Postnatal Cytomegalovirus Infection in a Neonatal Intensive Care Unit: A Retrospective Study.

INTRODUCTION: Postnatal cytomegalovirus (pCMV) infection is a relevant cause of morbidity in preterm infants, particularly those born extremely preterm or with very low birth weight. This study aimed to describe the clinical manifestations, management, and outcomes of neonates with pCMV infection admitted to a neonatal intensive care unit (NICU). METHODS: We conducted a retrospective case series of 12 neonates diagnosed with pCMV infection between 2013 and 2024 in a level III NICU. Congenital cytomegalovirus (CMV) infection was excluded by negative urine testing within the first two weeks of

Factors associated with persistent bloodstream infection in the Neonatal Intensive Care Unit.

OBJECTIVE: This study determined factors associated with persistent bloodstream infections (BSIs) for infants in the NICU to identify when follow up blood cultures (FUBCs) have increased utility. STUDY DESIGN: Single center study of all infants in a level IV NICU (n = 121) with a positive blood culture over a five-year period. Clinical and microbiological variables were examined with bivariate and multi-regression analyses to identify factors associated with persistent BSI, defined as growth of the same organism >48 h after the index culture. RESULTS: The recovery of Staphylococcus aureus (O

The Use of Low-Dose Dopamine in the Neonatal Intensive Care Unit.

Dopamine has been used for half a century in adult and pediatric patients for the treatment of hypotension, as well as for the theoretical prevention of acute kidney injury (AKI). Although activation of renal dopamine receptors leads to increased urine output, there is no evidence that low-dose dopamine reduces the incidence of AKI, need for dialysis, or death. Dopamine administration is also associated with multiple adverse effects, particularly in preterm infants. Despite the lack of evidence for its use, as well as the known adverse effects of dopamine, many neonatologists still use low-dos

Epidemiology and outcomes of bacterial meningitis in the neonatal intensive care unit.

OBJECTIVE: Examine pathogen distribution, antibiotic resistance patterns, and hospital outcomes of infants with bacterial meningitis in neonatal intensive care units (NICUs) in the US from 2013-2018. STUDY DESIGN: Infants were divided into 2 groups based on age at the time of meningitis: early-onset (0-3 days) and late-onset (>3 days). We compared demographics, clinical characteristics, epidemiology, hospital outcomes, distribution of organisms and resistance, and blood culture timing relative to cerebrospinal fluid culture. RESULTS: From 345 NICUs, 659 infants were diagnosed with bacterial

How is bioelectrical impedance used in neonatal intensive care? A scoping review.

UNLABELLED: Poor growth and nutrition management in the neonatal period can have a negative impact upon both the short- and long-term outcomes for the infant. Improvements in bioelectrical impedance technology and accompanying licencing agreements now make this enhanced device available for use in infants as small as 23 weeks gestational age. An exploration of this technology and its use is now timely. The aim of the scoping review was to answer the following question: in preterm and sick term infants in the neonatal intensive care unit, how is bioelectrical impedance being utilized, in what s

Burkholderia cenocepacia outbreak linked to taps in a neonatal intensive care unit.

BACKGROUND: Burkholderia cenocepacia complex is an important cause of hospital acquired infections. We describe the management of an outbreak in a neonatal intensive care unit (NICU) due to tap colonisation. METHODS: Microbiological testing of touch (n = 26) and non-touch taps (n = 28), sinks and drains, including genomic sequencing of selected isolates. Thermal shocking of taps with 30 min of water flush at 60 °C. Tap aerators were changed with each thermal shock. Adjuvant disinfecting measures were applied to aerator mesh at the water exit point of the tap, point-of-use water filters, drain

Gut pathogen colonization precedes bloodstream infection in the neonatal intensive care unit.

