[The practice guideline 'Bacterial skin infections' (first revision) from the Dutch College of General Practitioners; a response from the perspective of general practice].

The revised guideline 'Bacterial skin infections' from the Dutch College ofGeneral Practitioners offers a clear and extensive overview of the most prevalent superficial and deep bacterial infections in general practice. Given the lack of evidence, it is no longer recommended to keep children with impetigo out of school or daycare centres. Erysipelas and cellulitis are now considered variants of the same bacterial infection and require the same therapy. Due to its rising prevalence, methicillin-resistant Staphylococcus aureus should also be considered. In conclusion, the guideline is a practica…

Prospects for a group A streptococcal vaccine.

Group A streptococcal (GAS) infections are associated with a number of human diseases, including pharyngitis, impetigo, necrotizing fasciitis, streptococcal toxic shock syndrome and rheumatic heart disease. An increase in the incidence of severe GAS infections in Western countries, and the awareness of the burden of GAS-associated diseases in developing nations, which remains high in spite of the availability of antibiotics, has provided the impetus for development of a safe and efficacious GAS vaccine. This has focused on the M protein, a major GAS virulence factor, however, with the publicat…

Topical antibiotics in the dermatological clinical practice: Indications, efficacy, and adverse effects.

Topical antibiotic therapy is a central component of patient management for several skin conditions, including acne, hidradenitis suppurativa, rosacea, impetigo or other superinfected dermatitis, and prevention of wound infections. Moreover, particular situations, such as skin diseases of bacterial origin in pregnancy and infants often warrant topical therapy. However, the occurrence of local delayed hypersensitivity reactions and the rising rate of antibiotic resistance are becoming great challenges faced by many dermatologists today. This narrative review provides an overview of the main top…

Depressive Disorder and Dermatological Autoimmune Diseases.

Depressive disorders are a growing problem worldwide. They are also characterized by high comorbidity, including from the circle of dermatological diseases. Autoimmune diseases seem to be particularly correlated with depressive comorbidity, raising the question of their possible common pathomechanism. The PubMed database was searched, focusing on results published after 2016. A particular reciprocal correlation of depressive disorders with psoriasis, atopic dermatitis, alopecia areata, impetigo, lupus and systemic scleroderma was found. One possible explanation for the co-occurrence of the abo…

Skin infections in Tanna, Vanuatu in 1989.

Skin infections are an important health problem in Vanuatu and other Pacific island countries. In this study, 18,223 people, representing 90% of the population of the southern island of Tanna in Vanuatu, were examined. Scabies was found in 16% of those screened, with 24% of children under 10 years affected. Other skin infections were predominantly furuncles, abscesses, impetigo and cellulitis and were found in 12% overall and in 16% of children under 10 years of age. These infections were found to be more common in larger households and in the vicinity of the volcano Yasur. Strategies for prev…

Skin-isolated, community-acquired Staphylococcus aureus: in vitro resistance to methicillin and erythromycin.

During a 10-month period, skin culture specimens were taken from 1680 healthy outpatients with a variety of community-acquired skin infections. Staphylococcus aureus was found in 1035 (61.6%) of these patients. In vitro resistance to methicillin and erythromycin was 1.0% and 42.9%, respectively. Resistance rates to erythromycin in patients with furunculosis and impetigo were 51.5% and 26.2%, respectively (p less than 0.001). The emergence of erythromycin-resistant strains may be the result of widespread use of this drug in our geographic area. There is also the possibility that certain bacteri…

Children's scabies survey indicates high prevalence and misdiagnosis in Auckland educational institutions.

AIM: Here, we present results of a survey of scabies prevalence in childcare centres and primary schools in Auckland. METHODS: Children whose parents agreed to take part in participating centres in the Auckland region were examined for scabies by general practitioners and given questionnaires of relevant symptoms. Diagnoses of clinical or suspected scabies were made according to the International Alliance for the Control of Scabies (IACS) criteria. The survey was a stratified random sample of schools and early childcare centres. A quantitative polymerase chain reaction (PCR) test was also use

Papulovesicular Riddle in an Atopic Individual.

Atopic dermatitis (AD) characterized by pruritus and eczematous lesions makes individuals susceptible to various viral and bacterial infections. Eczema herpeticum (EH), also known as Kaposi's varicelliform eruption, is a severe herpes simplex virus infection that can be observed in individuals with AD. EH manifests with monomorphic vesicles and "punched-out" erosions accompanied by hemorrhagic crusts, primarily affecting eczematous areas. Misdiagnosis, often as impetigo, can lead to severe complications and even death. Timely diagnosis and treatment with acyclovir are crucial to avert these ou…

Skin and Soft Tissue Infections in the Emergency Department.

This article covers the diagnosis and treatment of skin and soft tissue infections commonly encountered in the emergency department: impetigo, cutaneous abscesses, purulent cellulitis, nonpurulent cellulitis, and necrotizing skin and soft tissue infections. Most purulent infections in the United States are caused by methicillin-resistant Staphylococcus aureus. For abscesses, we emphasize the importance of incision and drainage. Nonpurulent infections are usually caused by streptococcal species and initial empiric antibiotics need not cover methicillin-resistant Staphylococcus aureus. For uncom…

The present guideline was developed by the adaptation method. This clinical guideline provides recommendations for the diagnosis and management of SSTI, including impetigo/ecthyma, purulent skin and soft tissue infection, erysipelas and cellulitis, necrotizing fasciitis, pyomyositis, clostridial myonecrosis, and human/animal bite. This guideline targets community-acquired skin and soft tissue infection occurring among adult patients aged 16 years a…

Effectiveness of gentian violet and similar products commonly used to treat pyodermas.

The term pyoderma encompasses a variety of distinct entities including impetigo (bullous and nonbullous), erysipelas, cellulitis, folliculitis, and staphylococcal scalded skin syndrome. Treatment of pyodermas centers around wound care and appropriate antibiotic selection. Triphenylmethane dyes, such as gentian violet, represent a unique group of compounds that act as antiseptics and have shown clinical efficacy as antibiotics in a variety of pyodermas, including those secondary to methicillin-resistant Staphylococcus aureus. Given their low cost, ease of application, and favorable side effect …
