Multilevel complex interactions between genetic, epigenetic and environmental factors in the aetiology of anomalies of dental development.

Dental anomalies are caused by complex interactions between genetic, epigenetic and environmental factors during the long process of dental development. This process is multifactorial, multilevel, multidimensional and progressive over time. In this paper the evidence from animal models and from human studies is integrated to outline the current position and to construct and evaluate models, as a basis for future work. Dental development is multilevel entailing molecular and cellular interactions which have macroscopic outcomes. It is multidimensional, requiring developments in the three spatia

[Genetic collagen disorders and the impact on craniofacial development].

Extracellular matrix molecules provide to tissues their mechanical properties and constitute a reservoir of local or regional signals that regulate cellular function. Collagens, the major components of osseous and collagenous matrices, have structural similarities, but are encoded by different genes. We describe here osteogenesis imperfecta, a collagen I, the principal constituent of bone, genetic disease, and its craniofacial implications. By comparing it with genetic disorders of cartilage collagen (Kniest and Stickler syndromes) we try to clarify the respective influences of these matrix mo

[Maxillo-facial abnormalities in Kearns-Sayre syndrome].

Three cases of Kearns-Sayre Syndrome are reported, in which some facial anomalies, including facial asymmetry, high forehead, wide nasal bridge, upturned nose, flat philtrum, low set ears and short neck were present. In two cases, the diagnosis of oxidative phosphorylation deficiency was confirmed by hystoenzymatic and genetic studies. The relationship of these facial anomalies with neural crest maldevelopment is emphasized and a classification of the Kearns-Sayre Syndrome as metabolic neurocristopathy is proposed. The facial anomalies are suggestive of an antenatal expression of the oxidative

An analysis of the skeletal relationships in a group of young people with hypodontia.

The objective of this investigation was to examine the dentofacial features of a group of patients with hypodontia, in particular assessing whether cephalometric analysis confirmed the clinical assumption of a reduced lower face height, and to determine the relationship of these facial features with different numbers of missing teeth. It took the form of a cephalometric study, undertaken in a dedicated Dental Hospital clinic for patients with hypodontia. The study group comprised 59 patients seen on the Hypodontia Clinic: 32 females, 27 males, mean age 13.1+/-3.1 years (range 6-23 years). The

Three-dimensional magnetic resonance imaging of the orbit in craniofacial malformations and trauma.

Craniofacial malformations and trauma often lead to changes in orbital soft tissues, requiring surgical correction of both hard and soft tissues. Computed tomographic scans and 3-dimensional reconstructions are the optimal tools for evaluation of the bony structures. However, there is no equivalent method for the orbital soft tissues. The aim of this study was to establish a 3-dimensional magnetic resonance imaging (3-D MRI) technique that allows a differentiated visualization of the different types of soft tissue in the orbit. A total of 8 patients with different pathologic conditions of the

Oro-maxillofacial development in patients with Turner's syndrome.

Detailed oro-maxillofacial studies using dental casts, pantomograms and cephalograms were performed in 28 patients with Turner's syndrome and compared statistically to the results from 23 normal short children. Small tooth crown size, short tooth roots and advanced dental age were characteristic of patients with Turner's syndrome. However, the incidence of peg shaped teeth, malocclusion, high arched palate and congenital anodontia were not characteristic of patients with Turner's syndrome. The coronal arch width (C.A.W.) and basal arch width (B.A.W.) were greater and the coronal arch length (C

VALIDATION OF THE DIAGNOSTIC AND TREATMENT COMPLEX FOR PATIENTS WITH ORTHOGNATHIC DEFORMITIES AND PHONETIC DISORDERS.

Recently, there has been a tendency for the growth of dentognathic deformities of various origins, accompanied by phonetic abnormalities. Aim - to increase the effectiveness of orthodontic treatment of dentognathic deformities, accompanied by phonetic disorders, by developing and justifying a set of diagnostic and therapeutic measures based on a multidisciplinary approach. The influence of the state of ENT organs on the formation of dentognathic deformities and phonetic disturbances is studied in 155 children. A clinical dental examination and orthodontic treatment is performed in 82 patients

Growth and development considerations for craniomaxillofacial surgery.

