Compared to late closure (3-6 months), early closure (10-14 days) of the TLI may facilitate faster patient recovery and shorter hospital stays. AIM: To compare early and late closure of TLIs and demonstrate that early stoma closure can be performed without increasing morbidity. METHODS: This study included patients who underwent

Construction and application of home dietary management program for postoperative patients with preventive ileostomy for rectal cancer.

BACKGROUND: Rectal cancer is a highly prevalent malignant tumor, and surgery is one of the main treatment methods. Although preventive ileostomy can reduce postoperative complications, it is also prone to cause malnutrition and other problems. OBJECTIVE: To construct and validate an early postoperative home dietary management program for patients with preventive ileostomy for rectal cancer, aiming to improve their nutritional status and mitigate clinical ileostomy complications. METHODS: An evidence-based dietary management program was developed, covering dietary transition, nutrient intake,

Unusual presentation of a neuroendocrine tumor in the ileostomy specimen after rectal cancer treatment: a case report.

BACKGROUND: Neuroendocrine tumors of the small intestine are uncommon, but at the same time they are the most frequent subtype of neuroendocrine tumor in the gastrointestinal system. They originate from enterochromaffin cells, which are involved in the creation of serotonin. This asymptomatic characteristic in the initial presentation is usually why these tumors are discovered at a late stage, sometimes in association with symptomatic metastatic disease. CASE DESCRIPTION: We present a case-report of a 52-year-old gentleman with a suggestive family history of hereditary cancer syndrome (mother

Overall readmissions and readmissions related to dehydration after creation of an ileostomy: a systematic review and meta-analysis.

BACKGROUND: Hospital readmissions after creation of an ileostomy are common and come with a high clinical and financial burden. The aim of this review with pooled analysis was to determine the incidence of dehydration-related and all-cause readmissions after formation of an ileostomy, and the associated costs. METHODS: A systematic literature search was conducted for studies reporting on dehydration-related and overall readmission rates after formation of a loop or end ileostomy between January 1990 and April 2021. Analyses were performed using R Statistical Software Version 3.6.1. RESULTS:

Successful closure of ileostomy in a patient with intestinal Behçet's disease after therapy with adalimumab: A case report.

RATIONALE: Behçet's disease (BD) is a chronic immune-mediated inflammatory disorder involving multiple organ systems. In BD, intestinal ulcers can present as a refractory lesion capable of perforation, which makes the choice of treatment difficult. PATIENT CONCERNS: A 34-year-old male who was diagnosed with intestinal BD and suffered with an ileocecal perforation. He underwent surgery for an ileostomy and was given corticosteroids as treatment. However, the ulcerative lesion remained resistant to the therapy that was provided which delayed the closure operation. DIAGNOSIS: Intestinal BD with

[Rehabilitation of patients with acid-base and fluid balance disorders with short bowel syndrome after ileostomies].

BACKGROUND: Patients with ileostomies regularly suffer from short bowel syndrome or high volume output associated with loss of absorptive surface and subsequent impairment of absorption for drugs and different nutrients resulting in electrolyte and fluid balance disorders as well as renal insufficiency. Adaptation of these fundamental functions of the gut with adequate fluid uptake, absorption of sufficient different nutrients and vitamins represents a major challenge to rehabilitate these patients shortly after surgery. Patients with ileostomy often develop metabolic acidosis with normal anio

Small bowel absorption of magnesium and calcium sulphate from a natural mineral water in subjects with ileostomy.

BACKGROUND: In many developed countries, magnesium and calcium intakes do not reach recommendations for a large part of the population. Mineral water may be a useful alternative source of dietary minerals, especially in groups of people at risk for developing deficiency due to low intakes. AIM: To assess if the addition of a natural and mineral-rich water increased small bowel mineral absorption in people with ileostomy. METHODS: A controlled randomised crossover study with two periods of two days each and a minimum 5 days of washout was performed in six ileostomy subjects. Apparent mineral

Loop ileostomy closure at an ambulatory surgery facility: a safe and cost-effective alternative to routine hospitalization.

INTRODUCTION: Temporary loop ileostomies have become widely used in colorectal surgery. Subsequent ileostomy closure has traditionally required hospital admission with observation until return of bowel function. On the basis of clinical observation, the authors hypothesized that loop ileostomy closure may be performed safely without prolonged in-hospital observation. METHODS: A protocol for 23-hour observation after loop ileostomy closure was implemented at a single institution and applied to 28 patients at an ambulatory surgery facility. Patient outcomes were reviewed and results compared wi

Role of intraoperative prograde colonic lavage and a decompressive loop ileostomy in the management of transmural amoebic colitis.

A prospective analysis is presented of a selected group of 45 consecutive patients with transmural amoebic colitis treated by laparotomy, colonic lavage and ileostomy (phase 1 surgery) over 3 years. The diagnosis of amoebic colitis and amoebic perforation of the bowel were difficult and therefore all patients with 'acute abdomen' had proctosigmoidoscopy and a trial of metronidazole for 24-48 h before laparotomy. At laparotomy, adhesive wraps were present in all patients; 13 perforations were exposed by inadvertent disturbance of adhesive wraps but were successfully closed by suture to any avai

Exigent ileostomy hemorrhage. A complication of proctocolectomy in patients with chronic ulcerative colitis and primary sclerosing cholangitis.

The Portland experience with patients requiring proctocolectomy for chronic ulcerative colitis in association with primary sclerosing cholangitis has been reported. Nineteen patients had conventional ileostomy reconstruction, 5 of whom had development of stomal varices with recurrent hemorrhage due to cirrhosis and portal hypertension. When this combination of conditions exists, the therapeutic options must be carefully weighed. Perhaps ileal pouch to anal anastomosis should be considered when proctocolectomy becomes mandatory. Ileostomy is presently a contraindication to liver transplantation

A Comparison of Transanal Drainage Tube and Diverting Ileostomy Following Low Anterior Resection for Rectal Cancer.

INTRODUCTION: Diverting ileostomy (DS) and transanal drainage tube (TDT) are two common strategies to protect the anastomosis after low anterior resection (LAR). This study aimed to compare outcomes between patients receiving either a DS or a TDT following LAR. METHODS: In this retrospective cohort study, we included eligible patients with rectal cancer who underwent LAR between March 2021 and February 2025 at our centers and had no history of neoadjuvant radiotherapy. Hospital length of stay, as a short-term outcome, and long-term outcomes--including low anterior resection syndrome (LARS) sc
