A 14.5-year-old girl with Fraley's syndrome, which caused left flank pain and massive haematuria with anaemia underwent left renal surgery. The infundibulum for the left upper calyx group was entrapped between the lower segmental renal artery and one branch of the renal vein. Despite other known surgical procedures, the surgeon explored the area around the entrapped infundibulum and resected the vein. Nephrosonographic checks were made on 25 women during pregnancy (prior to and after treatment) and after childbirth, in order to obtain morphological information on dynamics of upper urinary tract in cases of acute pyelonephritis gravidarum. Pyelectasia was recorded from 19 patients (76 per cent) on the right side, one patient (four per cent) on the left, two patients (eight per cent) on either side, and was not recorded at all from three women (twelve per cent). Moderate to severe dilatations had been more often recordable from patients with high temperature, but not as often from mild cases.

The excretory urograms performed on 1716 children and 3480 adults have been examined to find the incidence and complications of renal duplication. Ninety-five patients with duplication were found, unilateral in 79 and bilateral in 16 patients. It was equally common on each side and twice as common in females as in males. Non-duplex kidneys had a mean of 9.4 calyces and duplex kidneys had a mean 3.7 upper and 7-6 calyces in lower moieties. In 102 patients with ectasia of the renal calyx from degree I to III a pyelograft after Anderson-Hynes was performed. 35 children and adolescents were submitted to ureterocalicostomy for PUJ obstruction. Since the position of the longitudinal axis of the renal pelvis is of special significance in the diagnosis of the renal masses, the physiological range of the angle formed by this axis and the spine was determined by retrospective evaluation of 516 urograms. Stones of the renal pelvis can be treated either by extracorporeal shock wave lithotripsy (SWL) or percutaneous nephrolithotomy (PCNL). BACKGROUND: Megacalycosis is a rare non-obstructive congenital anomaly of the kidneys. The underlying pathogenesis has not yet been understood.
