The aim of this study was to audit the introduction of the use of the CO2 laser into our department and to compare hearing outcomes and complication rates in patients who underwent either laser or mechanical stapedectomy. Laser treatment in carcinoma of the bronchus is essentially palliative and is suitable for only a few patients. Patients selected for laser treatment must have predominantly endobronchial growth with normal bronchial anatomy still identifiable and symptoms due to the obstruction or to haemoptysis. Five women undergoing endometrial ablation with the Nd-YAG laser developed sudden gas embolism. In each case laser intrauterine contact surgery was carried out utilizing coaxial fibers with (three cases) and without (two cases) artificial sapphire tips but commonly cooled by air or nitrogen. Four of the five women died in a sequence of sudden cardiovascular collapse followed by irreversible cardiac arrest. One woman was critically ill and following prolonged hospitalization survived with neurological deficits.

We demonstrate the high sensitivity of gas sensing using a novel air-guiding photonic bandgap fiber. The bandgap fiber is spliced to a standard single-mode fiber at the input end for easy coupling and filled with gas through the other end placed in a vacuum chamber. The technique is applied to characterize absorption lines of acetylene and hydrogen cyanide employing a tunable laser as light source. Measurements with a LED are also performed for comparison. Detection of weakly absorbing gases such as methane and ammonia is explored.
