Leucocyte ascorbic acid levels in Nigerian children with protein-energy malnutrition.

Leucocyte ascorbic acid (LAA) levels were estimated in 26 Nigerian infants and preschool children with protein energy malnutrition (PEM) in order to ascertain their ascorbic acid status. The children included eight with kwashiorkor, 12 with marasmus and six with marasmic kwashiorkor. The mean (SD) LAA level of 13.7 (8.2) micrograms/10(8) leucocytes in the malnourished children did not vary significantly from the mean level of 14.4 (8.8) micrograms/10(8) leucocytes found in 26 age-matched controls, and the values were unrelated to the presence or absence of megaloblastic change in the 14 bone m

Pancytopenia: a clinico hematological study.

BACKGROUND: Pancytopenia is a relatively common hematological entity. It is a striking feature of many serious and life-threatening illnesses, ranging from simple drug-induced bone marrow hypoplasia, megaloblastic anemia to fatal bone marrow aplasias and leukemias. The severity of pancytopenia and the underlying pathology determine the management and prognosis. Thus, identification of the correct cause will help in implementing appropriate therapy. OBJECTIVES: To study the clinical presentations in pancytopenia due to various causes; and to evaluate hematological parameters, including bone ma

Intestinal iron absorption under the influence of available storage iron and erythroblastic hyperplasia. Comparative studies in children with hereditary spherocytosis, nonspherocytic enzymopenic hemolytic anemia, acquired hemolytic anemia, vitamin B12 deficiency induced megaloblastic anemia, erythroblastic hypoplasia and aplastic anemia.

A high negative correlation (coefficient similar to 0.9) between increased 59Fe absorption from a diagnostic 0.56 mg 59Fe2+ dose and the depletion of available storage iron was observed in menstruating and pregnant women, fullterm and premature infants, blood donors, patients with infections, inflammations, tumors, hepatic cirrhosis, gastric surgery, increased urogenital or gastrointestinal blood loss. The increased diagnostic 59Fe2+ absorption is a reliable and sensitive indicator of at least depleted iron stores or prelatent iron deficiency as caused by iron malnutrition or maldigestion, inc

Systemic Sclerosis Presenting With Chronic Atrophic Gastritis and Retinal Artery Occlusion: A Case Report.

Systemic sclerosis (SSc), also known as scleroderma, is a rare autoimmune disease characterized by fibroproliferative changes, vascular dysfunction, and small-vessel proliferation. It can affect multiple organ systems, including the gastrointestinal tract, skin, heart, eyes, kidneys, and lungs. We report a case of a 67-year-old woman with systemic sclerosis who tested negative for specific antibodies and simultaneously developed two rare complications: autoimmune atrophic gastritis resulting in megaloblastic anemia and acute retinal artery occlusion. Notably, she presented with megaloblastic a

Vitamin B12 Deficiency Resembling Acute Leukemia: A Case Report.

Vitamin B12 deficiency in children can cause megaloblastic anemia, poor growth, and increased chances of infections. It is an important reversible cause of bone marrow suppression which at the time of presentation can mimic hematological malignancy. Therefore, it should be considered as a differential diagnosis in cases suspected of acute leukemia. We report a case of 14 months old child who had atypical presentation of vitamin B12 deficiency. He had chronic fever, decreased feeding and increased paleness for one year. Pancytopenia with severe anemia was present along with 19% reactive/atypica

Neonatal diabetes mellitus: remission induced by novel therapy.

A female child with deafness was diagnosed to have neonatal diabetes mellitus at the age of 6 months, on routine evaluation prior to cochlear implant surgery. She presented to us at 11 months of age with diabetic ketoacidosis due to an intercurrent febrile illness. Her haematological parameters showed megaloblastic anaemia and thrombocytopenia. Therefore a possibility of Thiamine Responsive Megaloblastic Anaemia (TRMA) syndrome was considered. She was empirically treated with parenteral thiamine hydrochloride (Hcl). Subsequently, due to the unavailability of pharmacological preparation of oral

Bone marrow cells from vitamin B12- and folate-deficient patients misincorporate uracil into DNA.

Bone marrow cells from 15 patients with normal deoxyuridine (dU) suppression test results, 3 healthy subjects, and 11 patients with megaloblastic anemia caused by vitamin B12 or folate deficiency were examined for misincorporation of uracil into DNA. Cells were incubated with [5-3H] uridine for 2 hours and their DNA extracted. The DNA was hydrolyzed to deoxyribonucleosides with DNase 1, phosphodiesterase and alkaline phosphatase, and any dU present was separated from other deoxyribonucleosides by Aminex A6 chromatography. The quantity of dU/mg DNA and the radioactivity in the dU peak/mg DNA we

[Hematologic features of nutritional anemias in children].

In eighty nine anemic children, aged from 6 months to 13 years, the etiology of their anemia was studied: 67(75%) of the cases presented iron deficiency, 18(20%) iron and folate deficiency and 4 megaloblastic anemia (2 folate and 2 vitamin B12 deficiency). Patients with vitamin B12 deficiency showed severe pancytopenia and megaloblastic changes in bone marrow; but no morphological difference, either in circulating blood or bone marrow was found in patients with iron deficiency, compared to iron plus folate deficiency. The seric measurement of iron, folate and B12 were essential in order to est

The plasma clearance of injected doses of folic acid as an index of folic acid deficiency.

