Between 1987 and 1992, 48 patients with ruptured Achilles tendons were operated on using a standardized microinvasive technique; 30 patients underwent functional postoperative treatment, whereas 18 underwent conventional cast immobilization. We report four cases of one leaflet immobilization which occurred early after mitral valve replacement with a Carbomedics bileaflet valve. The first case was diagnosed incidentally six days after surgery and did not require reoperation because of the spontaneous normalization of the valve movements the day before the scheduled reoperation. Stress induced analgesia (SIA) and stress-induced changes in body temperature were studied in mice and rats. Immobilization was used as the stressor. Nociception was measured with the tail-flick method and body temperature was recorded in the colon. Within 5 min of immobilization a similar increase in tail-flick latencies was observed in the two species. Concomitantly, the body temperature increased in the rats and decreased in the mice. The left thoracic limb was immobilized in a plaster cast in 6 grade weanling ponies for 6 weeks. Two ponies were injected intramuscularly each day with 2.4 micrograms of 25-hydroxycholecalciferol [25(OH)D3] per kg bodyweight, two with 1.2 micrograms and two received no injections. The kinesio, phychopathologic and psychosocial effects of prolonged immobilization of the ill aged are outlined. The iatrogenic (physician-induced) and nurisgenic (nurse-induced) factors related to such functional disabilities are described. Illustrative case histories are given. The syndrome is reversible. Thus physicians and nurses have a continued responsibility to support a sustained rehabilitation program for these patients. The biochemical effects of prolonged inactivity indicate that immobilization of the elderly patient results in adverse physical and psychologic phenomena.
