The tumor was resected with wide margins through a Fisch infratemporal fossa approach type A, followed by radiotherapy. Results of the immunophenotyping along with histological features primarily matched moderately differentiated chondrosarcoma. The patient is recurrence free at 6 months after treatment. Middle ear discharge …

PURPOSE: Treatment of tumors arising in the upper parapharyngeal space (PPS) or the floor of the middle cranial fossa is challenging. This study aims to present anatomical landmarks for a combined endoscopic transnasal and anterior transmaxillary approach to the upper PPS and the floor of the middle cranial fossa and to further evaluate their clinical application.
METHODS: Dissection of the upper PPS using a combined endoscopic endonasal transpterygoid and anterior transmaxillary approach was performed in six cadaveric heads. Surgical landmarks associated with the approach were defined. The d

Tumors occurring in the infratemporal region present a surgical challenge and access osteotomies of the facial skeleton is the answer to access these deeply situated, inaccessible tumors of the head and neck. Various approaches have been devised for their better exposure and it is our expertise as maxillofacial surgeons to provide surgical access by transmaxillary, transzygomatic and transmandibular approaches. We followed this concept in our institute and report here two case reports. The first is a 45-year-old female who presented with right facial pain and temporal swelling due to schwannom

IntroductionRheumatoid nodules are one of the most common extra-articular manifestations in rheumatoid arthritis (RA) being extremely uncommon in the ocular and periocular area. To the authors' knowledge, this is the second case of an orbital rheumatoid nodule.Case descriptionA 44-year-old woman with RA presented with a painful hard mass in the inferolateral right orbital rim causing mild lower lid retraction, ptosis and exophthalmos. Magnetic Resonance Imaging showed a extraconal mass involving the lower eyelid and fat tissue of the infratemporal fossa. An incisional biopsy demonstrated an or…

Indications for the use of computer-assisted surgery (CAS) in otorhinolaryngology can be grouped into several domains including the following: difficult cases involving the anterior, middle, and posterior skull base, especially the rhinobasis or laterobasis, and diseases of the petrous bone, the infratemporal fossa, the retromaxillar space, and the orbit; extended paranasal sinus surgery and revision cases with surgically altered anatomic landmarks; and use for educational purposes where CAS can provide better instruction in surgical anatomy by relating the surgical field to radiological secti…

Congenital carotid aneurysms are frequently found in the intracranial arteries but rarely seen in extracranial part of the internal carotid artery. A case of a congenital internal carotid artery aneurysm at the skull base was treated in a 13-year-old female child. This case is being presented for its rarity and also to highlight the ease of exposing the upper end of the aneurysm via lateral skull base approach. Infratemporal fossa approach was adopted in this case. The carotid canal was drilled off and the upper end of the aneurysm in the intrapetrous part was mobilized and vascular clip appli…

The maxillary artery (MA) passes over the lateral pterygoid muscle in the infratemporal fossa and enters the pterygopalatine fossa through the pterygomaxillary fissure. Refractory epistaxis is managed by ligation of the sphenopalatine artery via a transmaxillary-transantral approach; there is considerable risk of complications associated with such invasive surgical approaches. The aim of this study was to describe the gross anatomy and variations therein of the MA and its branches at the pterygopalatine fossa. One hundred hemifaces of embalmed Korean adult cadavers were dissected to establish …

Skull base meningiomas, although histologically benign, can exhibit locally aggressive behavior, particularly when arising from the sphenoid wing. Their potential to infiltrate bone and extend extracranially into the orbit, infratemporal fossa, and facial soft tissues significantly increases surgical complexity and recurrence risk. Complete resection in these cases requires wide anatomical exposure and multidisciplinary planning to preserve critical neurovascular structures while achieving oncologic control. We present the case of a woman with a recurrent sphenoid wing meningioma extending int…

Juvenile nasopharyngeal angiofibroma is a rare vascular neoplasm, mostly occurring in adolescent males, and representing 0.05% of all head and neck tumors. Nevertheless, it is usually recognized as the most common benign mesenchymal neoplasm of the nasopharynx. Usually, it originates from the posterolateral wall of the nasopharynx and, although histologically benign, classically shows a locally aggressive behavior with bone destruction as well as spreading through natural foramina and/or fissures to the nasopharynx, nasal and paranasal cavities, spheno-palatine foramen, infratemporal fossa and…

A 13-year-old Japanese boy with a 6-month history of bilateral nasal obstruction and a 3-week history of recurrent epistaxis from the right nose was admitted to our department. Nasal endoscopy revealed a reddish, smooth-walled tumor occupying the right nasal cavity. Computed tomography scan revealed a 3.5 × 4.5 × 7.0-cm heterogeneously enhancing mass involving the right nasal cavity and extending posteriorly to the nasopharynx, and laterally to the pterygopalatine fossa and the medial part of the infratemporal fossa. We diagnosed as juvenile nasopharyngeal angiofibroma with Radkowski classific…

INTRODUCTION: Skull base meningoencephaloceles are rare congenital malformations. The two cases of transalar transsphenoidal malformation reported here differ from the classical transsphenoidal meningoencephalocele.
CASE REPORTS: Case 1 was a three-and-a-half-year-old boy and case 2 was a 36-year-old man. Both cases presented with clinical features of recurrent meningitis. Surgical management of case 1 was performed via an intradural infratemporal fossa craniotomy with reoperation 2years later. In the second case, surgery was initially performed via an endonasal approach and then via pteriona…

In 1982, Fisch described his results for the surgical treatment of 74 paragangliomas of the temporal bone, 5 years after his description of the infratemporal fossa approaches (types A and B). This study reviews the subsequent experience of the Department of Otolaryngology-Head and Neck Surgery of the University of Zürich with more than 136 surgically treated cases of paraganglioma of the temporal bone and discusses our current therapy 20 years after the initial description. One hundred nineteen (90%) of the patients had advanced tumors (Fisch class C or C+D), and 81 (68%) had intracranial exte…

A 36-year-old female presented with a long-standing history of headache. Computed tomography scan and magnetic resonance imaging revealed an enhancing lesion occupying the right Meckel's cave, enlarging the trigeminal ganglion and extending through the foramen ovale into the infratemporal fossa. A right frontotemporal extradural approach to the cavernous sinus was performed and a firm, pinkish lesion intermingled with nerve fibers enlarging the trigeminal ganglion and V3 branch was identified. Frozen section was suggestive of a neuromuscular hamartoma. The lesion was sub-totally resected to av…

Clinical features of temporal bone cholesteatoma are miscellaneous, and sometimes misleading: signs of middle ear cholesteatoma, progressive or sudden facial palsy, sensorineural deafness as in acoustic neuroma, conductive deafness as in otosclerosis, secretory otitis media, or intracranial complications. Polytomography is the only way to pinpoint topography and extension. CT scanning is very useful in determining extension. Topography and severity of deafness are the guidelines for surgical approach. Among sixteen cases, total deafness was produced by the lesion itself in ten cases, and six h
