Deficiency and dysfunction of corneal cells leads to the blindness observed in corneal diseases such as limbal stem cell deficiency (LSCD) and bullous keratopathy. Regenerative cell therapies and engineered corneal tissue are promising treatments for these diseases [1]. However, these treatments are not yet clinically feasible due to inadequate cell sources. The discovery of induced pluripotent stem cells (iPSCs) by Shinya Yamanaka has provided a multitude of opportunities in research because iPSCs can be generated from somatic cells, thus providing an autologous and unlimited source for corne…

PURPOSE: To relate central corneal epithelial phenotype to degree of keratopathy in a limbal stem cell deficient population.
METHODS: 37 patients (67 eyes) with aniridia-associated keratopathy (AAK) underwent corneal examination including slit lamp biomicroscopy to determine the Grade of AAK, Cochet-Bonnet esthesiometry, and in vivo confocal microscopy (IVCM) to assess morphology of the central corneal epithelium and subepithelial region.
RESULTS: AAK Grade ranged from 1 (limbal involvement only) to 4 (total conjunctivalization), with progression from Grade 1 occurring after the age of 20. 3

Signals downstream of Akt can either favor or oppose stem cell (SC) maintenance, but how this dual role can be achieved is still undefined. Using human limbal keratinocyte stem cells (LKSCs), a SC type used in transplantation therapies for corneal regeneration, we show that Akt signaling is prominent in SC populations both in vivo and in vitro, and that Akt1 promotes while Akt2 opposes SC self-renewal. Noteworthy, loss of Akt2 signaling enhances LKSC maintenance ex vivo, whereas Akt1 depletion anticipates SC exhaustion. Mechanistically, the antagonistic functions of Akt1 and Akt2 in SC control

PURPOSE: To investigate the utility of in vivo confocal microscopy (IVCM) combined with standardized image grading for identifying early morphological changes in the central corneal and superior limbal basal epithelial cells during short-term orthokeratology (Ortho-K) lens wear, and its potential for identifying subclinical limbal stem cell stress.
METHODS: In this prospective study, 12 healthy participants (aged 18-29 years) with no prior Ortho-K experience wore 10.5 mm total diameter Ortho-K lenses for 14 days. IVCM was performed at baseline, Day 7, and Day 14 to assess central corneal and

PURPOSE: To investigate the incidence of limbal stem cell deficiency (LSCD) in donor eyes after conjunctival limbal autograft (CLAU).
METHODS: An observational retrospective review was performed on all patients who underwent CLAU alone, combined keratolimbal allograft with CLAU ("Modified Cincinnati Procedure"), or combined living-related conjunctival limbal allograft (lr-CLAL) with CLAU having ≥6 months of follow-up after surgery. The outcome measures were best-corrected visual acuity (BCVA) and ocular surface status.
RESULTS: The inclusion criteria were fulfilled by 45 patients. Of these, …

BACKGROUND: Limbal, conjunctival or oral mucosal (OM) epithelial explant cultures are used to treat limbal epithelial stem cell deficiency (LSCD), however, their characteristics remain elusive.
METHODS: Explant cultures established on silicon hydrogel contact lenses and source tissue samples were examined by immunofluorescence microscopy for limbal epithelial progenitor, corneal differentiation, and putative limbal niche markers. Quantitative analysis was done using confocal images of whole slides.
RESULTS: In all source tissue samples, many p63+ and some vimentin + basal epithelial as well

Amniotic membrane transplantation has been recently gained wide-spread attention as a new method for reconstruction of the ocular surface. However long term prognosis of the surgical intervention as well as action mechanism are poorly defined. Therefore, we review the current literature concerning the application of amniotic membrane in ophthalmology and the outcome of the surgery. Amniotic membrane is used as graft or patch to promote epithelial healing of persistent epithelial defects, corneal ulcers and following penetrating keratoplasty. It is also used to seal corneal perforations and to

We explore the interaction between corneal immunity and mesenchymal stem/stromal cells (MSCs) and their potential in treating corneal and ocular surface disorders. We outline the cornea's immune privilege mechanisms and the immunomodulatory substances involved. In this realm, MSCs are characterized by their immunomodulatory properties and regenerative potential, making them promising for therapeutic application. Therefore, we focus on the role of MSCs in immune-mediated corneal diseases such as dry eye disease, corneal transplantation rejection, limbal stem cell deficiency, and ocular graft-ve…

PURPOSE: To evaluate the outcome of limbal transplantation in eyes with bilateral severe ocular surface damage secondary to chemical injury.
PATIENTS AND METHODS: This was a retrospective case series that included 20 patients who had undergone living related limbal transplantation due to the presence of bilateral severe stem cell deficiency resulting from chemical injury. Medical records of the selected patients were reviewed. The following data were recorded and analyzed: gender; age at the time of the surgery; duration of follow-up; corrected distance visual acuity (CDVA); symptoms; intraoc

INTRODUCTION: We report the clinical outcome of a novel surgical technique called simple oral mucosal epithelial transplantation (SOMET) for the treatment of limbal stem cell deficiency (LSCD) in a patient with Stevens-Johnson Syndrome (SJS).
PRESENTATION OF CASE: An eighteen-year-old girl was diagnosed as having chronic bilateral ocular sequelae of SJS. She initially underwent mucous membrane grafting (MMG) in both eyes for lid margin keratinization. Over the course of the next decade, the ocular surface cicatrization worsened in her left eye, leading to progressive symblepharon formation wi…

Limbal niche dysfunction (LND) has been described as a medically reversible form of limbal stem cell deficiency. The current literature is sparse regarding therapeutic options to improve visual function after stabilization of the disease. A 61-year-old monocular woman with an extensive medical and ocular history presented with long-standing recalcitrant epitheliopathy in both eyes. History and examination findings on presentation led to a diagnosis of LND, and medical therapy was initiated. After 8 months, her ocular surface had improved and corrected distance visual acuity (CDVA) was 20/40. S…

PURPOSE: The improved surgical outcomes associated with transplantation of cultivated amniotic membrane expanded limbal epithelium (AMLE) compared to traditional donor methods has led to substantial adoption of this technique for treatment of limbal stem cell deficiency.
METHODS: The mRNA expression profiles of AMLE and CE were assayed using microarrays. Transcripts with a 1.5-fold change in either direction in addition to a Bonferroni adjusted P value < 0.05 were considered to be differentially expressed. Expression changes detected by microarray profiling and important corneal-limbal marker…

MATERIALS AND METHODS: In … after 6 months of treatment (p<0.0005).
CONCLUSION: In ocular GVHD, conjunctival cicatrization and limbal stem cell deficiency might be observed more often in adults. Topical cyclosporine, autologous/allogenic serum drops, and punctal plugs are helpful in moderate or more severe cases. With early diagnosis and treatment, an improvement in clinical signs and visual acuity mi …

PURPOSE: Limbal stem cells (LSCs) are essential for corneal epithelial regeneration and ocular surface homeostasis. Dysfunction of LSCs results in LSC deficiency (LSCD), a leading cause of global blindness. Although ex vivo expansion and autologous transplantation of LSCs have demonstrated promising clinical outcomes, no Food and Drug Administration-approved therapies for LSCD are available in the United States. To address this gap, we developed a novel 2-step process for isolating and expanding LSCs on human amniotic membrane under good manufacturing practice conditions, using a xenobiotic-fr
