Extracellular Vesicles in Hemostasis.

Extracellular vesicles (EVs) are key modulators of hemostasis, influencing coagulation under physiologic and pathologic conditions. Depending on their cellular origin and their releasing conditions, EVs promote coagulation predominantly by the exposure of phosphatidylserine and tissue factor. Elevated EV levels have been reported in various diseases associated with increased thrombotic risk. This review explores how distinct EV subpopulations in the circulation contribute to hemostasis and thrombosis. Furthermore, it highlights their emerging potential as biomarkers for diagnosing and monitori

Radio-Telemetric Assessment of Cardiac Variables and Locomotion With Experimentally Induced Hypermagnesemia in Horses Using Chronically Implanted Catheters.

The objective of this study was to characterize the pharmacokinetics and pharmacodynamics of intravenous administration of magnesium sulfate to horses using a novel radio-telemetry system for physiologic signal capture. Five Horses were surgically implanted with a radio-telemetric carotid catheter. Implants were paired with a non-invasive telemetric unit which acquired a six lead ECG and 3-axis acceleration to assess activity acquired wirelessly in real-time for future analysis. Horses were exposed to a new stall environment before (baseline) and after 60 mg/kg (30 mL) of magnesium sulfate (Mg

Total laparoscopic surgery of cystic adenomyoma under hydroultrasonographic monitoring.

A 19-year-old-woman had a cystic adenomyoma located within the myometrium. She complained of severe dysmenorrhea. Gonadotropin-releasing hormone agonist therapy was administered, but her dysmenorrhea was more pronounced than before treatment. Therefore, total laparoscopic resection of the lesion was performed. The external appearance of the patient's uterus was almost normal. A hydroultrasonographic monitoring method was devised (transvaginal ultrasonography with peritoneal hydration of physiologic saline) that proved useful in locating the adenomyoma. The patient's dysmenorrhea disappeared po

Real-time telemetric monitoring in whole-body 60Co gamma-photon irradiated rhesus macaques (Macaca mulatta).

BACKGROUND: Animals undergoing experimental manipulations, such as exposure to radiation, may exhibit physiologic and behavioral signs of pain and distress. Telemetry permits close monitoring of these parameters for early and effective management during procedures. METHODS: Radiotelemetric units were surgically implanted into 24 Macaca mulatta before 6.5-Gy cobalt-60 γ-photon irradiation. Each unit transmitted electrocardiogram, intrathoracic pressure, and body temperature leads. Primate irradiation-restraint boxes and housing cages were modified to collect telemetric signals before, during,

Personalized mechanical ventilation in acute respiratory distress syndrome.

A personalized mechanical ventilation approach for patients with adult respiratory distress syndrome (ARDS) based on lung physiology and morphology, ARDS etiology, lung imaging, and biological phenotypes may improve ventilation practice and outcome. However, additional research is warranted before personalized mechanical ventilation strategies can be applied at the bedside. Ventilatory parameters should be titrated based on close monitoring of targeted physiologic variables and individualized goals. Although low tidal volume (VT) is a standard of care, further individualization of VT may neces

Exploring the temporal statistical relationship between commercial cerebral near-infrared spectroscopy signals and intracranial pressure in traumatic brain injury.

BACKGROUND: Monitoring of intracranial pressure (ICP) is important for not only assessing neurological status of patients following traumatic brain injury (TBI), but also for guiding therapeutic interventions for the prevention of secondary neurological injury. Although current ICP monitoring techniques are highly temporally accurate, they come with some significant limitations. These limitations include the invasive nature, expertise required for placement of these devices, high cost associated with maintenance, and poor spatial resolution. Cerebral near-infrared spectroscopy (NIRS) offers a

Myocardial Positron Emission Tomography for Evaluation of Cardiac Sarcoidosis: Specialized Protocols for Better Diagnosis.

Sarcoidosis is a multisystemic granulomatous disease of unknown etiology with various clinical presentations depending on the organs involved. Since cardiac sarcoidosis (CS) portends a higher risk of morbidity and mortality, early diagnosis and aggressive medical treatment are essential to improve the prognosis. 18F-Fluorodeoxyglucose (FDG) positron emission tomography (PET) has emerged as an important tool with practical advantages in assessing disease activity and monitoring the treatment response in patients with CS. While it has high sensitivity, it also has great variability in specificit

Validation of the inadequate delivery of oxygen index in an adult cardiovascular intensive care unit.

OBJECTIVE: Machine learning (ML) may allow for improved discernment of hemodynamics and oxygen delivery compared to standard invasive monitoring. We hypothesized that an ML algorithm could predict impaired delivery of oxygen (IDO2) with comparable discrimination to invasive mixed venous oxygen saturation (SvO2) measurement. METHODS: A total of 230 patients not on mechanical circulatory support (MCS) managed with a pulmonary artery catheter (PAC) were identified from 1012 patients admitted to a single cardiovascular intensive care unit (CVICU) between April 2021 and January 2022. Transcutaneous carbon dioxide monitoring in infants and children.

