Closing the gap in implementation of HIV clinical guidelines in a low resource setting using electronic medical records.

BACKGROUND: Universal health coverage promises equity in access to and quality of health services. However, there is variability in the quality of the care (QoC) delivered at health facilities in low and middle-income countries (LMICs). Detecting gaps in implementation of clinical guidelines is key to prioritizing the efforts to improve quality of care. The aim of this study was to present statistical methods that maximize the use of existing electronic medical records (EMR) to monitor compliance with evidence-based care guidelines in LMICs. METHODS: We used iSanté, Haiti's largest EMR to ass…

Drug-drug interaction between valproic acid and meropenem: a retrospective analysis of electronic medical records from neurosurgery inpatients.

WHAT IS KNOWN AND OBJECTIVE: A series of studies have indicated that valproic acid (VPA) plasma concentration decreased rapidly when used concomitantly with carbapenem antibiotics, including meropenem (MEPM), imipenem and panipenem, which may increase the risk of seizure breakthrough. However, the cause for the change in VPA pharmacokinetics is unclear. A retrospective analysis of VPA therapeutic drug monitoring (TDM) records was performed to investigate this VPA pharmacokinetics drug-drug interaction. METHODS: Three hundred and eighty one VPA TDM records from the Department of Neurosurgery o

Channeling in the Use of Nonprescription Paracetamol and Ibuprofen in an Electronic Medical Records Database: Evidence and Implications.

INTRODUCTION: Over-the-counter analgesics such as paracetamol and ibuprofen are among the most widely used, and having a good understanding of their safety profile is important to public health. Prior observational studies estimating the risks associated with paracetamol use acknowledge the inherent limitations of these studies. One threat to the validity of observational studies is channeling bias, i.e. the notion that patients are systematically exposed to one drug or the other, based on current and past comorbidities, in a manner that affects estimated relative risk. OBJECTIVES: The aim of

Using an Electronic Medical Records Database to Identify Non-Traditional Cardiovascular Risk Factors in Nonalcoholic Fatty Liver Disease.

OBJECTIVES: Among adults with nonalcoholic fatty liver disease (NAFLD), 25% of deaths are attributable to cardiovascular disease (CVD). CVD risk reduction in NAFLD requires not only modification of traditional CVD risk factors but identification of risk factors unique to NAFLD. METHODS: In a NAFLD cohort, we sought to identify non-traditional risk factors associated with CVD. NAFLD was determined by a previously described algorithm and a multivariable logistic regression model determined predictors of CVD. RESULTS: Of the 8,409 individuals with NAFLD, 3,243 had CVD and 5,166 did not. On mult

Using automated medical records for rapid identification of illness syndromes (syndromic surveillance): the example of lower respiratory infection.

BACKGROUND: Gaps in disease surveillance capacity, particularly for emerging infections and bioterrorist attack, highlight a need for efficient, real time identification of diseases. METHODS: We studied automated records from 1996 through 1999 of approximately 250,000 health plan members in greater Boston. RESULTS: We identified 152,435 lower respiratory infection illness visits, comprising 106,670 episodes during 1,143,208 person-years. Three diagnoses, cough (ICD9CM 786.2), pneumonia not otherwise specified (ICD9CM 486) and acute bronchitis (ICD9CM 466.0) accounted for 91% of these visits,

The challenges of using electronic medical records (EMR) to facilitate guideline-directed medical therapy (GDMT) for patients with heart failure (HF) and chronic kidney disease (CKD).

Heart failure (HF) and chronic kidney disease (CKD) are prevalent comorbidities that significantly impact patient outcomes, with nearly half of HF patients experiencing renal impairment. The challenges associated with implementing guideline-directed medical therapy (GDMT) for patients with HF and CKD emphasize the role of electronic medical records (EMRs) as clinical decision-support tools. Despite the proven benefits of GDMT in improving survival and reducing hospital readmissions, many eligible patients do not receive optimal therapy due to barriers such as alert fatigue, medication costs, a…

Maternal infections and medications in pregnancy: how does self-report compare to medical records in childhood cancer case-control studies?

BACKGROUND: Studies examining the potential impact of mothers' health during pregnancy on the health of their offspring often rely on self-reported information gathered several years later. To assess the validity of this approach, we analysed data from a national case-control study of childhood cancer (diagnosed <15 years) that collected health information from both interviews and medical records. METHODS: Mothers' interview reports of infections and medications in pregnancy were compared with primary care records. Taking clinical diagnoses and prescriptions as the reference, sensitivity and …
