[Long-term recovery of regional wall motion in patients with medically treated anterior myocardial infarction: quantitative assessment of the post-infarction left ventriculograms].

The effect of medical treatment without reperfusion therapy on the long-term recovery of regional wall motion was evaluated retrospectively in 28 patients with transmural acute anterior myocardial infarction who had coronary angiography and left ventriculography at 1-6 months after the onset of the episode and were followed for a mean of 65 months. In all patients, initial coronary angiography revealed significant stenosis in only the left anterior descending artery (LAD). All patients were treated medically without reperfusion therapy (such as thrombolytic therapy, angioplasty, or bypass graf

[Detection of wall motion abnormality in old myocardial infarction with principal component analysis to body surface potential distribution].

The purpose of this study was to examine the correlation between body surface potential distribution and the extent of abnormally contracting segments (ACS) of the left ventricle in patients with old myocardial infarction (MI). Body surface potential distribution was quantitatively analyzed using principal component analysis. The first 6 principal components were extracted from data set obtained from both 25 normal subjects and 100 patients. The z scores were calculated at every 4 or 8 msec in another 81 patients with previous MI. In conclusion, the principal component analysis on body surface

Relation between the transmural extent of acute myocardial infarction and associated myocardial contractility two weeks after infarction.

To characterize the relation between the transmural extent of acute myocardial infarction (AMI) and associated regional contractility after recovery from ischemia, 11 mongrel dogs underwent occlusion of the proximal left anterior descending coronary artery and were evaluated 2 weeks after infarction. Occlusion was permanent in 5 dogs, and reperfusion was allowed after 2 hours of occlusion in 6 dogs. All dogs had computer-assisted quantitative wall-thickening analysis by 2-dimensional echocardiography and infarct localization by the triphenyl-tetrazolium chloride technique. Percent systolic wal

Monitoring pulmonary artery pressures in critically ill patients.

A flow-directed Swan-Ganz catheter was used for the catheterisation of the pulmonary artery in critically ill patients. The monitoring of pulmonary artery pressures provided reliable information regarding left ventricular function and pulmonary vascular resistance. Moreover, the pulmonary artery catheter can be used for mixed venous blood sampling. Experiences with pulmonary pressure monitoring for the assessment of left ventricular performance in patients with myocardial infarction and left ventricular failure, for the estimation of lung function in patients with respiratory failure and for t

Myocardial Doppler at rest for the identification of myocardial viability.

BACKGROUND: Echocardiography may permit the detection of a nonviable myocardium. The aim of this study was to test if resting pulsed wave-tissue Doppler imaging (PW-TDI) might yield additional markers. METHODS: Fifty patients (38 males, 12 females, mean age 63 +/- 6 years) with left ventricular dysfunction (ejection fraction 35 +/- 10%) underwent echocardiography. The posterior septum, anterior septum, lateral, inferior, anterior and posterior walls were sampled on the basal segments in the apical views at PW-TDI. The following variables and cardiac phases were tested: 1) the isovolumic contr

Is positron emission tomography enough to rule out cardiac sarcoidosis? A case report.

BACKGROUND: Cardiac sarcoidosis (CS) is associated with poor prognosis, yet the clinical diagnosis is often challenging. Advanced cardiac imaging including cardiac magnetic resonance (CMR) and positron emission tomographic (PET) have emerged as useful modalities to diagnose CS. CASE SUMMARY: A 66-year-old woman presented with palpitations. A 24-h Holter monitor detected a high premature ventricular contraction burden of 25.6%. She underwent two transthoracic echocardiograms; both showed normal results. Stress perfusion CMR did not show any evidence of ischaemic aetiology; however, myocardial

Inferior epigastric artery as a conduit for myocardial revascularization.

In 18 patients the inferior epigastric artery (IEA) was used for myocardial revascularization along with the left internal mammary artery and saphenous vein (4.5 grafts/patient). Preoperative duplex scan assessment of IEA correctly predicted its distal diameter. At operation the mean internal diameter was 1.6 (1.5-2.0) mm distally and 2.1 (2.0-2.5) mm proximally. The mean in situ flow after intraluminal injection of papaverine was 115 (36-210) ml/min in IEA and 136 (56-210) ml/min in internal mammary artery. There was no perioperative death or myocardial infarction. Morbidity associated with I

[The treatment of chronic left ventricular insufficiency].

Left ventricular failure is caused by a variety of myocardial diseases and its symptoms results from adjustments attempted by the left ventricle and the circulatory system to cope with the initial myocardial pathology. Treatment of heart failure has various objectives, depending on whether one tries to correct the initial myocardial disease, or its consequences on the myocardium, or its repercussions on blood circulation, or, more simply, to alleviate the symptoms experienced by the patient. The ideal treatment of heart failure would include a drug acting on its cause, another drug to restore

Cardiac injury in short duration anoxia and modification by diltiazem, a calcium channel blocking agent.