OBJECTIVE: To review the technology required for and the applications of transcutaneous carbon dioxide (TC-CO2) monitoring in infants and children. DATA SOURCE: A computerized, bibliographic search regarding the applications of transcutaneous carbon dioxide (TC-CO2) monitoring in infants and children. RESULTS: Although the direct measurement of P(a)CO2 remains the gold standard, it provides only a single measurement of what is often a rapidly changing and evolving clinical picture. Given these concerns, there remains a clinical need for a means to continuously monitor P(a)CO2 without the nee

Detecting initial orthostatic hypotension: a novel approach.

Our purpose, by modification of standard bedside tilt-testing, was to search for lesser known but important initial orthostatic hypotension (IOH), occurring transiently within the first 30 seconds of standing, heretofore only detectable with sophisticated continuous photoplethysmographic monitoring systems, not readily available in most medical facilities. In screened outpatients over 60 years of age, supine blood pressure (BP) parameters were recorded. To achieve readiness for immediate BP after standing, the cuff was re-inflated prior to standing, rather than after. Immediate, 1-, and 3-minu

A transformer-graph neural network framework for cyber twin-driven personalized respiratory monitoring.

Maintaining healthy breathing is important to health and preventing serious respiratory complications, many of which can result in death. Individuals living with ongoing chronic respiratory disease (asthma, chronic obstructive pulmonary disease, sleep apnea) need ongoing personal patient-centred continuous non-hospital-based monitoring for diagnosis and appropriate treatment. Many traditional existing patient-centred physical health monitoring systems are insufficient due to their limited flexibility, combining multiple sources of sensor data that monitor patients continuously and act in real

Monitoring and modulation of respiratory drive in patients with acute hypoxemic respiratory failure in spontaneous breathing.

Non-invasive respiratory support, namely, non-invasive ventilation, continuous positive airway pressure, and high-flow nasal cannula, has been increasingly used worldwide to treat acute hypoxemic respiratory failure, giving the benefits of keeping spontaneous breathing preserved. In this scenario, monitoring and controlling respiratory drive could be helpful to avoid patient self-inflicted lung injury and promptly identify those patients that require an upgrade to invasive mechanical ventilation. In this review, we first describe the physiological components affecting respiratory drive to outl

Patient's airway monitoring during cardiopulmonary resuscitation using deep networks.

Cardiopulmonary resuscitation (CPR) is a crucial life-saving technique commonly administered to individuals experiencing cardiac arrest. Among the important aspects of CPR is ensuring the correct airway position of the patient, which is typically monitored by human tutors or supervisors. This study aims to utilize deep transfer learning for the detection of the patient's correct and incorrect airway position during cardiopulmonary resuscitation. To address the challenge of identifying the airway position, we curated a dataset consisting of 198 recorded video sequences, each lasting 6-8 s, show

Change-point detection using diffusion maps for sleep apnea monitoring with contact-free sensors.

Monitoring and improving the quality of sleep are crucial from a public health perspective. In this study, we propose a change-point detection method using diffusion maps for a more accurate detection of respiratory arrest points. Conventional change-point detection methods are limited when dealing with complex nonlinear data structures, and the proposed method overcomes these limitations. The proposed method embeds subsequence data in a low-dimensional space while considering the global and local structures of the data and uses the distance between the data as the score of the change point. An All-in-One Array of Pressure Sensors and sEMG Electrodes for Scoliosis Monitoring.

Scoliosis often occurs in adolescents and seriously affects physical development and health. Traditionally, medical imaging is the most common means of evaluating the corrective effect of bracing during treatment. However, the imaging approach falls short in providing real-time feedback, and the optimal corrective force remains unclear, potentially slowing the patient's recovery progress. To tackle these challenges, an all-in-one integrated array of pressure sensors and sEMG electrodes based on hierarchical MXene/chitosan/polydimethylsiloxane (PDMS)/polyurethane sponge and MXene/polyimide (PI)

Technical validation of the EMMA capnometer under hyperbaric conditions.

INTRODUCTION: End-tidal carbon dioxide (ETCO2) monitoring is essential for monitoring intubated critical care patients, yet its use in hyperbaric environments can be problematic. We postulated that the EMMA mainstream capnometer may function accurately under hyperbaric conditions. METHODS: Stage 1. The EMMA mainstream capnometer was tested at 101 kPa against a reference side-stream capnometer, Philips IntelliVue M3015B microstream, using 10 customised reference gases of various carbon dioxide (CO₂) concentrations (2.47%-8.09%, or 18.5-60.7 mmHg at 101 kPa) in either air or oxygen. Stage 2. Th

Management of Status Epilepticus and Indications for Inpatient Electroencephalography Monitoring.

Status epilepticus (SE) is a neurologic emergency requiring immediate time-sensitive treatment to minimize neuronal injury and systemic complications. Minimizing time to administration of first- and second-line therapy is necessary to optimize the chances of successful seizure termination in generalized convulsive SE (GCSE). The approach to refractory and superrefractory GCSE is less well defined. Multiple agents with differing complementary actions that facilitate seizure termination are recommended. Nonconvulsive SE (NCSE) has a wide range of presentations and approaches to treatment. Contin
