Role of supplementary motor area in cervical dystonia and sensory tricks.

Sensory trick is a characteristic feature of cervical dystonia (CD), where a light touch on the area adjacent to the dystonia temporarily improves symptoms. Clinical benefit from sensory tricks can be observed before tactile contact is made or even by imagination. The supplementary motor area (SMA) may dynamically interact with the sensorimotor network and other brain regions during sensory tricks in patients with CD. In this study, we examined the functional connectivity of the SMA at rest and during sensory trick performance and imagination in CD patients compared to healthy controls using f…

Effect of Magnetic Resonance Imaging vs Conventional Treat-to-Target Strategies on Disease Activity Remission and Radiographic Progression in Rheumatoid Arthritis: The IMAGINE-RA Randomized Clinical Trial.

IMPORTANCE: Whether using magnetic resonance imaging (MRI) to guide treatment in patients with rheumatoid arthritis (RA) improves disease activity and slows joint damage progression is unknown. OBJECTIVE: To determine whether an MRI-guided treat-to-target strategy vs a conventional clinical treat-to-target strategy improves outcomes in patients with RA in clinical remission. DESIGN, SETTING, AND PARTICIPANTS: Two-year, randomized, multicenter trial conducted at 9 hospitals in Denmark. Two hundred patients with RA in clinical remission (disease activity score in 28 joints-C-reactive protein [

Cerebral processes related to visuomotor imagery and generation of simple finger movements studied with positron emission tomography.

Positron emission tomography was used to compare the functional anatomy of visual imagination and generation of movement. Subjects were asked to generate visual images of their finger movement in response to a preparatory signal. Four conditions were tested: in two, no actual movement was required; in the other two, a second signal prompted the subjects to execute the imagined movement. Which movement to imagine was either specified by the preparatory stimulus or freely selected by the subjects. Compared with a rest condition, tasks involving only imagination activated several cortical regions…

'We All Try to Imagine What It's Like for Someone to Be Dependent': Attitudes Towards Drug Users Among Medical Students in the United Kingdom and Its Impact on Their Professional Identity Formation.

BACKGROUND: Evidence suggests that some doctors and medical students hold stigmatising attitudes towards drug users. This area remains underexplored in the United Kingdom (UK), and current medical students' attitudes to drug users are unknown. AIMS: This study aimed to explore the UK medical students' attitudes towards drug use, factors influencing these attitudes and their impact on professional identity formation. METHODS: A mixed methods study used a questionnaire and interviews in two UK medical schools. RESULTS: Data were collected from 62 questionnaires and 12 interviews. Most UK medi

Functional relationships between Notch, Su(H) and the bHLH genes of the E(spl) complex: the E(spl) genes mediate only a subset of Notch activities during imaginal development.

The basic helix-loop-helix proteins of the Enhancer of split complex constitute a link between activation of the transmembrane receptor Notch and the resulting effects on transcription of downstream genes. The Suppressor of Hairless protein is the intermediary between Notch activation and expression of all Enhancer of split genes even though individual genes have distinct patterns of expression in imaginal discs. A comparison between the phenotypes produced by Notch, Suppressor of Hairless and Enhancer of split mutations in the wing and thorax indicate that Suppressor of Hairless and Notch req

A Randomized Controlled Trial of Self-Guided Digital Written Imaginal Exposure Therapy for Eating Disorders in Patients Recently Discharged From Higher Level Specialty Eating Disorder Care.

OBJECTIVE: Imaginal exposure therapy facilitates the approach of feared stimuli (e.g., fear of weight gain) not accessible in everyday life (e.g., rapidly gaining weight) and has initial promising data for the treatment of eating disorders (EDs). METHODS: In the current randomized controlled trial (RCT) participants (N = 130) were randomly assigned to a five-session digital imaginal exposure condition or a control online journaling condition within 4 months of discharge from intensive (e.g., inpatient) treatment. We examined the feasibility and acceptability of the treatment conditions, targe

(Re)imagining the Future of Genetic Counseling: A Reflexive Qualitative Analysis of Sociopolitical Power, Cultural Safety, Systemic Racism, and Comparative Practice in the United Kingdom, Aotearoa New Zealand and, Australia.

Genetic counseling is undergoing a rapid transformation as genomic medicine becomes embedded within mainstream healthcare systems. At the same time, the profession is being challenged to respond to systemic racism, colonial legacies, technological change, and evolving expectations regarding equity and justice. Historically, genetic counseling emerged within twentieth-century medical genetics and was influenced by political, social, scientific, and medical forces that included eugenic ideology, values, and practices. The profession has since evolved substantially toward psychosocial, patient-ce

The evolutionary origin of psychosis.