The protective effects of diltiazem were studied in isolated perfused rat hearts after 10, 20 and 30 minutes of anoxia and 10 minutes of reoxygenation. The hearts were treated with diltiazem, 4 mg/liter, during anoxia and reoxygenation. The myocardial tissue was processed for electron microscopy and tissue calcium was measured. Four types of cell injury ranging from normal to severe were observed. The prominent morphologic changes in the nondiltiazem-treated tissue were contraction bands, distortion and calcification of mitochondria and loss of glycogen. In the treated group, a partial reducti

Role of pannexin and adenosine triphosphate (ATP) following myocardial ischemia/reperfusion.

OBJECTIVES: The purinergic system has not been investigated in detail following ischemia/reperfusion (I/R) injury in the heart. In the present study, we focus on both release and response to extracellular adenosine triphosphate (ATP). Pannexin (Panx) channels have been shown to be involved in ATP release from myocytes and can activate P2X1 and P2Y2 receptors on the coronary artery. DESIGN: We applied a well-characterized I/R model in rats, with 24 hours of reperfusion. Panx expression in the myocardial tissue was measured with quantitative polymerase chain reaction (qPCR) and flow cytometry.

Intracoronary Delivery of Mitochondria to the Ischemic Heart for Cardioprotection.

We have previously shown that transplantation of autologously derived, respiration-competent mitochondria by direct injection into the heart following transient ischemia and reperfusion enhances cell viability and contractile function. To increase the therapeutic potential of this approach, we investigated whether exogenous mitochondria can be effectively delivered through the coronary vasculature to protect the ischemic myocardium and studied the fate of these transplanted organelles in the heart. Langendorff-perfused rabbit hearts were subjected to 30 minutes of ischemia and then reperfused

Pharmacologic stress myocardial perfusion imaging.

Pharmacologic stress agents (dipyridamole, adenosine and dobutamine) allow virtually all patients to be safely assessed for ischemic heart disease. These agents have mild but significant side effects, mandating a thorough knowledge of indications, contraindications, side effects and management before their use. Adjunctive exercise improves image quality in vasodilator pharmacologic myocardial perfusion imaging. Diabetics, especially women, have a much higher cardiac event rate than nondiabetics for an equal amount of ischemia. They also have a higher incidence of asymptomatic ischemia. There i

Myocardial perfusion and wall motion in infarction border zone: assessment by myocardial contrast echocardiography.

Several mechanisms have been proposed to explain the decreased wall motion (WM) at the borders of myocardial infarction (MI). We used myocardial contrast echocardiography (MCE) to investigate the relation of perfusion to WM in infarcted border zones (BZs) 6 weeks after MI in 5 sheep. After quantifying the extent of WM abnormality and the perfusion defect, normal (NL), infarcted, and BZs were defined. Peak intensity after contrast was measured in acoustic units (AU). Radiolabeled microspheres were injected to measure regional blood flow. The heart was stained with 2,3, 5-triphenyltetrazolium ch

Wall thickness changes considered as regional myocardial function in ischemic heart disease.

Dynamic changes in left ventricular wall thickness represent a function of the myocardium which can be described, in normal subjects or in disease, without reference to behavior of other parts of the wall. Abnormalities of amplitude, rate and timing of wall thickness change interact in influencing overall ventricular performance, so that even this local function cannot be expressed as a single "index". Infarcted and acutely ischemic myocardium display reduced amplitude of thickness change. The degree to which they also show delayed wall dynamics in man is uncertain. Cross-sectional echocardiog

Detection of myocardial ischemia.

Myocardial ischemia causes a series of anatomic and physiologic abnormalities that can be detected and quantified by assessment of myocardial perfusion, mechanical function, electrophysiology, and metabolism. These methods of assessment vary widely in sensitivity, specificity, relevance, cost, and ease of application. Although occasionally the appropriate choice of diagnostic procedures is clinically difficult, the demonstrated potential of coronary artery bypass surgery to reverse both acute and chronic myocardial ischemia makes the detection of ischemia an important effort in the care of pat

Effect of cardioselective beta-blockade on heart function and chest pain in acute myocardial infarction.

Systolic time intervals and the a/H ratio were recorded in 20 patients with uncomplicated acute myocardial infarction over a period of five days. The initial high heart rate and systolic blood pressure and the short PEP and ICT indicating a sympathetic overactivity were spontaneously normalized during the first week of infarction. LVET was reduced indicating a fall in stroke volume and the a/H ratio was unchanged at the high levels suggestive of elevated preload or LVEDP. In 10 patients with acute myocardial infarction and recurrent chest pain recordings on noninvasive parameters were made bef
