Bone Density, Geometry, and Mass by Peripheral Quantitative Computed Tomography and Bone Turnover Markers in Children with Diabetes Mellitus Type 1.

BACKGROUND: The type 1 diabetes mellitus (T1DM) is a chronic systemic autoimmune-mediated disease characterised by the insulin deficiency and hyperglycaemia. Its deleterious effect on bones concerns not only bone mass, density, and fracture risk but also may involve the linear growth of long bones. Studies on the lower leg in children with T1DM by pQCT have generated conflicting results, and most of the studies published so far focused only on a selected features of the bone. An additional information about growth, modelling, and remodelling processes can be gathered by the bone turnover marke

Spinal decompression and vertebroplasty in Paget's disease of the spine.

Paget's disease is an osteometabolic disorder affecting in particular long bones. The spine is the second most commonly involved site in Paget's disease. This pathology can cause low back pain, spinal stenosis, myeloradiculopathy, and vertebral collapse. Medical therapy is the first choice for the treatment of Paget's disease of the spine; in case of failure, surgery remains a valid option. In the present article, we report a case of a patient with leg disability due to myelopathy caused by spinal Paget's disease treated with spinal decompression and vertebroplasty. To our knowledge, this is t

Lower limb deformities in children: two-stage correction using the Taylor spatial frame.

The Taylor spatial frame is a new external ring fixation system developed for correction of axial, sagittal, planar and rotational deformities of the extremities. Between August 1999 and October 2000 five children were treated by this system. Four had shortened and deformed femurs after fracture, osteomyelitis or a congenital short femur. One patient had a pseudoarthrosis of the tibia. Mean corrections were leg lengthening 5.9 cm (1.7-7.2), valgus correction 10.6 degrees (0-19), lateral translation 12.6 mm (0-33) and rotation 10 degrees. The mean healing index was 48.4 days/cm. In all patients

Callus formation in femur and tibia during leg lengthening: 7 patients examined with DXA.

We examined the callus formation during leg lengthening in 7 achondroplastic patients who underwent 3 bilateral femoral and 4 bilateral tibial lengthenings. Bone mineral content and bone mineral density (BMD) in the lengthened callus space were evaluated every 1 or 2 weeks for 10 weeks after the start of distraction using dual energy X-ray absorptiometry. The mean rate of callus mineralization in femurs (0.64 g/wk) was higher than in tibias (0.22 g/wk). The mean BMD at 10 weeks after the start was 0.35 g/cm2 in the femur and 0.14 g/cm2 in the tibia. Different rates of callus formation in diffe

Robotically assisted long bone biopsy under MRI: cadaver study results.

RATIONALE AND OBJECTIVES: We have designed and constructed an MR-safe robot made entirely of nonmetallic components with pneumatic actuators and optical encoders. The robot was developed to enable bone biopsies to be performed under magnetic resonance imaging (MRI) guidance in pediatric patients. The purpose of this study was to show the feasibility of using the robot for biopsy of the femur and tibia in a cadaver leg. Our long-term goal is to eliminate radiation exposure during bone biopsy procedures and provide more timely and accurate diagnosis for children with bone cancers and bone infect

Surgical correction of short stature in skeletal dysplasias.

Limb lengthening has been undertaken in patients with short stature, including achondroplasia, hypochondroplasia and other skeletal dysplasias. The procedure consists of osteotomy of the tibia and fibula, and fixation of the limb using an external unilateral or circular fixator. Distraction (elongation) begins 1 week postoperatively, and during distraction osseous autoregeneration occurs. Consolidation of the autoregenerate takes about twice the time needed for lengthening, and the fixator is gradually mobilized during this time to allow greater weight-bearing. In 26 patients with short statur

Impact of low-load resistance exercise with and without blood flow restriction on muscle strength, endurance, and oxidative capacity: A pilot study.

Low-load resistance exercise (LLRE) to failure can increase muscle mass, strength, endurance, and mitochondrial oxidative capacity (OXPHOS). However, the impact of adding blood flow restriction to low-load resistance exercise (LLBFR) when matched for volume on these outcomes is incompletely understood. This pilot study examined the impact of 6 weeks of single-legged LLBFR and volume-matched LLRE on thigh bone-free lean mass, strength, endurance, and mitochondrial OXPHOS. Twenty (12 males and 8 females) untrained young adults (mean ± SD; 21 ± 2 years, 168 ± 11 cm, 68 ± 12 kg) completed 6 weeks

Recurrence from the Spinal Region of the Patient Whose Treatment Was Completed with Liver and Lung Cystic Echinococcosis: A Rare Pediatric Case of Spinal Cystic Echinococcosis.

Cystic echinococcosis is a parasitic disease caused by the Echinococcus tapeworm. The disease can often affect organs such as the liver and lungs, muscles, bones, kidneys, brain, and spleen. Spinal cystic echinococcosis has been reported very rarely in the literature. In this report; we present a pediatric case with spinal cystic echinococcosis, who was diagnosed with multiple cystic echinococcosis in the liver and lungs and was admitted with complaints of difficulty in walking and leg pain 1 year after the albendazole treatment, which he had been taking for 3.5 years. If a diagnosis of cystic

["Adamantinoma" of the tibia and reactive bone changes (author's transl)].