Imagination, the driving force of creativity, and primary psychosis are human-specific, since we do not observe behaviors in other species that would convincingly suggest they possess the same traits. Both these traits have been linked to the function of the prefrontal cortex, which is the most evolutionarily novel region of the human brain. A number of evolutionarily novel genetic and epigenetic changes that determine the human brain-specific structure and function have been discovered in recent years. Among them are genomic loci subjected to increased rates of single nucleotide substitutions…

"I really can't imagine anything that could've been more easy": patient perspectives on low-threshold peer-facilitated telemedicine hepatitis C treatment for people who use drugs.

BACKGROUND: Injection drug use among nonurban people has fueled sharp rises in Hepatitis C (HCV) infections. Innovative treatment models are needed that circumvent healthcare system barriers for people who use drugs (PWUD), particularly in rural areas. The Oregon HOPE TeleHCV study randomized PWUD living with HCV in rural Oregon to peer-facilitated and streamlined telemedicine HCV treatment (Peer TeleHCV) versus enhanced usual care (EUC) and assessed sustained virologic response at 12 weeks post treatment (SVR12). Peer Support Specialists (peers) conducted HCV screening in the community, facil

Does treating emotional memories come at a price? Effects of single-session EMDR, imaginal exposure, and imagery rescripting on forced-choice recognition of event details in healthy adults - a laboratory study.

Eye Movement Desensitization and Reprocessing (EMDR), Imaginal Exposure (IE), and Imagery Rescripting (ImRs) are effective trauma-focused interventions. However, concerns persist that they may impair the accuracy of memories addressed in treatment. This laboratory study tested whether a single session of EMDR, IE, or ImRs affects forced-choice recognition in healthy adults. Two hundred sixty-five participants underwent the Trier Social Stress Test (TSST) and received EMDR, IE, ImRs, or no intervention (NIC) the following day. One week later, memory for the TSST was assessed using a forced-choi

Investigating unilateral and bilateral motor imagery control using electrocorticography and fMRI in awake craniotomy.

BACKGROUND: The rapid development of neurosurgical techniques, such as awake craniotomy, has increased opportunities to explore the mysteries of the brain. This is crucial for deepening our understanding of motor control and imagination processes, especially in developing brain-computer interface (BCI) technologies and improving neurorehabilitation strategies for neurological disorders. OBJECTIVE: This study aimed to analyze brain activity patterns in patients undergoing awake craniotomy during actual movements and motor imagery, mainly focusing on the motor control processes of the bilateral…

Essaying the mechanical hypothesis: Descartes, La Forge, and Malebranche on the formation of birthmarks.

This essay examines the determination by Cartesians to explain the maternal imagination's alleged role in the formation of birthmarks and the changing notion of monstrosity. Cartesians saw the formation of birthmarks as a challenge through which to demonstrate the heuristic capacity of mechanism. Descartes claimed to be able to explain the transmission of a perception from the mother's imagination to the fetus' skin without having recourse to the little pictures postulated by his contemporaries. La Forge offered a detailed account stating that the failure to explain the maternal imagination's …

Repetitive behaviour and play in typically developing children and children with autism spectrum disorders.

The view of a triad of impairments [(Wing and Gould (1979). Journal of Autism and Developmental Disorders, 9, 11-30] in which impaired imagination is linked with repetitive behaviour is widely accepted. However this categorisation differs from the international classification systems, which link imagination to communication impairments rather than to repetitive behaviours. To investigate this relationship, the Activities and Play Questionnaire-Revised was completed by 196 parents of 2-8-year-old children with autism spectrum disorders (ASD) and typical development. Results showed that repetiti…

A pilot evaluation of the efficacy and acceptability of a novel imaginal exposure prevention (I-ERP) group programme to treat core weight, shape and social fears or phobias in adolescents with anorexia nervosa in an inpatient setting.

BACKGROUND: Anxiety due to a phobia of normal body weight is a core feature and maintenance factor of anorexia nervosa (AN). This is the first study to explore the efficacy and acceptability of using a novel imaginal exposure response prevention group to target fears associated with being a normal body weight to reduce anxiety in adolescents with AN. METHODS: The lead author adapted an I-ERP manual used to treat AN in adults in 1-1 therapy. Content was adapted for an adolescent population, sessions reduced from 10 to 4 and was delivered in a group format with audio recordings to be more acces
