OBJECTIVE: To test our hypothesis that surgeon-placed paravertebral block (PVB) placement during open renal surgery is effective, feasible, and safe. Neuraxial analgesia represents the current standard of care for perioperative anesthesia for open renal surgery. However, potential catastrophic complications such as neuraxial bleeding and infection may occur. An alternative to neuraxial analgesia widely used in thoracic surgery is the surgeon-placed PVB.

OBJECTIVES: Laryngoceles are pathologic air filled dilations of the laryngeal ventricle. They are most often benign and incidental findings. Resection may be necessary in the setting of infection, airway obstruction, dysphagia, and phonatory disturbances. A bifid uvula, midline diastasis of the palatal muscles, and notching of the posterior hard palate have classically formed a triad diagnosing submucosal clefts. Pancreatic tumors represent a major field of application of intraoperative radiotherapy (IORT) with electron beams. 10 children between 3 and 9 years old, have been operated on for an echinococcosis cyst of the lung, by a thoracoscopic procedure. It was a vomited cyst in 6 cases, a pyopneumocyst in 1 case, and a jung noncomplicated and univesicular cyst in 3 cases. The surgical procedure is described. We brought into being the operative conditions such it is nowadays an excellent alternative to the classical thoracotomy.

Sigmoid volvulus is a leading cause of bowel obstruction in many parts of Africa. Several aspects of management remain controversial, particularly the operative strategy to be adopted in patients in whom sigmoidoscopy has failed to deflate viable bowel. On-table lavage is especially useful in these circumstances, allowing safe primary resection and anastomosis. This short review summarizes the theoretical and clinical background to such an approach.

Possible complications during colposuspension for urinary stress incontinence are inadvertent injury to the bladder or ureters. Surprisingly, endoscopic control during the procedure was never described. We suggest the routine use of cystoscopy during colposuspension for the early diagnosis of complications and immediate repair.

A 2-year-old girl with cyanotic congenital heart disease needed to undergo gastrostomy. To avoid the use of general anesthesia, a percutaneous gastrostomy was performed as an interventional radiologic procedure with the patient under sedation and with the use of local anesthesia. The placement of a nasogastrocutaneous guide wire under fluoroscopic guidance provided a stable tract for placement of the gastrostomy tube.
