The level of 6-oxymelatonin sulfate, the main metabolite of melatonin (MT), in morning urine samples from patients with metabolic syndrome (MS) suggests enhanced MT production at night. This inference equally holds for aged and elderly patients despite their significantly impaired MT secretion due to age-related involution of the pineal gland. Correlation between MT hypersecretion and manifestations of MT activity also depends on the patients" age. MT test can be used as biological marker of MT and a prognostic factor of MS progress.

The pattern of melatonin secretion is controlled by an endogenous circadian timing system and conveys information about the light-dark cycle to the organism, thereby organizing its seasonal and circadian rhythms. A delicate synchronization of many inputs at these three different levels is vital for normal reproductive function. From the median basal hypothalamus, the median eminence releases gonadotrophin releasing hormone into the portal circulation to reach the anterior pituitary gland. Melatonin produces a great variety of effects in human organism and plays an essential role in normal function of the body. This explains growing interest of medical researchers to this substance. The paper provides the results of 3-year studies of seasonal rhythm of melatonin production in patients with duodenal ulcer. Acute disorders of circadian and seasonal melatonin production in summer and autumn were discovered in patients with duodenal ulcer. These findings suggest involvement of abnormal melatonin production in pathological mechanisms of autumn exacerbations of duodenal ulcer.

It is generally accepted that migraine is caused by a primary biochemical disorder of the central nervous system involving neurotransmitters, specifically serotonin. The pathogenetic mechanism triggered by external and internal stimuli is not well explained or understood. This article points to the possibility that the pineal gland, a primary source of central serotonin and melatonin, contributes significantly to migraine attacks.
