Entero-vesical fistulae in surgical practice.

Any disease or traumatic event that involving the integrity of the urinary tract or of the bowel, may led to an entero-vesical fistula formation. The clinical onset of this pathology, is described on the basis of usual clinical pictures and of some unusual cases mimicking a bladder cancer. Moreover, the guide lines for a gold standard in the diagnosis and treatment of such pathology are described.

Appendicovesical fistula associated with papillovillous adenoma of the appendix.

Appendicovesical fistulae represent 5% of all intestinal vesical fistulae. Usually a known cause is appendicitis, cecal diverticulitis, cystadenocarcinoma of the appendix and carcinoid tumors. We report the 2nd case, to our knowledge, where a benign tumor is a possible reason for the formation of an appendicovesical fistula.

Crohn's disease manifesting as ileo-urachal fistula: Two cases reports and review of literatures.

INTRODUCTION: A fistula involving a patent urachus in a patient with Crohn's disease is rare. Here we report ileourachal fistula formation in two patients with Crohn' disease. CASE PRESENTATIONS: The first patient was a 29-year-old man with Crohn's disease and ileitis, and the second patient was a 43-year-old man with Crohn's disease and ileitis. One of the patients showed pus/fecal discharge via the umbilicus. Both patients were eventually diagnosed with an ileourachal fistula associated with Crohn's disease. In the first patient, the urachal remnant was connected to the urinary bladder and

Textiloma mimicking Crohn's disease in its features: A case report.

INTRODUCTION AND IMPORTANCE: Textiloma is a retained surgical item such as a sponge or gauze that is unintentionally left in the surgical field after the wound closure. Here, we present the first reported surgical gauze that penetrated the intestine, made a duodenal-ceca fistula and then stuck far away in the ileum. Mechanical obstruction didn't appear clinically or even in radiological investigations because of the fistula which provided the intestinal continuity. CASE PRESENTATION: We report a 34-year-old man with a previous abdominal interventions complained of cramping, frequent vomiting

[Case of a biliodigestive fistula--a variant of Bouveret's syndrome].

The female patient N. I. G., aged 65, case report No 21336/416 was admitted to the Clinic of Gastroeneterology on October 12, 1982, with the following diagnosis: Cholelithiasis, Cholecystitis chronica calculosa, Fistula biliodigestiva. The anamnestic data, the objective examination as the data from operation protocol revealed that the case reported was a peculiar, very rare, favourable evolution of Bouveret syndrome. The biliary concrements (the biggest 10 X 15 mm), that had passed through the fistula opening into the permanently deformed and constricted duodenal bulb have twice caused short-t

Ileocaecal Tuberculosis Presenting with Massive Hematochezia: A Rare Clinical Manifestation.

Abdominal pain, diarrhea, weight loss, anorexia, and fever are common symptoms of intestinal tuberculosis, while bleeding from the lumen, intestinal obstruction, perforation, and fistula formation are uncommon presentations in ileocaecal tuberculosis. Here, we present a case of a 33-year-old male with intestinal tuberculosis who initially presented with massive bleeding per rectum.

Population-based birth defects data in the United States, 2010-2014: A focus on gastrointestinal defects.

BACKGROUND: Gastrointestinal defects are a phenotypically and etiologically diverse group of malformations. Despite their combined prevalence and clinical impact, little is known about the epidemiology of these birth defects. Therefore, the objective of the 2017 National Birth Defects Prevention Network (NBDPN) data brief was to better describe the occurrence of gastrointestinal defects. METHODS: As part of the 2017 NBDPN annual report, 28 state programs provided additional data on gastrointestinal defects for the period 2010-2014. Counts and prevalence estimates (per 10,000 live births) were

[Nutrition therapy in enterocutaneous fistula; from physiology to individualized treatment].

Enterocutaneous fistula is the most common of all intestinal fistulas. Is a condition that requires prolonged hospital stay due to complications such as electrolyte imbalance, malnutrition, metabolic disorders and sepsis. Nutritional support is an essential part of the management; it favors intestinal and immune function, promotes wound healing and decreases catabolism. Despite the recognition of the importance of nutrition support, there is no strong evidence on its comprehensive management, which can be limiting when establishing specific strategies. The metabolic imbalance that a fistula ca…

Enterocutaneous Fistula in a COVID-19 Obese Patient During Prolonged Prone Position for Severe Acute Respiratory Distress Syndrome.