A case of "adamantinoma" of the tibia is reported. 10 years ago a giant tumor with fibrous dysplasia had been diagnosed in a biopsy taken from the same site of cystic bone changes. These findings may occur as reaction in the periphery of "adamantinomas" of long bones. By this experience the importance was emphasized, to obtain tissue from the center portion of the tumor to include the typical epithelial islets pathognomonic for "adamantinomas", which in our case, were found only 10 years after the first operation. Our patient was treated by curetting and filling of the defect with bony splinte

[Treatment of a bone bridge by transplantation of mesenchymal stem cells and chondrocytes in a composite scaffold in pigs: experimental study].

PURPOSE OF THE STUDY: The presented experimental study describes the results of using a combination of allogeneic mesenchymal cells (MSCs) with chondrocytes (CHCs) and a novel scaffold based on type I collagen and chitosan fibres. This biocomposite was transplanted into a defect produced by excision of a bone bridge to induce new cartilaginous tissue formation. The left femur was treated by transplantation into a defect of distal epiphysis; the right femur with implantation of the scaffold only served as control. A better therapeutic result was therefore expected in the left femur - the reduct

Osteochondroma of the iliac crest as a mimicker of prolapsed intervertebral disc- A case report and review of literature.

Osteochondromas of the pelvis are benign bone tumors. They might be accidently discovered during imaging studies and can stay inactive without causing any symptoms. However, as they grow larger, they may cause compressive symptoms in adjacent areas. A 17-year-old male presented with a history of back strain sustained while participating in sports. He experienced shooting pain radiating down his left leg, following the L4 dermatome pattern. Nerve irritation tests, such as the straight leg raise test and reported paresthesia in the L4 dermatome, were positive. The classic symptoms and clinical f

Exploring the morphology of adult tibia and fibula from Sima de los Huesos site in sierra de Atapuerca, Burgos, Spain.

The analysis of the locomotor anatomy of Late Pleistocene Homo has largely focused on changes in proximal femur and pelvic morphologies, with much attention centered on the emergence of modern humans. Although much of the focus has been on changes in the proximal femur, some research has also been conducted on tibiae and, to a lesser extent, fibulae. With this in mind, we present one of the largest samples of the same population of human tibiae and fibulae from the Middle Pleistocene to determine their main characteristic traits and establish similarities and differences, primarily with those

Prophylactic effect of plaster and cataplasm contained ketoprofen in rats with adjuvant arthritis.

This study demonstrates that prophylactic effect of plaster and cataplasm contained ketoprofen in adjuvant arthritis therapy by X-ray. Adjuvant arthritis was induced by a single injection of Freund's complete adjuvant. Mature female Sprague-Dawley rats were designated to 3 groups such as nontreated control, plaster-treated (PT) and cataplasm-treated (CT), each of which was composed of ten animals. The PT and the CT groups showed reduced primary paw swelling, but secondary paw swelling was not affected. Bony changes were observed in all regions of the femur and tibia of the nonadjuvant-injected

Predictability of sacral base levelness based on iliac crest measurements.

A level sacral base plane is necessary to allow normalization of complex lumbosacral mechanics. Palpatory examinations are often used to evaluate for leg length discrepancy and pelvic obliquity despite improved accuracy and consistency of radiographic techniques. Treatment based on palpatory examinations suppose a direct and consistent relationship between the pelvic bones (innominates) and the sacral base. To evaluate the relationship between iliac crest levelness and sacral base levelness, a radiographic postural survey in the upright, weight-bearing position was performed on 358 men and wom

Efficacy of PRECICE Nail in Treatment of Adult Patients With Posttraumatic Femoral Leg Length Discrepancy.

Posttraumatic, limb length discrepancy in adults is a challenge to treat, and multiple treatment protocols over the years have shown varying levels of success and complications. Before the introduction of the PRECICE nail in 2011, our preferred method of limb lengthening used an Ilizarov or Taylor Spatial frame. To assess the PRECICE nail, we evaluated the accuracy and complications during treatment in a series of skeletally mature patients with posttraumatic femoral limb length discrepancy. The surgical technique along with a case series of 8 patients are described in detail. On average, the

Recovery of decreased bone mineral mass after lower-limb fractures in adolescents.

BACKGROUND: Loss of bone mineral mass, muscle atrophy, and functional limitations are predictable consequences of immobilization and subsequent weight-bearing restriction due to leg or ankle fractures. The aim of this study was to prospectively determine whether decreased bone mineral mass following lower-limb fractures recovers at follow-up durations of six and eighteen months in adolescents. METHODS: In the present study, we included fifty adolescents who underwent cast immobilization for a leg or ankle fracture. Dual x-ray absorptiometry scans of four different sites (total hip, femoral ne

Heterogeneity in the growth of the axial and appendicular skeleton in boys: implications for the pathogenesis of bone fragility in men.

Men with spine fractures have reduced vertebral body (VB) volume and volumetric bone mineral density (vBMD). Men with hip fractures have reduced femoral neck (FN) volume and vBMD, site-specific deficits that may have their origins in growth. To describe the tempo of growth in regional bone size, bone mineral content (BMC), and vBMD, we measured bone length, periosteal and endocortical diameters, BMC, and vBMD using dual-energy X-ray absorptiometry in 184 boys aged between 7 and 17 years. Before puberty, growth was more rapid in the legs than in the trunk. During puberty, leg growth slowed whil
