Improved treatment outcome of pembrolizumab in patients with nonsmall cell lung cancer and chronic obstructive pulmonary disease.

Emerging immune profiling data suggest a higher sensitivity to immune checkpoint inhibitors (ICIs) in nonsmall cell lung cancer (NSCLC) patients with chronic obstructive pulmonary disease (COPD), compared to those without COPD. This study aimed to investigate the clinical impact of COPD on the treatment response to ICIs in a large number of patients with NSCLC. In total, 133 patients with spirometry test results were retrospectively identified among those who received palliative pembrolizumab for NSCLC. COPD was defined as pre-bronchodilator forced expiratory volume in 1 s/forced vital capacit

Colonization by Pneumocystis jirovecii in patients with chronic obstructive pulmonary disease: association with exacerbations and lung function status.

Exposure to Pneumocystis jirovecii (P. jirovecii) can lead to a wide variety of presenting features ranging from colonization in immunocompetent patients with lung disease, to invasive infections in immunocompromised hosts. Colonization by this fungus in patients with chronic obstructive pulmonary disease (COPD) could be associated with higher rates of exacerbations and impaired lung function in these patients. Our objective was to determine whether colonization by P. jirovecii in patients with COPD is associated with increased exacerbations and deterioration of lung function. This was a prosp

Endoplasmic reticulum, Golgi, and lysosomes are disorganized in lung fibroblasts from chronic obstructive pulmonary disease patients.

Chronic Obstructive Pulmonary Disease (COPD) is often caused by smoking and other stressors. This causes oxidative stress, which induces numerous changes on both the transcriptome and proteome of the cell. We aimed to examine if the endomembrane pathway, including the endoplasmic reticulum (ER), Golgi, and lysosomes, was disrupted in fibroblasts from COPD patients as opposed to healthy ever-smokers or never-smokers, and if the response to stress differed. Different cellular compartments involved in the endomembrane pathway, as well as mRNA expression and apoptosis, were examined before and aft

Impact of overweight and obesity on acute exacerbations of COPD - subgroup analysis of the Taiwan Obstructive Lung Disease cohort.

PURPOSE: A low body mass index (BMI) is a poor prognostic marker of acute exacerbations and mortality in patients with COPD. However, the impact of overweight and obesity on COPD-related outcomes is uncertain. The aim of this study was to examine whether a high BMI is associated with the frequent exacerbator phenotype (≥2/year) in Taiwanese patients with COPD. PATIENTS AND METHODS: Data were obtained from the Taiwan Obstructive Lung Disease study, a retrospective, observational nationwide survey of COPD patients conducted at 12 hospitals in Taiwan. Multivariate logistic regression models were

Effect of lung volume on airway luminal area assessed by computed tomography in chronic obstructive pulmonary disease.

BACKGROUND: Although airway luminal area (Ai) is affected by lung volume (LV), how is not precisely understood. We hypothesized that the effect of LV on Ai would differ by airway generation, lung lobe, and chronic obstructive pulmonary disease (COPD) severity. METHODS: Sixty-seven subjects (15 at risk, 18, 20, and 14 for COPD stages 1, 2, and 3) underwent pulmonary function tests and computed tomography scans at full inspiration and expiration (at functional residual capacity). LV and eight selected identical airways were measured in the right lung. Ai was measured at the mid-portion of the 3

A dynamic bronchial airway gene expression signature of chronic obstructive pulmonary disease and lung function impairment.

RATIONALE: Molecular phenotyping of chronic obstructive pulmonary disease (COPD) has been impeded in part by the difficulty in obtaining lung tissue samples from individuals with impaired lung function. OBJECTIVES: We sought to determine whether COPD-associated processes are reflected in gene expression profiles of bronchial airway epithelial cells obtained by bronchoscopy. METHODS: Gene expression profiling of bronchial brushings obtained from 238 current and former smokers with and without COPD was performed using Affymetrix Human Gene 1.0 ST Arrays. MEASUREMENTS AND MAIN RESULTS: We iden

Trends of bacterial colonisation and the risk of postoperative pneumonia in lung cancer patients with chronic obstructive pulmonary disease.

BACKGROUND: Lung cancer patients with chronic obstructive pulmonary disease (COPD) have a high risk of developing postoperative pneumonia (POP). This study aims to investigate the impact of COPD on POP and the trends for perioperative bronchial colonisation by micro-organisms. METHODS: A retrospective chart review was made for 626 patients who underwent lung cancer surgeries at the Chiba University Hospital between 1996 and 2005. The patients were categorised as non-COPD (n=475) and COPD (forced expiratory volume in 1s/forced vital capacity (FEV1/FVC) <70%; n=151). All the patients had sputum

Effect of living close to a main road on asthma, allergy, lung function and chronic obstructive pulmonary disease.

BACKGROUND: A number of epidemiological studies suggest that the risk of asthma is increased among those living in close proximity to major roads. However, the evidence is inconsistent, and effects on asthma and related respiratory and allergic conditions using objective measures such as lung function and allergic sensitisation have not been widely investigated. METHODS: In 1995, 1996 and 2001 data on respiratory and allergic disease, along with demographic and lifestyle factors, were collected in 59 285 children (aged 2-16 years) and adults as part of the Health Survey for England, a nationa

Are there sex differences in health-related quality of life after lung transplantation for chronic obstructive pulmonary disease?

