[Prenatal diagnosis of severe and hereditary immune deficiencies].

Antenatal diagnosis is now available for most severe inherited immune deficiencies. Several techniques are used: the development of methods for sampling fetal tissue as soon as the tenth week of gestation has made possible the antenatal diagnosis of immune deficiencies associated with detectable enzyme defects, and, in combination with recent molecular biology techniques, can be expected to allow early identification of severe combined immune deficiencies due to the absence of T lymphocyte precursors, agammaglobulinemia, and some instances of X-linked chronic granulomatous disease. A great num

Pulmonary aspergillosis in a case of humoral immunodeficiency.

A case of recurrent pulmonary aspergillosis in a young girl with common variable hypogammaglobulinaemia and the treatment adopted is described. We employed the plasma-exchange, at the beginning of immunosubstitutive treatment, to provide a high serum concentration of all classes of immunoglobulins and to better control the patient's state of shock. Intravenous immunoglobulins and Amphotericin B were also given in combination with plasma-exchange, having a synergic therapeutic effect. The good condition of the patient, despite the recurrent episodes of fungal pneumonia, suggests that such an ap

A new guinea pig mutant with abnormal hair production and immunodeficiency.

Hairless guinea pigs were born in a closed colony of Hartley guinea pigs. At birth, hairless guinea pigs were smaller than littermates, had wrinkled skin and stunted vibrissae. The most striking abnormality histologically was distension of the upper portion of the pilary canal with thickening of the epidermis. Hairlessness resulted from production of abnormal hair shafts. Preliminary work indicated that in addition to hairlessness, the guinea pigs were athymic or hypothymic. Initial immunologic studies showed that the guinea pigs also were agammaglobulinemic. Animals that survived the weaning

Cryptosporidiosis in immunodeficient Arabian foals.

Five of six immunodeficient Arabian foals that died of adenoviral infection were found to be infected with an intestinal coccidian of the genus Cryptosporidium. Various developmental stages of the organism were found in the microvillous border of the intestinal mucosa. The foals had diarrhea but it was not possible to separate the effects of the cryptosporidial infection from those of the concomitant adenoviral enteritis.

Autoimmunity and inborn errors of immunity: a complex coexistence.

Inborn errors of immunity (IEI) and autoimmune diseases represent two major consequences of immune system dysregulation. Although recurrent infections are a hallmark of IEI, autoimmune manifestations are also highly prevalent in affected patients. Nevertheless, identifying an underlying IEI in individuals presenting primarily with autoimmune disorders remains challenging, as these manifestations can be highly heterogeneous. Many pediatricians and specialists managing autoimmune conditions do not systematically consider an early immunological evaluation, often underestimating the likelihood of

Population Pharmacokinetics and Pharmacodynamics of Immunoglobulins: A Systematic Review.

BACKGROUND AND OBJECTIVE: Intravenous and/or subcutaneous immunoglobulins (IVIg/SCIg) are used as immunoglobulin replacement therapy (IgRT) in primary and secondary immunodeficiencies (PID/SID) and as immunomodulatory therapy in immune-mediated diseases. Despite empiric immunoglobulin G (IgG) target concentrations in PID, dosing remains variable and suboptimal, particularly in immune-mediated diseases lacking clear IgG target concentrations. Population pharmacokinetic (popPK) and pharmacodynamic (PD) modelling may clarify interindividual variability and support individualised dosing. Recent de

A global perspective on autoimmunity and immunodeficiency: exploring geoepidemiology trends.

Autoimmune and primary immunodeficiency disorders represent a growing global health burden influenced by a complex interplay of genetic, environmental, and socioeconomic factors. This paper offers a geoepidemiological analysis of these conditions, highlighting regional differences in incidence, diagnosis, and outcomes. Immunodeficiencies such as Common Variable Immunodeficiency (CVID), complement deficiencies, Selective IgA Deficiency, and Severe Combined Immunodeficiency illustrate the influence of healthcare access and genetic variation on detection and management. For instance, CVID is disp

A scoping review of clinical management guidelines in inborn errors of immunity.

BACKGROUND: A scoping review was conducted for the European Society for Immunodeficiencies Clinical Working Party to evaluate clinical management guidelines for inborn errors of immunity (IEI). OBJECTIVE: We sought to identify gaps to inform future IEI guideline development, thereby supporting improved clinical practice and patient outcomes. METHODS: A search strategy was developed in collaboration with the European Society for Immunodeficiencies Clinical Working Party and an information specialist. The search was performed using Embase, Medline, and Google Scholar. Search terms were based o

Infection prevention in patients with inborn errors of immunity: Insights from Japan's nationwide study.

BACKGROUND: Differences in infection prevention methods and their corresponding effects on patients with inborn errors of immunity (IEI) between departments that treat patients with IEI and other departments, such as local hospitals, are not known. METHODS: We compared the differences in infection prevention methods between low-volume departments (treating fewer than 20 patients with IEI) and high-volume departments (treating 20 or more patients with IEI) using recent nationwide survey data from Japan. RESULTS: There was an uneven distribution of IEI categories between low- and high-volume d

Sphingosine phosphate lyase insufficiency syndrome as a primary immunodeficiency state.

Sphingosine phosphate lyase insufficiency syndrome (SPLIS) is a genetic disease associated with renal, endocrine, neurological, skin and immune defects. SPLIS is caused by inactivating mutations in SGPL1, which encodes sphingosine phosphate lyase (SPL). SPL catalyzes the irreversible degradation of the bioactive sphingolipid sphingosine-1-phosphate (S1P), a key regulator of lymphocyte egress. The SPL reaction represents the only exit point of sphingolipid metabolism, and SPL insufficiency causes widespread sphingolipid derangements that could additionally contribute to immunodeficiency. Immunodeficiency: Quantitative and qualitative phagocytic cell defects.

The immune system is divided into two major branches: innate and adaptive. The innate immune system is the body's first line of defense and rapidly responds in a nonspecific manner to various microorganisms, foreign materials, or injuries. Phagocytes, which include macrophages, monocytes, and neutrophils, are innate immune cells that can surround and kill microorganisms, ingest foreign material, and remove dead cells. They also indirectly boost both innate and adaptive immune responses through various activation signals. Phagocytic defects characteristically lead to fungal and bacterial infect
