Blood glucose and insulin levels, thyroid function, and serology in Ménière's disease, recurrent vestibulopathy, and psychogenic vertigo.

Nineteen patients with unilateral Ménière's disease, 20 with psychogenic dizziness, and 20 with recurrent vestibulopathy (diagnostic criteria in text of paper) were found to have normal five hour glucose tolerance tests, serum thyroxine and effective thyroid indices, and serologic tests for syphilis. Hypothyroidism and hypoglycemia were absent in all groups. An unexplained finding of each diagnostic group was significant increase of fasting blood glucose and insulin levels, and elevated insulin:glucose ratios, compared to a control group. There appears to be no diagnostic indication for perfor

Magnetic Resonance Imaging in Prediction of Sensorineural Hearing Loss in Patients with Neuroinfections.

BACKGROUND: Magnetic resonance imaging (MRI) may be useful in detecting labyrinthitis and thereby predicting the development of sensorineural hearing loss (SNHL) in adults with central nervous system (CNS) infections. We therefore investigated the coherence between brain MRI and SNHL among adults with CNS infections. METHODS: Twenty-eight patients with bacterial or viral meningitis, viral encephalitis, or Lyme neuroborreliosis, who had a brain MRI during the acute disease and pure-tone audiometry at follow-up, were included. Neuroradiologists were blinded to the audiometric outcome when ratin

Idiopathic bilateral vestibulopathy: an autoimmune disease?

Bilateral vestibulopathy (BV) is the loss of function of both peripheral labyrinths or of the eighth nerves. Its etiology remains obscure in approximately 20% to 50% of cases (so-called idiopathic bilateral vestibulopathy, IBV). Alternatively, the cause could be viral or vascular; to date, causative gene mutations have not been identified. Other potential disease mechanisms include autoimmune disorders. Antibodies have been detected against inner ear tissue (primarily against vestibular membranous labyrinth). The data suggest that the bulk of anti-labyrinthine autoantibodies may be an epipheno

Middle ear exploration results in suspected otosclerosis cases: are ossicular and footplate area anomalies rare?

INTRODUCTION: Otosclerosis is a disease of bony labyrinth. Structural changes in the labyrinth often cause ossicular fixation, and thus conductive hearing loss. The purpose of this study was to evaluate middle ear exploration findings and frequency of ossicular and footplate area anomalies in patients with suspected otosclerosis referred to Amiralmomenin and Golsar Hospitals in Rasht, Iran. MATERIALS AND METHODS: In 47 patients undergone middle ear exploration in Amiralmomenin and Golsar hospitals from April 2001 to March 2011, the intraoperative findings, and other data were extracted from t

Nitrogen and oxygen molecules in meningitis-associated labyrinthitis and hearing impairment.

Pneumococcal meningitis remains a serious disease with a case fatality rate of 15%-25%. Furthermore, long-term residues affect up to 50% of survivors. One of the most frequent sequelae is sensorineural hearing loss, which occurs in 26% of survivors of pneumococcal meningitis. Unfortunately, sufficient treatment regimens are still missing. New insights into the pathology and pathophysiology of meningitis-associated hearing loss have come from animal models of bacterial meningitis. Most likely, bacteria reach the cochlea through the cochlear aquaeduct. Once arrived in the perilymphatic spaces, t

[The variation of the vestibular response vs. the variations of the auditory threshold after administration of glycerol in patients with Meniere's disease].

Vestibular and auditory functions after glycerol intake were assessed correlated in twenty-eight patients diagnosed as menieric according to AAOO criteria. All patients had unilateral Ménière's disease confirmed by monolateral hypoacusia and labyrinthine hypofunction. Hearing thresholds were tested by tone-audiometry, according to the standard glycerol test. Variation in vestibular response were assessed comparing the VOR during the rotatory sinusoidal test as well as the caloric test, before glycerol intake and 2 hours after drug administration. Slow phase angolar velocity, directional prepon

Rare Disorders of the Vestibular Labyrinth: of Zebras, Chameleons and Wolves in Sheep's Clothing.

