[Movement disorders in adult-onset measles encephalitis].

There are three different neurological complications of measles infections in the brain: acute postinfectious encephalitis, acute progressive infectious encephalitis, and subacute sclerosing panencephalitis. The diagnosis of measles encephalitis (ME) is established when supported by the clinical picture, mainly of juvenile onset, and confirmed by the presence of cerebrospinal measles antibodies. Although ME is clinically characterized by progressive behavioral and mental deterioration associated with myoclonus, prior reports have suggested that adult-onset may have atypical features. We descri

[Movement disorders and motor activity during sleep].

OBJECTIVE: To describe different types of motor activity and movement disorder which occur during sleep in relation to episodes of arousal and sleep disorder. DEVELOPMENT: During sleep, normal motor activity should be distinguished from paroxystic episodes: parasomnias; abnormal movements such as nocturnal paroxystic dystonia, which is very similar to epilepsy of frontal origin; nocturnal epileptic crises and especially periodic movements of the limbs and the restless legs syndrome, which is relates lo it. Physiological cyclical fluctuations of sleep are common to all these conditions and due

Selective serotonin-reuptake inhibitor-induced movement disorders.

OBJECTIVE: To compile and evaluate all available data suggesting an association between selective serotonin-reuptake inhibitor (SSRI) administration and the occurrence of movement disorders, and to characterize these reactions in terms of onset, duration, treatment and outcome, and potential predisposing factors. METHODOLOGY: Reports of movement disorders were identified by conducting a comprehensive literature search that included tertiary adverse drug reaction resources, MEDLINE, EmBASE, Biological Abstracts, Current Contents, Reactions, ClinAlert, and International Pharmaceutical Abstracts…

Adult head-banging and stereotypic movement disorders.

Stereotypic movement disorders (SMD) such as head-banging, which are common among children with mental retardation or pervasive developmental disorders, may also occur in intellectually normal adults. We report a 27-year history of daily head-banging with self-injury in a 49-year-old man with normal cognition. The patient had no personal or family history of Tourette's syndrome, tic disorder, obsessive-compulsive disorder (OCD), or mental retardation. The frequency of his stereotypical head-banging increased with anxiety, loud noises with startle, and boredom. He reported a sense of pleasure f…

Mesencephalic clefts with associated eye movement disorders.

OBJECTIVE: To describe two patients with mesencephalic midline clefts and associated eye movement disorders. DESIGN: Case reports. RESULTS: The first patient developed bilateral internuclear ophthalmoplegia with exotropia, reduced convergence, right ptosis, right fourth-nerve palsy, and right elevator palsy several years after meningitis with hydrocephalus. The second patient had bilateral internuclear ophthalmoplegia with exotropia, reduced convergence, bilateral ptosis, limited upward gaze, and right hypertropia since childhood. In both patients, magnetic resonance imaging showed a midline…

Prevalence of movement disorders in institutionalized elderly.

The proportion of the population over age 65 years is steadily increasing in Canada and the elderly who are unable to live independently are frequently institutionalized in chronic care facilities. We report our observations on movement disorders (MD) in an institutionalized elderly population of Saskatchewan. A representative sample of 67 subjects aged > or = 65 years had a detailed neurological evaluation on three separate occasions. MD were detected in 13 (19%) cases. The majority, 11 (16%), were females. Seven (10%) had essential tremor, 4 (6%) Parkinson's disease and 2 (3%) had drug-induc…

Treatment of acute neuroleptic-induced movement disorders.

Acute extrapyramidal syndromes (EPS), including dystonia, parkinsonism, and akathisia, are associated with the use of virtually all neuroleptic agents. They may be alleviated by reducing the neuroleptic dosage, switching to a lower-potency drug, or administering an adjunctive agent such as an anticholinergic, amantadine, benzodiazepine, or beta-blocker. Akathisia may be only partly dispelled by anticholinergics; alternatives are beta-blockers, benzodiazepines, and clonidine. In patients receiving long-term neuroleptic therapy, both the prophylactic use and the duration of treatment with concom

Childhood movement disorders and obsessive compulsive disorder.

Recent investigations of childhood-onset obsessive compulsive disorder (OCD) and pediatric movement disorders such as tics, Tourette's syndrome (TS), and Sydenham's chorea suggest that these disorders may be related. Although comorbid obsessive-compulsive symptoms have long been recognized in individuals with TS, more recent studies have demonstrated that tics and TS are surprisingly common in children with primary OCD, and further, that the two disorders seem to have a common genetic vulnerability. Obsessive-compulsive symptoms are also manifest in Sydenham's chorea, a neurologic variant of r…

[Clinical diagnosis of movement disorders--evoked potentials].

