[Acute Mastoiditis in Children: a series of 38 Cases].

OBJECTIVE: We reviewed retrospectively the clinical, radiological and therapeutic findings in 38 infants and children with acute mastoiditis. MATERIAL AND METHODS: From 1988 to 2001, 38 children (mean age 36 months) were treated for acute mastoiditis and periostitis or retroauricular abscess. RESULTS: Diagnosis was usually made on the basis of typical retroauricular signs (60% of the cases), but was sometimes delayed owing to inappropriate antibiotics in 16 cases (42%) or atypical presentation in 3 (8%). Twenty-four children (70%) had no otolaryngological history. Pneumococci were isolated i…

Acute mastoiditis--the antibiotic era: a multicenter study.

OBJECTIVES: To evaluate the clinical course and identify the causative organisms of acute mastoiditis in a community where most of the patients who develop acute otitis media are treated with antibiotics. METHODS: A multicenter retrospective review of a series of 223 consecutive cases of acute mastoiditis. SETTING: Nine secondary or tertiary academic or non-academic referral centers. RESULTS: Prior to the diagnosis of acute mastoiditis, 121 of the patients (54.3%) had been receiving oral antibiotic treatment for acute otitis media for periods ranging from 1 to 21 days (mean 5.3 days). Sampl…

Impact of resistant pneumococcus on rates of acute mastoiditis.

OBJECTIVE: To determine whether the rate of acute mastoiditis is rising, specifically as a result of antibiotic-resistant strains of Streptococcus pneumoniae. METHODS: A retrospective chart review of all patients with a discharge diagnosis of acute mastoiditis between July 1, 1987, and June 30, 1997, was performed at our academic, tertiary-care medical center. There were no interventions, and the main outcome measures included the number of cases of acute or coalescent mastoiditis, stratified by pathogen, per year. RESULTS: The rate of acute mastoiditis as a proportion of yearly otorhinolary…

Acute mastoiditis in children: is surgical treatment necessary?

Acute mastoiditis in children remains an otological problem. Although the widespread use of antibiotics has reduced the need for surgical intervention, surgery is frequently used in the treatment of acute mastoiditis and its complications. The charts of 44 patients hospitalized with signs of acute mastoiditis were reviewed. In 43.2 per cent of all patients, acute mastoiditis was the presenting sign of acute middle-ear infection. Post-auricular erythema and protrusion of the auricle were the most frequent signs at presentation. All four signs (post-auricular erythema, oedema, tenderness, and pr…

Acute mastoiditis with complications: a report of two cases.

The authors report two cases of lateral sinus thrombosis (LST) which were recently observed after more than a 20-year absence of such pathology at their Institutions. After reviewing the epidemiologic data, the clinical observations are described in detail. Changes in symptomatology mainly due to the improper use of antibiotics are then discussed as well as the diagnostic improvement offered by modern imaging techniques. Problems still under debate concern the therapeutic strategy: internal jugular vein ligation or anticoagulation therapy versus aminoglycoside administration for patients refra

[Acute mastoiditis in childhood. A study of 15 cases].

We present 15 cases of acute mastoiditis diagnosed in previously healthy children. Diagnostic criteria were the presence of acute otitis media, retroauricular inflammatory signs and radiographic alteration of the mastoid bone. The average age of the children was 3 years, with a range between 5 months and 7 years. Two children were younger than 1 year of age. The lesion was unilateral in all cases. Blood analysis revealed an increased erythrocyte sedimentation rate, being higher than 66 mm in the first hour in all patients. In 4 children it was higher than 110 mm. Ten children received previous…

A comparison of mastoid pneumatization in adults and children with cholesteatoma.

The records of 190 adults who presented over a 10-year period with previously untreated aural cholesteatomas were analyzed with respect to age, mode of presentation at the level of the tympanic membrane, and pneumatization of the mastoid. The findings were compared with those obtained in an earlier study of 109 children (aged 13 years and younger) seen over the same period of time. Two main types of cholesteatomas were found in both adults and children, most of them (82.3%) associated with sclerotic or diploic mastoids (i.e., poorly pneumatized mastoids) and the rest (17.3%) with pneumatized m…

Acute mastoiditis: clinical, microbiological, and therapeutic aspects.

The charts of 73 children (31 girls, 42 boys) aged 4 months to 14 years (mean 4.5 years) with acute mastoiditis managed during a 16-year period were reviewed. Of the patients 36% were less than 24 months old. Retro-auricular swelling was described in 63 of the 73 children, tenderness in 59, erythema in 58, and protrusion of the auricle in 45. A pathological tympanic membrane was noted in 33% of the patients and fever in only 29%. Apart from local inflammation, the most frequent complaints and symptoms were otalgia (n = 42), recent upper respiratory tract infections (n = 22), and fever alone (n…

Anaerobic mastoiditis: a report of two cases with complications.

Aerobic bacteria are responsible for most cases of suppurative otitis media/mastoiditis (OM/M). As many as 20-30% of middle ear aspirates in acute otitis media, however, are reported to be sterile on aerobic culture. Some of these cases may be due to anaerobic bacteria. Two cases of OM/M due to anaerobic bacteria are reported. The first patient had no antecedent ear disease and developed a large Bezold abscess caused by Fusobacterium varium. The second patient had attic retraction pouches bilaterally and a history of otorrhea. This patient developed labyrinthitis and meningitis due to B. fragi…

Neonatal meningitis and mastoiditis caused by Hemophilus influenzae.

A newborn infant developed Hemophilus influenzae meningitis associated with acute coalescent mastoiditis and a cutaneous abscess in the mastoid region. Mastoidectomy was followed by prompt recovery from the meningitis, which had failed to clear previously despite antibiotic therapy. Mastoiditis may exist as an infective focus in neonatal meningitis more frequently than has been appreciated. Mastoid roentgenograms are usually the only clue to diagnosis of this infection and should be obtained in patients with neonatal meningitis responding poorly to antibiotic therapy.
