BACKGROUND: Autism is a heritable developmental disorder of communication and socialization that has not been well studied in Hispanic populations. Therefore, we are collecting and evaluating all possible cases of autism from a population isolate in the Central Valley of Costa Rica (CVCR) for a clinical and genetic study.
METHODS: We are assessing all subjects and parents, as appropriate, using the newly translated Spanish versions of the Autism Diagnostic Interview-Revised (ADI-R) and the Autism Diagnostic Observation Schedule (ADOS) as well as tests of intelligence and adaptive behavior. De

It has proven very difficult to determine the causes of early infant mortality and morbidity in Africa. We undertook a two-year, prospective birth cohort study in a rural Kenyan District Hospital to estimate cause-specific mortality and severe morbidity in infants too young to gain benefit from routine immunization approaches. A total of 2,359 infants eligible for the cohort were delivered. Of these, 136 (6%) were stillborn and 77 (3.5%) subsequently died. Prematurity (34%), birth asphyxia (27%), and infection (18.5%) were the predominant causes of death in the first 98 days of life, although

This is the third paper in the peripheral arterial disease (PAD) Series. The first and second papers in this Series outline that PAD affects over 300 million people worldwide and can be diagnosed by non-invasive tests, as well as that treatment requires medications to reduce modifiable risk factors and that exercise therapy and revascularisation are required in selected patients. In this paper, the devastating effects of PAD at the individual level are described, including chronic pain, functional limitation, loss of independence, impaired physical and mental quality of life, unemployment, and

Health inequalities, deeply connected to broader social determinants such as unemployment, inadequate housing, and limited access to healthcare and essential services, remain a global concern. These factors disproportionately impact socioeconomically underserved communities, even in high-income countries. In Ireland, similar disparities are evident within urban areas. Although policy and practice increasingly promote community participation, there is limited evidence on how communities perceive and articulate their challenges and priorities. This study, conducted in collaboration with Healthy

BACKGROUND: The North East of England has the lowest healthy life expectancy and the highest health inequalities of any region in England. The conventional model, whereby we 'expect' individuals to be motivated to attend a 'healthcare setting' to undergo cardiovascular disease (CVD) health checks every 5 years has low levels of uptake, with populations most at risk frequently failing to engage.
OBJECTIVES: The objective of this study was to gather behavioural insights into the barriers/challenges that limit engagement with the current NHS CVD Health Checks.

OBJECTIVE: To establish causes and patterns of deaths among adolescents and adults (age >11 years) using verbal autopsy (VA) in a rural area of western Kenya where malaria and HIV are common.
METHODS: Village reporters reported all deaths in Asembo and Gem (population 135 000), an area under routine demographic surveillance. After an interval of >/=1 month, a trained interviewer used a structured questionnaire to ask the caretaker about signs and symptoms that preceded death. Three clinical officers independently reviewed the interviews and assigned two unranked causes of death; a common caus

Current WHO methods were employed to estimate the prevalence of toothbrush possession, tea drinking, tobacco habits, fluorosis, dentofacial anomalies, periodontal diseases, dental caries as well as treatment needs in schoolchildren in Tunisia, the smallest country in North Africa. Fluorosis was endemic in some regions. Dentofacial anomalies were more frequent than in the other countries of the Eastern Mediterranean Region (EMR). The prevalence of periodontal disease was high despite the high percentage of toothbrush possession. Altogether 60% of 12-year-olds and 70% of 15-year-olds required or

OBJECTIVE: To summarize the unique aspects of managing headache in gender minorities and current research in this area including the potential relationship between gender-affirming hormone therapy (GAHT) and headache.
BACKGROUND: The study of headache in gender minorities is intrinsically important. Gender minorities are medically underserved, and their medical care to date has been limited by socioeconomic disadvantages including stigma and an unsupportive clinical environment. Despite the rising population of transgender and gender-diverse adults and youth, headache research has also been l

UNLABELLED: The relationship between attendance at birth and maternal mortality rates: an exploration of United Nations' data sets including the ratios of physicians and nurses to population, GNP per capita and female literacy.
BACKGROUND: This is the third and final paper drawing on data taken from United Nations (UN) data sets. The first paper examined the global distribution of health professionals (as measured by ratios of physicians and nurses to population), and its relationship to gross national product per capita (GNP) (Wharrad & Robinson 1999). The second paper explored the relations

BACKGROUND: The Mexican mandatory year of social service following medical school, or pasantía, is designed to provide a safety net for the underserved. However, social service physicians (pasantes) are typically unpracticed, unsupervised, and unsupported. Significant demotivation, absenteeism, and underperformance typically plague the social service year.
OBJECTIVE: Compañeros en Salud (CES) aimed to create an education-support package to turn the pasantía into a transformative learning experience.
DESIGN: CES recruited pasantes to complete their pasantía in CES-supported Ministry of Health

Introduction: Lung cancer is a leading cause of cancer deaths globally. Despite favorable recommendations, low-dose computed tomography (LDCT) lung screening adoption remains low in China. Barriers such as limited infrastructure, costs, distance, and personnel shortages restrict screening access in disadvantaged regions. We initiated a telemedicine-enabled lung cancer screening (LCS) program in a medical consortium to serve people at risk in underserved communities. The objective of this study was to describe the implementation and initial results of the program. Methods: From 2020 to 2021, in

Urban slums are densely populated areas characterized by inadequate housing, poor sanitation, and limited access to essential services. As part of the MBBS curriculum at Patan Academy of Health Sciences (PAHS), we visited an urban slum in Balkhu to assess the community's health-seeking behaviour, water facilities, and education. This article highlights the significance of such postings in equipping medical students with critical skills, including empathy, teamwork, and leadership, necessary for addressing the socio-economic determinants of health. The experience provided an invaluable opportun

BACKGROUND: In the United States, cancer is more prevalent in racial and ethnic minority groups and in rural-dwelling and low-income people. Compared with White people of non-Hispanic descent, Black and African American people have higher cancer mortality and Hispanic people are more likely to be diagnosed with infection-related cancers. In addition, people who live in persistent poverty areas are more vulnerable to cancer mortality. Tailoring health information technologies (HITs) can help bridge health inequities by providing these populations with relevant health information and cancer care

PURPOSE: To examine the use of hypofractionation among underserved populations, with a focus on implications for policy.
METHODS AND MATERIALS: Using a 20% Medicare claims sample, we evaluated the receipt of hypofractionation, a short-course radiation therapy approach, among underserved versus nonunderserved populations. Hypofractionation was defined based on cancer/site-specific guidelines and expert review. Underserved groups included racially/ethnically minoritized, rural, and patients with low socioeconomic status. We included 4 cancer types that represent the majority of new cancer diagn