The differential diagnosis of vertigo syndromes is a challenging issue, as many - and in particular - rare disorders of the vestibular labyrinth can hide behind the very common symptoms of "vertigo" and "dizziness". The following article presents an overview of those rare disorders of the balance organ that are of special interest for the otorhinolaryngologist dealing with vertigo disorders. For a better orientation, these disorders are categorized as acute (AVS), episodic (EVS) and chronic vestibular syndromes (CVS) according to their clinical presentation. The main focus lies on EVS sorted b

Cochlear fistula in a noncholesteatomatous ear.

Bony destruction of the labyrinth is usually associated with long-standing cholesteatomatous otitis media. The promontory is not a common site for bone resorption because (1) it is not an area that is involved in accumulation of cholesteatoma perimatrix substances, (2) it is the densest bone of the human body, and (3) pressure necrosis from overlying tissue is uncommon. We report a case of cochlear erosion associated with noncholesteatomatous middle ear disease. As far as we know, this is only the second such case reported in the literature. We also review decision-making factors and technique

[The influence of the cervical proprioception on the postural reflexes--experimental and clinical study].

The aim of the study was to estimate the effect of the neck proprioception on the postural reflexes in the cases of the cervical pathology in the whiplash injury and degenerative disease of the cervical vertebral column. The investigation using ENG and stabilometry was performed in 81 patients. Experimental observations were done among 10 pigeons with limitation of the neck proprioceptors action by special neck tutor. Their postural reflexes were analysed during balance test on the seesaw. Our conclusion is, that disorders of the proprioceptors impulastion can damage the postural reflexes even

[Fatal complications of otitis 60 years ago].

The temporal bone collection of the ENT Department of the Charité Hospital in Berlin contains 128 cases from 1924 to 1941 of fatal complications of acute otitis media and cholesteatoma. In 1927 alone, one patient died on average every 2 weeks at Charité Hospital from fatal otitic disease. Lesions include labyrinthitis, meningitis, sinus thrombosis and brain abscess. Each case is documented by clinical history, autopsy findings and temporal bone sections. The reasons for the various features of each illness are briefly discussed.

Pathology of Ménière's disease as it relates to the sac and tack procedures.

The rationale for endolymphatic sac and tack operations is to prevent the further accumulation of endolymph, the former by draining endolymph into extralabyrinthine tissues and the latter by shunting endolymph into the perilymphatic space. While the basic concepts are reasonable enough, the probability of achieving these objectives seems remote. There appears to be adequate clinical evidence, however, to show that these procedures are of therapeutic value in selected cases. It is tempting to speculate that these surgical insults to the labyrinth, with the associated inflammatory and biochemica

Tuberculous otitis media.

Tuberculous otitis media is a variable and puzzling infectious disease which may remain undiagnosed or confused with other acute or chronic middle ear conditions if appropriate diagnostic studies are not performed. Among the common clinical findings are insiduous onset of painless watery drainage, severe hearing loss, multiple perforations of the tympanic membrane and subsequent appearance of profuse granulation tissue. As the disease progresses destruction of the middle ear conductive apparatus, facial paralysis, cochlear involvement with labyrinthitis and further hearing loss, and finally in

Vertigo: assessment and management.

Elucidation of the cause of vertigo must consist of an adequate history to see if a clinical entity such as BPPV or Meniere's Disease can be recognised, and a general physical examination with emphasis on: The ear--discharge, perforation, fistula sign and hearing. Vestibular system--nystagmus and Romberg test with past pointing and positional testing. CNS--cranial nerves, cerebellar signs and gait. CVS--BP and bruits in neck and cardiac irregularities. Audiogram, ENG and CT or MRI. The specific treatment must be supplemented by sympathetic, symptomatic therapy and reassurance that the conditio

[Clinical case of angiosarcoma of ethmoidal labyrinth and frontal sinus, issues of morphological verification of diagnosis].

A clinical case of incorrect verification of the angiosarcoma of the ethmoidal labyrinth and frontal sinus in a 36-year-old man is presented. A feature of this case is the disregard of additional research methods without taking into account the unilateralism of the pathological process, which led to the wrong tactics of surgical treatment and the development of a relapse of the disease complicated by nasal liquorrhea. A thorough examination of the patient and an analysis of a series of computer tomograms of the paranasal sinuses suggested a diagnosis of malignancy. The patient underwent neopla