The primary study aim was to examine sex differences in health-related quality of life (HRQoL) in a sample of 37 patients (20 men, 17 women) with chronic obstructive pulmonary disease (COPD) who were evaluated both before and after lung transplantation. Main outcome measures were change in HRQoL from pre- to post-transplantation assessment as measured by the Short Form-36 Health Survey, Transplant Symptom Frequency Questionnaire, forced expiratory volume in 1 second percent predicted, and 6-minute walk test. Study findings showed (1) substantial gains in HRQoL, lung function, and physical endu

Can high resolution computed tomography predict lung function in patients with chronic obstructive pulmonary disease?

High-resolution computed tomography (HRCT) is a useful method for quantifying the extent of emphysema. Few reports have mentioned the relationships between HRCT scans and pulmonary function tests in chronic obstructive pulmonary disease (COPD). For diagnosis, COPD requires chronic airflow limitation and emphysema and/or chronic bronchitis. We examined 20 who were previous smokers with middle to moderate COPD. All were normocapnic with mean arterial oxygen pressure (PaO2) 77,52 +/- 16,789 mmHg. Forced spirometry, somatic plethysmography and cardiopulmonary exercise test were performed in each p

[Noninvasive ventilation: current concepts in the treatment of chronic respiratory failure in patients with chronic obstructive lung disease].

Long-term oxygen therapy remains the mainstay treatment for advanced-stage chronic obstructive pulmonary disease that has reached the state of chronic respiratory failure. Noninvasive ventilation should be discussed for some patients with a instability and frequent hospitalizations for decompensation and progressive degradation of blood gases. Although this subpopulation remains to be perfectly defined, it is reasonable to discuss the implementation of noninvasive ventilation at night in association with daytime oxygen therapy for patients who do not respond to well conducted and well-implemen

[Signs of right heart stress in diurnal normoxemic patients with chronic obstructive lung disease and nocturnal hypoxemia].

Nocturnal hypoxemia in daytime normoxemic patients with COPD may lead to an increased right ventricular afterload due to pulmonary hypertension. We investigated the frequency of clinical, electrocardiographical, and radiological signs of right cardiac insufficiency (SRCI) in 178 consecutive COPD-patients [71 bronchitis, 25 emphysema, 82 bronchitis plus emphysema; PaO2 = 60 mm Hg]. Patients with asthma, left ventricular impairment, obstructive sleep apnea syndrome, primary pulmonary hypertension, and neuromuscular diseases were excluded. Polysomnography was performed in all patients. They were

Chronic obstructive lung disease and respiratory muscle function: the role of nutrition and exercise training in cystic fibrosis.

Malnutrition may be an extremely important complicating factor in patients with a variety of chronic lung diseases. The impact of malnutrition on lung disease and respiratory muscle function has been extensively studied in patients with chronic lung disease due to cystic fibrosis (CF). In patients with CF it appears that malnutrition and deterioration of lung function are closely interrelated and interdependent, with each affecting the other, leading to a spiral decline in both. The occurrence of malnutrition during childhood seems to be associated with impaired growth and repair of the airway

Prevalence of obstructive lung disease in a general population: relation to occupational title and exposure to some airborne agents.

BACKGROUND: The importance of occupational exposure to airborne agents in the development of obstructive disease is uncertain. Studying the relation in a community population has the benefit of reducing the healthy worker effect seen in studies of working populations. METHODS: The prevalence of obstructive lung disease was examined in a Norwegian general population aged 18-73 in a two phased cross sectional survey. In the second phase a stratified sample (n = 1512) of those responding in the first phase was invited for clinical and spirometric examination (attendance rate 84%). Attenders were

Oral immunisation with killed Haemophilus influenzae for protection against acute bronchitis in chronic obstructive lung disease.

Fifty patients with chronic obstructive lung disease were randomly allocated to three groups, to assess whether an oral vaccine containing non-typable Haemophilus influenzae protected against acute bronchitis. The double-blind prospective study over a three-month winter period included two placebo groups and one test group. Oral immunisation with H influenzae induced a tenfold reduction in the incidence of infection (p less than 0.001). During the subsequent winter, without further immunisation, protection by the vaccine was no longer statistically significant. There was no clear correlation b

Double-blind comparison of the respiratory effects of parenteral lorazepam and diazepam in patients with chronic obstructive lung disease.

The effects on the respiratory function of two tranquillising drugs, lorazepam and diazepam, have been compared in 20 patients with chronic obstructive lung disease. Both drugs induce a respiratory depression (decrease in tidal volume and minute ventilation with acceleration of the respiratory frequency) with slight respiratory acidosis, but lorazepam causes no significant hypoxemia and has a shorter duration of action than diazepam. Nevertheless, if tranquillisers are indicated in such patients, they have to be used with care.

The role of the gut-lung axis in persistent inflammation and disease progression in chronic obstructive pulmonary disease: mechanistic evidence and therapeutic implications.

Chronic obstructive pulmonary disease (COPD) is a highly prevalent chronic respiratory disease worldwide, and the mechanisms underlying its persistent airway inflammation, systemic inflammation, and acute exacerbations remain incompletely understood. In recent years, the concept of the gut-lung axis has provided a new perspective for understanding the pathophysiological processes of COPD, suggesting that gut microbiota imbalance may participate in the regulation of pulmonary inflammation via this bidirectional pathway. This article systematically reviews the biological basis of the gut-lung ax