The folic acid clearance test of Chanarin (1958) has been studied in normal white subjects, patients with megaloblastic anaemia, healthy Bantu males, Bantu females, hospital patients without anaemia, and in patients with scurvy. The test clearly distinguishes patients with megaloblastic anaemia, where clearance from the plasma of injected folic acid is abnormally rapid, from normal subjects. The rate of clearance of folic acid is abnormally rapid both in vitamin B(12) and in folic acid deficiency. Rapid clearance of injected folic acid in patients with primary B(12) deficiency may be the resul

Erythroblast kinetics in pernicious anaemia, erythroleukaemia and sideroblastic anaemia.

DNA synthesis time (Ts) and 3H-thymidine labelling index (TLI) of erythroblasts have been determined in 25 patients with various types of ha … blasts, which was interpreted to reflect prevention of intramedullary premature death of basophilic megaloblasts. Erythroleukaemia showed a markedly prolonged Ts and lowered TLI indicating the presence of cells with prolonged cell cycle time. These findings contrasted to that of pernicious anaemia despite certain morphological as well as functional similaritie …

Folate and human reproduction.

The influence of folate nutritional status on various pregnancy outcomes has long been recognized. Studies conducted in the 1950s and 1960s led to the recognition of prenatal folic acid supplementation as a means to prevent pregnancy-induced megaloblastic anemia. In the 1990s, the utility of periconceptional folic acid supplementation and folic acid food fortification emerged when they were proven to prevent the occurrence of neural tube defects. These distinctively different uses of folic acid may well be ranked among the most significant public health measures for the prevention of pregnancy

Macrocytic anemia.

In approximately 2 to 4 percent of patients, laboratory evidence of macrocytosis is found. Macrocytic anemias are classified as those resulting from disorders of DNA synthesis of erythrocyte precursors in bone marrow (megaloblastic anemias) or those caused primarily by alcoholism, liver disease and hypothyroidism (nonmegaloblastic anemias). A blood smear should be performed to differentiate the two forms. Neutrophil hypersegmentation is one of the most sensitive and specific signs of megaloblastic anemia. Other testing should include vitamin B12 and red blood cell folate levels, reticulocyte c

Sulphasalazine associated pancytopenia may be caused by acute folate deficiency.

Agranulocytosis and aplastic anaemia associated with sulphasalazine are well recognised, but pancytopenia caused by acute megaloblastic arrest of haemopoiesis while taking sulphasalazine has not previously been described. We report three patients who, after taking sulphasalazine for over two years, suddenly developed severe pancytopenia with gross megaloblastic changes in the marrow. In two patients there was a good response to high dose oral folic acid but the third required folinic acid. The mechanism appears to be acute folate deficiency, and the requirement for folinic acid in one case sug

Severe thrombocytopenia probably due to acute folic acid deficiency.

Thirteen patients with significant hemorrhage, severe thrombocytopenia, and megaloblastic bone marrows are described. Unusual features of this problem included its acute onset, frequent absence of the typical peripheral blood changes of megaloblastic anemia, normal serum B12 levels, and serum folates which were often not clearly abnormal. Most patients were critically ill and common clinical features included reduced dietary intake, renal failure, renal dialysis, the postoperative state, and sepsis. These clinical features, the laboratory findings, and a platelet increase in most patients afte

A Case of Pernicious Anemia Induced by Atezolizumab in Hepatocellular Carcinoma and Renal Cell Carcinoma.

BACKGROUND Anemia derived from immune checkpoint inhibitor (ICI) is recognized as a systemic immune-related adverse event (irAE). However, only a few studies reported regarding pernicious anima (PA) or megaloblastic anemia induced by vitamin B12 (Vit.B12) deficiency after ICI treatment. Vit.B12 deficiency-induced PA remains unclear as an irAE. CASE REPORT We present a case of a 68-year-old man with hepatocellular carcinoma and renal cell carcinoma who developed Vit.B12 deficient megaloblastic anemia after the induction of atezolizumab-containing chemotherapy. Laboratory findings revealed hemog

B12 deficiency leading to marked poikilocytosis versus true schistocytosis, a pernicious problem.

Severe vitamin B12 deficiency is caused most commonly by autoimmune atrophic gastritis leading to loss of intrinsic factor. Vitamin B12 deficiency leading to megaloblastic anemia and demyelinating central nervous system disease is well known; however, a rare presentation of B12 deficiency described as pseudothrombotic microangiopathy is not well known. This complication presents with signs of mechanical hemolysis, elevated lactate dehydrogenase (LDH), thrombocytopenia, and a low reticulocyte count, which can be incorrectly diagnosed as thrombotic thrombocytopenic purpura and managed incorrectl

Impaired intestinal vitamin B1 (thiamin) uptake in thiamin transporter-2-deficient mice.

BACKGROUND & AIMS: Intestinal thiamin uptake process is vital for maintaining normal body homeostasis of the vitamin; in vitro studies suggest that both thiamin transporter-1 (THTR-1) and -2 (THTR-2) are involved. Mutations in THTR-1 cause thiamin-responsive megaloblastic anemia, a tissue-specific disease associated with diabetes mellitus, megaloblastic anemia, and sensorineural deafness. However, in patients with thiamin-responsive megaloblastic anemia, plasma thiamin levels are within normal range, indicating that THTR-2 (or another carrier) could provide sufficient intestinal thiamin absorp
