Baseline Predictors of Discontinuation of Prescription Drug Therapy for IBS-C: Cohort Analysis at an Integrated Healthcare System.

BACKGROUND: Effective prescription drug treatment of constipation-predominant irritable bowel syndrome (IBS-C) requires patients to remain on daily therapy, yet predictive factors to optimize treatment selection are unknown. AIMS: We assessed whether common comorbidities including chronic overlapping pain conditions (COPCs), mood disorders, or concurrent medications influence the risk of discontinuing IBS-C prescription drug therapy. METHODS: We included all IBS-C patients who initiated treatment with the secretagogues linaclotide or lubiprostone across the Michigan Medicine healthcare syste

Validation of the English version of the Mood Rhythm Instrument.

BACKGROUND: Disruption of biological rhythms has been linked to the pathophysiology of mental disorders. However, little is known regarding the rhythmicity of mood symptoms due to the lack of validated clinical questionnaires. A better understanding of the rhythmicity of mood symptoms can help identifying individuals whose severity of mood symptoms follows an altered circadian rhythm. The objective of this study was to validate the English version of the Mood Rhythm Instrument (MRhI), a self-reported measure of self-perceived rhythmicity of mood symptoms and behaviours, in a sample of the gene

Invited commentary: How can we reconcile the findings of Keyes et al.'s study with the experience of our patients in clinical practice?

Although the accompanying study by Keyes et al. (Am J Epidemiol. 2013;178(9):1378-1388) shows us that women currently using hormonal contraception (HC) have better scores on the Center for Epidemiologic Studies Depression Scale and report fewer suicide attempts, it does not show us that HC protects women from mood disorders or that HC is free of the mood-related side effects which cause high rates of discontinuation. The groups compared in the Keyes et al. study were different in many ways; the women using HC were younger, were more likely to engage in positive health behaviors, and had lower

The relationship between impulsivity and panic disorder-agoraphobia: The role of affective temperament.

There are opinions regarding that impulsivity may play a role in the pathogenesis of neuropsychiatric disorders. The aim of this study was to investigate the relationship between impulsivity and panic disorder (PD) in the patient group, to compare impulsivity and affective temperamental traits between patients and healthy controls and to investigate whether there is a relationship between impulsivity and affective temperamental traits. Participants comprised 70 patients with PD and 58 healthy volunteers. The Panic Agoraphobia Scale (PAS), the Barratt Impulsiveness Scale (BIS-11) and the Temper

Secondary mania after ventral pontine infarction.

Secondary mania is increasingly recognized clinically, and consists of acute exhibition of manic symptoms without past or family history of affective disorder. It has been reported with toxic and metabolic disturbances, primary and metastatic brain tumors, epilepsy, and cerebrovascular events. A multifactorial etiology has been suggested. We report two men, 52 and 56 years old, who developed grandiosity, sleeplessness, irritable mood, hyperactivity, and paranoid and religious delusions, with attempted violence in one case. Both had no premorbid psychiatric history and were healthy except for h…

[The personality-modulated dissimulative behavior of patients with paranoid schizophrenia].

The revealed variants dissimulation in 120 mental patients at initial stage of schizophrenia stimulated evaluation of premorbid personality changes and their significance in the formation of dissimulative tendencies. 63 patients showed reticence, 49--affective instability, 8--increased irritability and unmotivated mood changes. Comparative analysis of clinical variants of dissimulation with the aspects of personality permit to suggest their importance in formation of behaviour motives in the development of the schizophrenia process at the paranoia stage.

Psychometric properties of the affective reactivity index in Australian adults and adolescents.

Irritable mood is implicated in a range of psychiatric conditions in both adults and adolescents. Research into appropriate measures of irritability, however, has been sparse. Recently, Stringaris et al. (2012) published the Affective Reactivity Index (ARI), a measure of chronic irritability with promising psychometric properties. This article presents psychometric properties of the ARI with Australian adolescents and, for the first time, with adults. The adolescent sample (n = 396) was recruited from 11 secondary schools in southeastern Australia. The adult sample (n = 221) was recruited thro…

Elevated amygdala responses to emotional faces in youths with chronic irritability or bipolar disorder.

A major controversy in child psychiatry is whether bipolar disorder (BD) presents in children as severe, non-episodic irritability (operationalized here as severe mood dysregulation, SMD), rather than with manic episodes as in adults. Both classic, episodic BD and SMD are severe mood disorders characterized by deficits in processing emotional stimuli. Neuroimaging techniques can be used to test whether the pathophysiology mediating these deficits are similar across the two phenotypes. Amygdala dysfunction during face emotion processing is well-documented in BD, but little is known about amygda

Understanding tonic and phasic irritability in developmental psychopathology among help-seeking children and adolescents in Switzerland: Protocol for the longitudinal multimodal UTOPICA study.

INTRODUCTION: Irritability represents one of the most common causes of referral to child and adolescent mental health services. Conceptually, tonic irritability (i.e., persistent grumpy mood) can be distinguished from phasic irritability (i.e., temper outbursts). The objective of this research project is to develop a fine-grained, ecologically valid and multimodal characterisation of tonic and phasic irritability to better understand the differential role of the two components in developmental psychopathology. METHODS AND ANALYSIS: The study has a longitudinal observational and experimental d

Clozapine for Severe Irritability in Children and Adolescents With Autism Spectrum Disorder: A Retrospective Case Series and Brief Literature Review.

