Conservative versus surgical management of mandibular condylar fractures: a comparative study.

Condylar process fractures are a common mandibular injury predominantly affecting males, with a male-to-female ratio of 3:1. This study examines outcomes in patients with mandibular condylar fractures treated either conservatively or surgically, comparing long-term subjective results. A total of 281 cases were reviewed (144 treated conservatively and 137 treated surgically), with symptoms and functional outcomes assessed through questionnaires. Surgical intervention was more common in complex cases (e.g. bilateral fractures or concurrent maxillofacial injuries), and the surgical treatment grou

Opioid Use Following Open Reduction and Internal Fixation of Mandibular Fractures.

A mandible fracture is a common traumatic craniofacial injury for which surgery is sometimes indicated. Post-fracture and postoperative pain are often controlled through prescribed narcotics, which have received increased attention for potential abuse. The current work identified 90-day opioid prescribing patterns following surgically treated mandible fractures. Patients who underwent open reduction and internal fixation to treat a closed mandible fracture between 2011 and 2021 were identified in the PearlDiver Mariner165 database. Exclusion criteria included: (1) patients with records active

A 10-year retrospective study on mandibular fractures in Northern Taiwan.

BACKGROUND/PURPOSE: The mandible is an independent and protruding bone structure in the lower third portion of the human facial skeleton. Because of its prominent and unprotected position, the mandible is a primary site of facial trauma. Previous studies have not comprehensively discussed the association between the mandibular fractures and concomitant fractures of facial bones, the trunk, or limbs. This study analyzed the epidemiology of mandibular fractures and their correlation with concomitant fractures. MATERIALS AND METHODS: The present study enrolled 118 patients with a total of 202 ma…

Preauricular transparotid approach to mandibular condylar fractures without dissecting facial nerves.

Preauricular transparotid approach without dissecting the facial nerve was used for surgical treatment of 15 condylar fractures in 14 patients. The parotid fascia was opened just above the fracture site, and by dissecting the parotid gland and masseter muscle, the fracture was directly exposed. The facial nerve itself was not dissected expressly. All fractures could be reduced accurately and fixed firmly with miniplates. A direct approach just above the fracture site provided good vision of the fracture, avoiding facial nerve palsy caused by strong retraction. Moreover, by not dissecting the f

[Intermaxillary intraoperative fixation of mandibular fractures: arch bars or fixation screws?].

INTRODUCTION: The gold standard management of mandibular fractures is open reduction and osteosynthesis associated with intermaxillary fixation. The use of intermaxillary fixation screws for 20 years has considerably reduced the number of intermaxillary fixation with arch bars. The aim of our review was to identify current indications and contraindications of each technique. INTERMAXILLARY FIXATION TECHNIQUES: We present a short history and compare the two techniques with recently published assets, drawbacks, and complications. DISCUSSION: The indications of intermaxillary fixation screws ar…

Two load sharing plates fixation in mandibular condylar fractures: biomechanical basis.

Mandibular condylar fractures have a high incidence but there is no consensus regarding the best choice of osteosynthesis. From a review of the literature, it is evident that the technique used most frequently for fixation is the positioning of a single plate despite complications concerning plate fracture or screw loosening have been reported by various authors. Different studies have highlighted that the stability of osteosynthesis is correlated with the mechanical strains occurring in the condylar region, generated by the muscles of mastication. The aim of our study was, through a mandibula