Bacterial bloodstream infections (BSIs) resulting in late-onset sepsis affect up to half of extremely preterm infants and have substantial morbidity and mortality. Bacterial species associated with BSIs in neonatal intensive care units (NICUs) commonly colonize the preterm infant gut microbiome. Accordingly, we hypothesized that the gut microbiome is a reservoir of BSI-causing pathogenic strains that increase in abundance before BSI onset. We analyzed 550 previously published fecal metagenomes from 115 hospitalized neonates and found that recent ampicillin, gentamicin, or vancomycin exposure w

Mildred T. Stahlman: a centenarian who pioneered modern scientific neonatal intensive care.

Neonatology pioneer Mildred (Millie) T. Stahlman celebrated her 100th birthday on July 31, 2022. Her distinguished career at Vanderbilt University Medical Center in Nashville, TN, is reviewed to commemorate this milestone. Stahlman was arguably the first to establish a modern neonatal intensive care unit in 1961, successfully utilizing negative pressure ventilation and umbilical arterial and venous catheters to monitor blood gasses and pH levels. She received early invaluable training in newborn physiology at the Karolinska Institute in Stockholm, Sweden, under John Lind and Petter Karlberg, a

Maternal Stress Experience in the Neonatal Intensive Care Unit after Preterm Delivery.

OBJECTIVE: Preterm delivery with the admission of a child in the neonatal intensive care unit (NICU) is extremely challenging for parents. Being separated from the baby and seeing her infant feeling pain and being sick together with the complexity of the NICU environment, the mother experiences great anxiety, fear, and stress. The purpose of the study was to assess NICU-related stress and to identify maternal and infant factors associated with increased stress in mothers of preterm infants. STUDY DESIGN: The maternal stress level was assessed in 122 mothers of preterm infants using the Parent

NeoCHIRP: A model for intestinal rehabilitation in the neonatal intensive care unit.

Multidisciplinary intestinal rehabilitation (IR) teams have transformed care in pediatric intestinal failure (IF).1 Although most children with IF are identified in the neonatal intensive care unit (NICU), IR teams may not be involved at this stage. We describe our collaborative model, blending NICU and IR expertise to optimize care. Over 6 years, the NeoCHIRP (Neonatal Children's IR Program) team followed 164 babies for weekly visits (median, 8; range, 1-27). Bedside rounds included CHIRP team physician and surgeons, neonatologist champion, attending neonatologist and fellow, NICU dietitian,

Hypoxemia in infants with trisomy 21 in the neonatal intensive care unit.

OBJECTIVE: Newborns with trisomy 21 (T21) often require NICU hospitalization. Oxygen desaturations are frequently observed in these infants, even in the absence of congenital heart defects (CHD). We hypothesized that NICU patients with T21 have more hypoxemia than those without T21. DESIGN: All infants with T21 without significant CHD discharged home from the NICU between 2009 and 2018 were included (n = 23). Controls were matched 20:1 for gestational age and length of stay. We compared daily severe hypoxemia events (SpO2 < 80% for ≥10 s) for the whole NICU stay and the pre-discharge week. R

Pharyngomalacia diagnosed by laryngo-tracheo-bronchoscopy in the neonatal intensive care unit.

BACKGROUND: Flexible fiber-optic laryngo-tracheo-bronchoscopy has become widely performed in infants and neonates since the introduction of thin flexible fiberscopes. Laryngomalacia is the most common airway disease in infants causing stridor. Pharyngomalacia, termed pharyngeal occlusion during inspiration, was the second most common airway disease found in our hospital in patients that underwent laryngo-tracheo-bronchoscopy, but the incidence, patient characteristics, and natural course have not been reported in large numbers in Japan. METHODS: A retrospective review was performed of medical

Narcotic-Sparing Approaches and the Shift Toward Paracetamol in Neonatal Intensive Care.

Effective analgesia in neonates is relevant not only because of ethical aspects or empathy, but it is a crucial and integral part of medical and nursing care. However, there is also emerging evidence - although mainly in animal models - on the relation between the exposure to narcotics and impaired neurodevelopmental outcome, resulting in a CATCH-22 scenario. Consequently, a balanced approach is needed with the overarching intention to attain adequate pain management with minimal side effects. Despite the available evidence-based guidance on narcotics in ventilated neonates, observations on dr