BACKGROUND: Severe irritability in children and adolescents with autism spectrum disorder (ASD), including aggression toward others, self-injury, and object destruction, is a significant source of distress for individuals and may persist despite behavioral and psychosocial interventions. Although treatment with FDA-approved antipsychotics such as risperidone and aripiprazole is available, clozapine has been considered a potential option for the refractory cases. This study retrospectively examined the clinical response, tolerability, and safety of clozapine in children and adolescents with ASD

Prevalence of post-concussion-like symptoms in children and adolescents in Germany: a cross-sectional study.

BACKGROUND: Pediatric traumatic brain injury (TBI) is a major cause of disability and long-term sequelae in children and adolescents worldwide. To facilitate understanding of the emotional, cognitive, and somatic symptoms following pediatric TBI, the present study aims to assess the prevalence of post-concussion-like symptoms (PCLS) in children and adolescents aged 8 to 17 years in Germany and to investigate the associations between risk factors and the occurrence of symptoms. METHODS: A web-based survey included two age-appropriate versions of the Postconcussion Symptom Inventory (PCSI; for c

Therapeutic Effects of Dietary Supplements on Emotion-Related Symptoms in Children and Adolescents with Autism: A Systematic Review and Meta-analysis.

BACKGROUND: Autism spectrum disorder (ASD) is frequently accompanied by emotional symptoms such as irritability and anxiety, which significantly impact quality of life. Dietary supplements have been proposed as potential interventions, but their efficacy for emotion-related symptoms in children and adolescents with ASD remains unclear. OBJECTIVES: This systematic review and meta-analysis aimed to evaluate the efficacy of various dietary supplements in improving emotion-related symptoms in children and adolescents with ASD, and to explore differences across supplement types. Emotion regulation as central to psychopathology across childhood and adolescence: a commentary on Nobakht et al. (2023).

An important goal of clinical/developmental research is to identify factors contributing to the onset and maintenance of psychopathology - particularly factors that could be modified through intervention. Large-scale, multi-informant, longitudinal studies provide valuable opportunities for testing such etiological hypotheses, as illustrated by Nobakht et al.'s recent six-wave cohort study spanning ages 4-14. At a within-person level, emotion regulation (ER) deficits consistently predicted oppositional defiant disorder (ODD) symptoms (including both irritability and defiance), whereas victimiza

Irritability, Defiant and Obsessive-Compulsive Problems Development from Childhood to Adolescence.

Little is known about the coexistence of oppositionality and obsessive-compulsive problems (OCP) in community children and how it affects their development until adolescence to prevent possible dysfunctions. The co-development of oppositional defiant dimensions and OCP is studied in 563 children (49.7% female) from ages 6 to 13 years, assessed yearly with measures answered by parents and teachers. A 4-class model based on Latent Class Growth Analysis for three parallel processes (irritability, defiant, and OCP) was selected, which showed adequate fitting indexes. Class 1 (n = 349, 62.0%) child

The Behavioral and Psychological Symptoms of Dementia in Down Syndrome Scale (BPSD-DS II): Optimization and Further Validation.

BACKGROUND: People with Down syndrome (DS) are at high risk to develop Alzheimer's disease dementia (AD). Behavioral and psychological symptoms of dementia (BPSD) are common and may also serve as early signals for dementia. However, comprehensive evaluation scales for BPSD, adapted to DS, are lacking. Therefore, we previously developed the BPSD-DS scale to identify behavioral changes between the last six months and pre-existing life-long characteristic behavior. OBJECTIVE: To optimize and further study the scale (discriminative ability and reliability) in a large representative DS study popul

Preschool Irritability Predicts Adolescent Psychopathology and Functional Impairment: A 12-Year Prospective Study.

OBJECTIVE: The long-term clinical and functional outcomes of preschool-age irritability are unknown. This study examined longitudinal associations of preschool irritability with psychiatric disorders and functional impairment assessed in adolescence in a large community sample. METHOD: A total of 453 children were assessed at age 3 and again at ages 12 and/or 15. At age 3, parents were interviewed about their child's irritability, other psychiatric symptoms, and functional impairment with the Preschool Age Psychiatric Assessment (PAPA). In adolescence, both parents and youths were interviewed

Transdiagnostic trajectories of irritability and oppositional, depression and anxiety problems from preschool to early adolescence.

Irritability is a transdiagnostic symptom in oppositional defiant disorder, depression, and anxiety, all highly comorbid disorders but with unknown simultaneous evolution. The aim was to obtain the developmental trajectories of simultaneous irritability and oppositional, depression, and anxiety problems from preschool age to early adolescence. A sample of 493 community children was followed up annually from ages 3-11 years and assessed using categorical and dimensional measures answered by parents and teachers. Latent Class Growth Analysis for four parallel processes was used to identify disti

Anxious-Irritable Children: A Distinct Subtype of Childhood Anxiety?

Children with anxiety disorders often present with other co-occurring symptom clusters, of which irritability is among the most highly co-occurring. Despite compelling clinical and pathophysiological evidence linking anxiety and irritability, little is known regarding the clinical presentation and associated impairment of children with both anxiety and irritability. In this study, our aims were to confirm the preponderance of irritability in clinically anxious children and compare clinically anxious children with irritability to those without irritability across sociodemographic, clinical, psy
