Transitions in Technology-Mediated Cardiac Rehabilitation and Self-management: Qualitative Study Using the Theoretical Domains Framework.

BACKGROUND: An acute cardiac incident is a life-changing event that often necessitates surgery. Although surgery has high success rates, rehabilitation, behavioral changes, and self-care are critical to long-term health. Recent systematic reviews have highlighted the potential of technology in this area; however, significant shortcomings have also been identified, particularly with regard to patient experience. OBJECTIVE: This study aims to improve future systems and to explore the experiences of cardiac patients during key phases after hospitalization: recuperation, initial rehabilitation, a

Quality standards for the diagnosis and management of ATTR and AL amyloidosis: a nominal group technique study.

BACKGROUND: Transthyretin (ATTR) and immunoglobulin light-chain (AL) amyloidosis are progressive conditions that can have debilitating effects on people. Delayed diagnosis and variable access to high-quality care continue to affect quality of life and prognosis. Clear and ambitious quality standards could drive consistent care and improve outcomes. METHODS: A modified nominal group technique was used to develop consensus-based quality standards for the diagnosis and management of ATTR and AL amyloidosis. Eighteen experts participated, including patient representatives and clinicians from high

Similarities and Differences Between the Extravascular and the Subcutaneous Implantable Cardioverter-Defibrillator: Sensing, Discrimination, and Complications (Part 2).

PURPOSE OF REVIEW: The subcutaneous ICD (S-ICD) and the extravascular ICD (EV-ICD) both avoid intravascular hardware, but they solve sensing and detection differently. This review compares their sensing architectures, detection and discrimination algorithms, therapy capabilities, and system-specific complications, along with practical considerations including extraction, MRI compatibility, and patient-reported outcomes. RECENT FINDINGS: Inappropriate shock rates have declined substantially for both platforms through algorithmic refinements, two-zone programming and SMART Pass for S-ICD, and i

Reprogramming our fate: a postdoctoral reflection on current challenges and prospects for developmental biology.

Developmental biology stands at a crossroads. While some have suggested the field is in decline, we, early-career developmental biologists, see an era of renewal driven by conceptual expansion, technical innovation, and cross-disciplinary integration. In this Commentary, we reflect on discussions from a 2024 workshop of postdoctoral scholars from across North America, outlining shared challenges and opportunities that will shape the field's future. We argue that the perceived crisis in developmental biology stems not from a lack of relevance, but from a narrow definition that overlooks its bro

The Implications of Metabolic and Bariatric Surgery on Psychosocial and Relational Health: A Narrative Review.

Obesity is a globally prevalent condition associated with elevated morbidity and mortality. Metabolic and bariatric surgery offers a definitive treatment for class III (BMI > 40) obesity, achieving substantial, enduring weight loss and improving metabolic health. Despite extensive research on the physical benefits, comparatively fewer reviews investigate the psychosocial and relational changes accompanying these procedures. This narrative review examines how such procedures affect partner relationship quality, sexual function, and broader social integration, aiming to synthesise current findin

Space aquatic chemistry: A roadmap for drinking water treatment in microgravity.

Rapid advancement in aerospace technology has successfully enabled long-term life and economic activities in space, particularly in Low Earth Orbit (LEO), extending up to 2000 km from the mean sea level. However, the sustainance of the LEO Economy and its Environmental Control and Life Support System (ECLSS) still relies on a regular cargo supply of essential commodities (e.g., water, food) from Earth, for which there still is a lack of adequate and sustainable technologies. One key challenge in this context is developing water treatment technologies and standards that can perform effectively

From the streets to assisted living: perceptions of a vulnerable population.

The rapid growth of assisted-living facilities is paralleled by the necessity to understand the needs of the people living in them. A hallmark challenge for individuals who are poor and disabled, and often marginalized from mainstream society, is maintaining integrity and being a whole person, rather than a sum of broken parts. A key to maintaining this integrity is the ability to find stable housing and support systems. The inner-city assisted-living facility in this study is unique in that all of its residents are funded by Medicaid. The residents have complex needs related to histories of h

[Care for dementia patients by their families and home care].

Present problems concerning home care for the aged including the following three points: 1. The extent of the period during which care is needed has been prolonged as the aged live longer than in previous times. 2. The range of need for home care has increased. 3. As a result of the increase of the individual-dependent "life support infrastructure" due to the decreased role of the family-dependent "life support infrastructure", the aged, who were more self-supporting in past, have become more dependent on care both in quality and quantity. In particular, dementia patients gradually worsen, the

Impact of hetastarch on the intestinal microvascular barrier during ECLS.

The effects of hetastarch on microvascular fluid flux were determined in anesthetized dogs undergoing extracorporeal life support (ECLS) with a roller pump and membrane oxygenator. ECLS with a lactated Ringer priming solution resulted in a decrease in microvascular protein reflection coefficient and an increase in transvascular protein clearance. Use of a 6% hetastarch priming solution attenuated the decrease in microvascular protein reflection coefficient and blunted the increase in transvascular protein clearance. Ileal tissue water increased in the group treated with the lactated Ringer pri

Patient-reported outcome measures in palliative care: A systematic review to inform health policy and health system performance.

BACKGROUND: Health systems increasingly recognise palliative care as an essential component of universal health coverage. Assessing palliative care quality and value remains challenging, as key outcomes are often missed by routine indicators. Patient-reported outcome measures (PROMs) can address this gap, but their use remains fragmented. OBJECTIVE: To synthesise evidence on patient-reported outcomes in palliative care, the PROMs used to measure them, and implementation barriers and facilitators. METHODS: Five databases (PubMed, Web of Science, Scopus, CINAHL, and the Cochrane Library were s

Autonomic and attentional pathways in the emergence of autism: bridging mechanisms and real-world contexts in infancy.

Autism spectrum disorder (ASD) unfolds over the first two years of life through complex interactions among developmental systems. Attention and autonomic nervous system (ANS) regulation represent foundational processes critical for adaptive engagement with the environment. Disruptions in these systems during early infancy may initiate developmental cascades that contribute to core ASD features, including social-communication challenges and restricted and repetitive behaviors, as well as the vast heterogeneity found within ASD. This chapter reviews our approach to integrating mechanistic resear

The electronic health record Risk of Alzheimer's and Dementia Assessment Rule (eRADAR) Brain Health Trial: Protocol for an embedded, pragmatic clinical trial of a low-cost dementia detection algorithm.

BACKGROUND: About half of people living with dementia have not received a diagnosis, delaying access to treatment, education, and support. We previously developed a tool, eRADAR, which uses information in the electronic health record (EHR) to identify patients who may have undiagnosed dementia. This paper provides the protocol for an embedded, pragmatic clinical trial (ePCT) implementing eRADAR in two healthcare systems to determine whether an intervention using eRADAR increases dementia diagnosis rates and to examine the benefits and harms experienced by patients and other stakeholders.
