INTRODUCTION: Palmo-plantar Psoriasis is a non-infectious chronic inflammatory disease of the skin bounded to the palm and soles in extent. In Ayurveda, all skin diseases are recounted under a single rubric i.e., Kushtha (skin diseases); PPP (Palmo-plantar Psoriasis) may be correspondent with Vipadika (one of the Kshudra Kushtha described in Ayurveda), based on its clinical sign & symptom.

Two cases of thumb replantation and one of finger revascularization complicated by Aeromonas hydrophila infection are reported. Two digits were lost because of infection in soft tissue and osteomyelitis. One thumb had extensive necrosis. In all cases the infection was difficult to eradicate, probably because of ischemia. All three patients sustained their injuries while cutting meat or fish. These infection sources have not been previously reported. Note is made of Aeromonas hydrophila in medicinal leeches that are used in microvascular surgery and the potential for iatrogenic infection.

The term cutaneous pseudolymphoma (C-PSL) is defined in the literature as a benign, reactive lymphoproliferation that clinically and/or histopathologically imitates cutaneous lymphoma. The authors present a case of successful replantation of a totally avulsed ear in a 37-year-old patient, using a termino-terminal arterial anastomosis on the superficial temporal artery. As no suitable vein could be identified in the amputated part, they relied on leech therapy and systemic anticoagulation for venous drainage. Despite an 18-hr ischemia, the ear survived completely.

Venous congestion after microvascular replantation remains a formidable problem even under ideal circumstances. A 75 years old lady from the Dhaherah Province, Oman, presented at Ibri Hospital, complaining from abnormal vaginal bleeding. Histopathological investigations of a cervical biopsy revealed no atypical features. A worm-like organism was removed from the vagina. Histocytological investigations of the organism revealed typical structures of a leech. The case was identified as vaginal hirudiniasis, probably due to Limnatis nilotica that necessitated a note on internal hirudiniasis.

Choice of operative technique is often a major factor in the success of replantation of traumatic avulsions of the auricle. While microsurgery is considered to produce the best aesthetic results, this approach may not be an option due to vascular damage. An adenylate cyclase activity stimulated by serotonin and calmodulin is present in the segmental ganglia of the leech Hirudo medicinalis. Removal of the endogenous calcium binding protein does not alter the responsiveness of the enzyme to serotonin. The calmodulin antagonist, trifluoperazine, drastically reduces the amine stimulatory effect on both intact and calmodulin-depleted membranes. We suggest that calmodulin-sensitive and serotonin-stimulated adenylate cyclase are, at least functionally, independent.