The purpose of craniomaxillofacial surgery is to improve function, occlusion, craniofacial balance, and aesthetics. Accurate diagnosis, assessment, and careful treatment planning are essential in achieving a successful outcome, and an understanding of the pattern of facial growth is integral in this process. Patients with craniofacial congenital dysmorphologies, posttraumatic asymmetries, or disturbances of facial balance from radiation may have functional and/or aesthetic issues that require treatment. Understanding the complexities of growth in the skull and face is a key component to approp

Sox9 function in craniofacial development and disease.

The Sox family of transcriptional regulators has been implicated in the control of a broad array of developmental processes. One member of this family SOX9 was first identified as a candidate gene for campomelic dysplasia (CD), a human syndrome affecting skeletal, and testis development. In these patients most endochondral bones of the face fail to develop resulting in multiple defects such as micrognathia, cleft palate, and facial dysmorphia. In this review we describe Sox9 expression during embryonic development and summarize loss of function experiments in frog, fish, and mouse embryos high

Follow-up: orthognathic surgery. Is there a future? A national survey.

BACKGROUND: The authors recently documented a significant decrease in orthognathic surgical cases performed by both plastic and oral surgeons in Ohio over a recent 5-year period. The main reason noted was related to third-party reimbursement. This is a potentially serious issue that may affect the quality of health care for patients with dentofacial deformities. Therefore, an expanded survey was conducted to determine whether this was indicative of a national trend. METHODS: A three-page questionnaire was sent nationally to plastic surgeons and oral surgeons who were members of the American S

[Callus distraction of the mid-face in severe mid-face hypoplasia].

Callus distraction in the treatment of severe midfacial hypoplasia/atrophy may offer new therapeutical possibilities. Between May 1998 and March 1999, six patients underwent a subtotal Le Fort I-II osteotomy with subsequent distraction via a haloborne distractor (RED). Five patients suffered from CLP, four patients were totally or almost edentulous in the upper jaw. Corrections of sagittal discrepancies were possible in all cases (distraction 16-31 mm). No case of increased velopharyngeal insufficiency was seen. In one patient the halo had to be refixed. Three patients suffered from tension-li

Digital imaging in the assessment of facial deformity.

A combination of digital imaging and conventional radiographic techniques has been used to obtain a life-size composite image of the facial soft tissue profile, the skull, and the teeth, anatomically superimposed on each other. The system overcomes major problems associated with assessment of facial deformity such as magnification, the exposure of photographs and radiographs from different positions, radiographs with overexposed soft tissue profiles, lengthy procedures in photographic studios, and, in addition, enables a degree of partial automation in the management of patients.

The in utero repair of Tessier number 7 lateral facial clefts created by amniotic band-like compression.

The purpose of this study was to determine the effectiveness of intrauterine reconstruction of lateral facial clefts caused by surgically simulated amniotic band compression. The authors hypothesized that intrauterine lysis of these bands could: 1) prevent the progression of cleft formation; 2) normalize facial development; and 3) allow for the scarless repair of these lateral atypical facial clefts. In a prospective randomized trial, eight 65-day gestational fetal lambs (term, 140 days) were exposed via a maternal hysterotomy. A Tessier 7 facial cleft was made by applying a 2.0 nylon suture a

A paradigm in panfacial fracture treatment to improved dental occlusion.

INTRODUCTION: The prevalence of maxillofacial fractures is rising due to increased road traffic accidents, necessitating prompt and effective management, especially in cases of panfacial fractures. The primary objective in treating such fractures is to restore occlusion and stabilize midface buttresses and pillars. CASE REPORT: This article presents the case of a 56-year-old male who sustained panfacial fractures following a road accident, exhibiting symptoms including facial pain and nosebleeds. The treatment involved the use of arch bars for dental occlusion. Post-operative recovery was com

[Individual-specific symptoms of malignant neoplasms of the oropharynx occurring in otolaryngologist's practice].

According to statistical data given in the article the incidence of oropharyngeal cancer increases, despite the ease of access to the maxillofacial area and the possibility to make visual and manual examination of this site. It should be stressed that locally advanced oropharyngeal cancer of the III-IV stages was the first diagnosis in 82.9% of patients. It may be due to low level suspicion of primary care physicians. The paper considers risk factors for development of oropharyngeal cancer and presents the list of compulsory diagnostic tests. The authors suggest that recommended diagnostic alg
