We report on a boy with pseudo-cleft of the upper lip, cleft palate, bifid uvula, lobulated tongue, hypoplasia of the epiglottis, both preaxial and central polydactyly of the hands (Y-shaped fourth metacarpals), bilateral preaxial polydactyly of the feet, postaxial polydactyly of the left foot, hearing impairment, and congenital heart disease with endocardial cushion defect. We report a displaced body of hamate fracture in the coronal plane with dislocation of the ring and little metacarpals and interposition of these metacarpals between the two hamate fragments. The diagnosis, radiographic studies and the treatment of this unusual injury are discussed.

A 40-year-old patient presented with traumatic amputation of the thumb and index finger. The second metacarpal bone fragment was transposed to lengthen the first metacarpal bone. A free flap from the lateral forearm was used to reconstruct the thenar region. Later, the second free toe-to-hand transfer was done from which he made a good recovery, achieving active flexion and extension in all joints.

We present a case of a child born with metacarpal synostosis of the index, long, and ring fingers associated with a simple syndactyly of the small finger and an absent thumb. At age 7 months, a syndactyly release with skin grafting to the small finger was accomplished so that it became the new thumb. Hand deformities in 2 wild-born pygmy chimpanzees were investigated through dissection, linear measurements, bone mineral analysis and X-rays. Weight of tissue (skin, muscle, bone) was determined for each body segment (hand, forearm, upper arm), permitting comparison between the abnormal and normal upper limbs. In one individual, the right and left forearms and hands differed in tissue weights and bone mineral content. In the other, the hands differed in tissue composition, but not the forearms.

A prospective study of 73 patients with fractures of the metacarpal neck of the little finger is reported. Fifty-seven (78 per cent) of the patients were followed up between 1 and 2 years after the injury. Forty-three (75 per cent) of these patients complained of symptoms, though only one patient considered himself disabled. The normal head/shaft angle at the fifth metacarpal neck measured in 12 control subjects as viewed on an oblique radiograph was 26 degrees. Familial prevalence of mitral valve prolapse in a Chinese population was determined in 22 propositi of whom 10 had straight back (group A), three had abnormally high metacarpal index (group B), and nine had neither (group C). Of 71 (32 male and 39 female subjects) first degree relatives screened, mitral valve prolapse was found in 19 (seven male and 12 female subjects) (26.8%). A case of Ewing's Sarcoma of the thumb is described. Four years after a fracture of the first metacarpal, followed by a series of repeated traumata to the area, a tumour was discovered. According to Ewing's criteria of trauma induced malignancy, this case is highly suggestive to have been caused by trauma.

The cases of four patients with carpometacarpal fracture-dislocations of the ulnar four joints are reported, in all of whom there was extensive additional trauma. Open reduction was necessary in three patients, and only one was treated successfully by a closed reduction. He then had persistent but painless subluxation of the metacarpal bases.