Prone position (PP) has been widely used in patients under mechanical ventilation for COVID-19 acute respiratory distress syndrome (ARDS), usually for many hours per day. Complications are not rare, although most of them are mild. To our knowledge, we report the first case of enterocutaneous fistula after prolonged use of PP in the literature. Morbid obesity; yielding increased abdominal wall pressure when the patient was prone; pre-existing intestinal hernias; and increased vasopressor doses for septic shock due to secondary infections resulted in necrosis of the small intestine, the abdomina

Results of transanal irrigation in intestinal dysfunction associated with anorectal malformations.

INTRODUCTION: There are multiple tools available to optimize defecation in patients with anorectal malformation (ARM), such as habits, laxatives, and retrograde or anterograde irrigations, which are usually adapted in a progressive and combined fashion. The objective of this study was to assess the incorporation of transanal irrigation (TAI) to constipation and fecal incontinence treatment in patients with ARM. MATERIALS AND METHODS: A retrospective study of ARM patients with indication of TAI according to the colorectal pathology unit's intestinal management protocol from 2015 to 2022 was ca

Anti-MMP-9 Antibody: A Promising Therapeutic Strategy for Treatment of Inflammatory Bowel Disease Complications with Fibrosis.

BACKGROUND: Despite medical treatments or surgical options, more than one-third of patients with Crohn's disease suffer from recurring fistulae. Matrix metalloprotease 9 (MMP-9), a type IV collagenase that cleaves components of the extracellular matrix leading to tissue remodeling, is upregulated in crypt abscesses and around fistulae suggesting an important role for this enzyme in fistula formation. Our aims were (1) to correlate serum levels of MMP-9 degradation products in patients with CD with the presence of fistulae and (2) to investigate the impact of selective MMP-9 inhibition in a mou

Treating an Intractable Jejunocutaneous Fistula by Endoscopic Metallic Stent Placement: A Case Report of Successful Palliative Endoscopic Treatment in a Case Demonstrating Peritoneal Dissemination with Terminal Stage Gastric Cancer.

A 74-year-old woman with recurrent gastric cancer underwent laparotomy for peritoneal dissemination, and the damaged jejunum formed a jejunocutaneous fistula. Because conservative treatment alone could not cure the fistula, we performed an endoscopic placement of a partially covered self-expandable metallic stent (SEMS) to cover the fistula. After the procedure, the contrast medium no longer leaked from the intestinal lumen. One month after stent placement, the cutaneous opening had closed. This case report demonstrates the potential for using partially covered SEMS to treat intractable jejuno

Platelet-rich plasma combined with lyophilizing thrombin powder for the treatment of complicated enterocutaneous fistula: a case report.

BACKGROUND: Enterocutaneous fistula is one of the most challenging problems facing surgeons. In severe cases, a large amount of fluid loss can lead to problems such as water and electrolyte acid-base imbalance, malnutrition, infection, and organ dysfunction. Here we reported a case of platelet-rich plasma combined with lyophilizing thrombin powder for the treatment of complicated enterocutaneous fistula. CASE PRESENTATION: A 48-year-old male, more than 2 years after the operation of abdominal trauma, the leakage of the fistula in the right upper abdominal wall was accompanied by fever for 3 d

Regional enteritis leading to carcinoma of the small bowel.

A case of carcinoma of the small bowel intimately associated with long-standing regional enteritis (Crohn's disease) is described. Sixteen similar cases reported in the literature are briefly reviewed. This neoplasm presents at a younger age than other small bowel carcinomas and there seems little doubt that it is causally related to the inflammatory bowel lesion.

Hydrogen peroxide enhanced ultrasound- fistulography in the assessment of enterocutaneous fistulas complicating Crohn's disease.

BACKGROUND/AIMS: Proper management of enterocutaneous fistulas complicating Crohn's disease largely depends on the anatomical characteristics of the sinus tracks as well as the coexistence of complications such as abscesses and distal bowel stenosis. The aim of this prospective study was to evaluate the accuracy of a new technique (hydrogen peroxide enhanced ultrasound (US)-fistulography) compared with conventional x ray fistulogram and/or surgical findings in the detection of Crohn's disease associated enterocutaneous fistulas. METHODS: Patients with known Crohn's disease and a suspicion of
