The inventory of problems-29 (IOP-29) in a US clinical sample.

Objective: Research on the Inventory of Problems-29 (IOP-29) has primarily used simulation designs and non-US samples. This study evaluated the IOP-29 in US clinical samples relative to validity scales obtained from Minnesota Multiphasic Personality Inventory (MMPI). Method: Clinical patients (N = 217) were administered the IOP-29 and the second or third version of the Minnesota Multiphasic Personality Inventory (MMPI) as part of routine care. Correlations evaluated criterion validity of the IOP-29 False Disorder Probability Score (FDS). Diagnostic accuracy statistics were calculated, with the

Utility of the Minnesota Multiphasic Personality Inventory-2 in differentiating genuine from feigned dissociative identity disorder.

OBJECTIVE: This study sought to determine the utility of the Minnesota Multiphasic Personality Inventory-2 (MMPI-2) in accurately distinguishing genuine dissociative identity disorder (DID) patients from coached and uncoached DID simulators. METHOD: DID patients (n = 34) who were diagnosed using the Structured Clinical Interview for DSM-IV-Dissociative Disorders were recruited from inpatient and outpatient settings. Coached (n = 25) and uncoached (n = 64) simulator groups were recruited from a Mid-Atlantic university. All participants completed the MMPI-2. RESULTS: MMPI-2 validity scales rel

Electroconvulsive Therapy Malpractice: Verdict for the Defense.

OBJECTIVES: Malpractice cases involving electroconvulsive therapy (ECT) are rare. Even rarer are those malpractice cases alleging ECT-related brain damage. The few cases of ECT malpractice lawsuits are not described in the medical literature in detail. METHODS: We provide a detailed account of a case of a patient and subsequent alleged ECT-related malpractice. The details of the case were collated using the handwritten notes of one of the authors who was present at the trial and the pretrial documents of discovery that were entered into evidence. RESULTS: The plaintiff alleged complete autob

Diagnosis, physiology, pathology and rehabilitation of traumatic brain injuries.

Accumulating clinical and experimental studies continue to elucidate and further define the significance of intra- and extra-cranial factors which determine outcome of traumatic brain injury. These factors include severity of injury, age of the patient, presence or absence of premorbid brain insults, and associated pathophysiological events such as anoxia, respiratory arrest, hemorrage, edema, contrecoup and Wallerian degeneration. Following resolution of acute temporary symptoms, delayed complications include seizures, neurotic and psychotic disorders, earlier onset of stroke, earlier senesce

Multiple personality disorder in criminal law.

The authors review the recent literature on multiple personality disorder (MPD), the most severe and chronic of the dissociative disorders, in relation to court cases of competence to stand trial, the insanity defense, and research on malingerers feigning MPD. Issues relevant in the assessment of competency and insanity are described. Features characteristic of MPD, including amnesia and alterations in consciousness and personality, have varying degrees of influence over the criminal behavior of an individual with MPD. As in other psychiatric disorders, the influence of MPD on an individual's

Factitious apraxia.

A case is described in which a patient had pseudoneurological symptoms that were present only upon direct observation or when the patient was in clinical test situations. The differential diagnosis of apraxia is discussed as well as clinical suggestions for evaluating patients with suspected factitious apraxia.

[Artefacts in porphyria cutanea tarda].

A 56-year-old patient suffering from porphyria cutanea tarda (PCT) was sufficiently treated with chloroquine and occasional bleeding. After 19 months of therapy, the laboratory finding showed absolutely normal values, whereas the clinical symptoms still persisted. Intensive exploration led to the diagnosis artefacts clinically imitating florid PCT remarkably well.

The role of psychological disturbance in extending disability time among compensable back injured industrial workers.

Work-related injuries are invariably related to protracted disability; however, confusion abounds concerning the genesis of this phenomenon. The present study examined the linkage between psychological disturbance and protracted disability in patients with work-related and non work-related injuries. A comparison was made between 1373 patients injured at work and 417 patients injured away from work on the variables of psychological disturbance and disability time. High but comparable levels of psychological disturbance were identified for both groups. People injured on the job were disabled lon

Acuity profile perimetry: description of technique and preliminary clinical trials.

Acuity profile perimetry is a technique that allows visual acuity properties to be rapidly measured at any location in the visual field. This report describes our initial clinical trials with acuity profile perimetry, as performed in combination with standard static perimetry. Preliminary results from patients with cataracts, amblyopia, and central serous retinopathy show, in general, a much greater deficit for acuity profiles than for static sensitivity profiles. In contrast, patients with optic neuritis or other optic neuropathies display approximately equivalent losses for static and acuity

Exploration of PCL-5 symptom validity indices for detection of exaggerated and feigned PTSD.

INTRODUCTION: There are very few symptom validity indices directly examining overreported posttraumatic stress disorder (PTSD) symptomatology, and, until recently, there were no symptom validity indices embedded within the PTSD Checklist for the DSM-5 (PCL-5), which is one of the most commonly used PTSD measures. Given this, the current study sought to develop and cross-validate symptom validity indices for the PCL-5. METHOD: Multiple criterion groups comprised of Veteran patients were utilized (N = 210). Patients were determined to be valid or invalid responders based on Personality Asessmen

A tale of two constructs: confirmatory factor analysis of performance and symptom validity tests.

BACKGROUND: Performance validity (PV) and symptom validity (SV) tests assess biased responding that impact scores on neuropsychological tests. The extent to which PV and SV represent overlapping or unique constructs remains incompletely defined, especially among psychiatric patients in a non-forensic setting. The current study investigated this question using confirmatory factor analysis. METHOD: Eighty-two inpatients with mood disorders were administered the Word Memory Test, and its primary indices formed a latent variable of PV. From the Minnesota Multiphasic Personality Inventory-2 the Fa

Use of the SIRS in compensation cases: an examination of its validity and generalizability.

The Structured Interview of Reported Symptoms (SIRS; Rogers et al., Structured interview of reported symptoms (SIRS) and professional manual, 1992) is a well-validated psychological measure for the assessment of feigned mental disorders (FMD) in clinical, forensic, and correctional settings. Comparatively little work has evaluated its usefulness in compensation and disability contexts. The present study examined SIRS data from 569 individuals undergoing forensic neuropsychiatric examinations for the purposes of workers' compensation, personal injury, or disability proceedings. Using bootstrapp

[Visual field examination in limited patient cooperation].

Objective methods to estimate the visual field are necessary, if a conventional subjective perimetry is impossible due to limited cooperation. Objective methods are indicated in infants, handicapped patients, patients with psychogenic visual field loss, and malingerers. An objective estimation of the visual field can be performed by means of pupillary light reflexes, voluntary and involuntary eye movements, and visual evoked potentials. Systematically false responses contain useful information regarding the proof of misrepresentations. The reproducibility of visual field defects can be checked
