The nationwide epidemiological survey of syringomyelia was carried out in Japan by sending inquiries to neurologists, child neurologists, neurosurgeons and orthopedic surgeons for the period of 1991 and 1992. The clinical and biochemical features of patients with autoimmune hepatitis (AIH) in Japan during the 15-year period, 1975 to 1990, were evaluated by a nationwide survey using a questionnaire prepared by the Intractable Diseases Study Group organized by the Ministry of Health and Welfare. We evaluated the prevalence of Coxiella burnetii antibodies in 626 human serum samples (275 from veterinarians, 107 from meat-processing workers, 184 from respiratory-disorder patients, and 60 from healthy humans) by the indirect immunofluorescence test. Of the serum samples examined, 54 (8.6%) and 103 (16.5%) reacted positively to phase I and II antigens, respectively, of C. burnetii. The rates differed for healthy humans and respiratory-disorder patients. Antibody prevalence was high for healthy humans living in close contact with animals (e.g., veterinarians and meat-processing workers).

Two cases of tsutsugamushi disease misdiagnosed initially as drug eruption were reported. Clinical symptoms of both cases disappeared dramatically after starting minocycline. Statistical examinations were performed on 29 cases of tsutsugamushi disease, including those observed in Mie Prefecture since 1982. Half of them were seen in the last 2 years. The onset was predominantly recorded in November (59%). Presumptive sites of infection were forests (63%) and fields (21%). No patients were infected along river-banks.

Consecutive 431 patients with B-cell lymphoma seen in Aichi Cancer Center Hospital from 1964 to 1988 were analysed. Median age was 56 yr (range 8-86). There were 76 patients (17.6%) with follicular lymphoma and 355 with diffuse lymphoma. Among the 30 patients with follicular lymphoma, 10 (33%) were found to have a bcl-2 rearrangement. Two nationwide questionnaire surveys of intestinal obstruction in Japan were undertaken, covering two two-year periods, from January, 1975 to December, 1976 and from January, 1985 to December, 1986, respectively. A 2-year-old girl living in southwestern Japan had a nodule of 2 months' duration on the left foot. A biopsy from the lesion showed transverse sections of a worm surrounded by granulomatous tissue. The worm was identified as an Onchocerca sp. from the morphological characteristics such as relatively thick cuticles, annular ridges on the cortical layer, and high somatic muscles. We recently encountered a case of intrabronchial neurilemmoma. The patient was a 15-year-old boy who had a wheeze. After observation with a bronchofiberscope, a wedge resection of the left main bronchus was performed. No postoperative complications nor recurrence was seen. To date, five cases of intrabronchial neurilemmoma have been reported in the Japanese Journals. Surgical removal was performed in 4 of 5 and bronchofiberscopic excision in 1. Children can maintain their normal life cycle by out-patient treatment. There are two main complications of childhood asthma. Fourteen persons have been diagnosed Bloom's syndrome in Japan, with cytological verification in 11. This paper is primarily concerned with environmental risks arising from utilizing modern technology. Our attention focuses on the interface between policy decisions and regulatory measures. The interface involves not only science and technology but also politics and social values, and perceptions which are deeply rooted in the industrial countries. A case of setting air quality standards and regulatory practices in Japan is studied on the basis of scientific and technological knowledge, economic cost, and subjective judgment of what constitutes an acceptable level of risk or health hazard.