As a clinical diagnostic approach, the evoked potential has been used to differentiate movement disorders, especially myoclonic movements, Giant somatosensory evoked potentials (SEPs) and long-loop reflex to electrical stimulation of the peripheral nerve are commonly employed in the evaluation of patients with myoclonus. Recently, abnormality in a specific component of SEP frontal N30 has been reported in focal dystonia. Jerk locked back averaging has been applied to cortical reflex myoclonus, minipolymyoclonus, cortical tremor, simple tics, chorea-acanthocytosis, Huntington's chorea, etc. Sil…

Hereditary motor-sensory neuropathy and movement disorders.

To explore the relationship between hereditary motor and sensory neuropathy (HMSN) and movement disorders, we examined 7 patients with HMSN referred to our Movement Disorders Clinic and surveyed members of the Charcot-Marie-Tooth association. The following movement disorders were observed in the index patients: postural tremor in 6, rest tremor in 3, and Parkinsonism and dystonia in 2. Tremor, present in 40% of the 201 patients who responded to the survey, was first noted at a mean age of 36 years, and mostly involved the hands. Family history of tremor was more frequent in the tremor group (P…

[Treatment of movement disorders using botulinum toxin].

Intramuscular injections of botulinum toxin (Botox) are followed by a dose-dependent focal paresis which can be used to treat several focal movement disorders. Botox injections are recommended as effective for the treatment of blepharospasm, hemifacial spasm, and cervical dystonia (torticollis). Focal dystonias elsewhere (for example, writer's cramp) can often be treated with similar success. Others, such as oromandibular dystonia, are more difficult to treat. In the case of more generalized dystonias, some focal muscle spasms can be treated with success by local intramuscular injections. New …

Risk factors for neuroleptic-induced movement disorders.

Chronic neuroleptic therapy may be associated with the development of diverse movement disorders including Tardive dyskinesia (TD), Parkinsonism, dystonia, and akathisia in a subset of schizophrenic patients. It is presently unknown why only a proportion of neuroleptic-treated patients develop these movement disorders. In the following communication, we present a series of studies which demonstrate that the development of these movement disorders may be facilitated by certain risk factors including disturbances in pineal melatonin functions, diabetes mellitus, cognitive deficits, suicidal beha…

Vermal infarct with pursuit eye movement disorders.

Severe deficits of foveal smooth pursuit and optokinetic nystagmus in all directions were electro-oculographically recorded in an 80 year old woman. Magnetic resonance imaging (MRI) showed an infarct involving the postero-inferior part of the vermis (lobules VI to X) and a portion of the left cerebellar hemisphere, with apparent preservation of the flocculus and the brainstem. The role of the vermal lesion in these pursuit eye movement disorders is discussed.

Drug-induced parkinsonism and other movement disorders.

This is a review of reserpine, haloperidol, and various phenothiazines that produce parkinsonism and other movement disorders. The by-products of illicit meperidine synthesis, MPTP and its more sinister companion, MPP, are also discussed. Movement disorders, transient or fixed, frank parkinsonism and/or dyskinesia, due to a variety of other medications and toxic agents are included. These are methanol, lithium, methyldopa, antimetabolites, antidepressants, sympathomimetic anorexiants, some types of antihistamines, and various combinations of agricultural chemicals.

Eyelid movement disorders following unilateral hemispheric stroke.

The presence of voluntary eyelid movement disorders was systematically checked in 51 patients with a recent stroke supposedly confined to one hemisphere and presenting with contralateral gaze impairment. Inability to initiate lid closure was found in 5 patients, inability to keep the eyes closed for more than 1-3 sec in 5 patients and inability to initiate eyelid opening in 1 patient. One patient failed both to close his eyes and to open them when closed. In 3 patients the deficit was restricted to the eye contralateral to the lesion. CT scan findings pointed to the parietal lobe as the cortic…

Treatment of involuntary movement disorders with tetrabenazine.

Seventeen patients with choreiform, athetoid, or ballistic involuntary movements, or with spasmodic torticollis, were treated with tetrabenazine in doses of 25 to 200 mg daily for periods varying from two weeks to more than six months. Randomized ciné film of the patients' involuntary movements, taken before, during, and after treatment was assessed individually by seven `blind' observers. Eight patients were judged improved; two had Huntington's chorea, two athetosis, two dystonia musculorum deformans, one hemiballismus, and one spasmodic torticollis. Four of the eight improved patients have

Neuroscience of human ventral lateral thalamic nucleus related to movement and movement disorders.

The ventral lateral nucleus (VL) in the human ventral group exhibits activity associated with active and passive limb movements, and pathological movements found in patients with movement disorders. This group includes the anterior (VLa) and posterior (VLp) nuclei, which receive excitatory inputs from the deep cerebellar nuclei and inhibitory inputs from the internal segment of the globus pallidus (GPi). These nuclei primarily project to the primary and supplementary motor cortices. Despite differences in input sources, neurons in VLa and VLp often show similar activity patterns during active …
