We measured the alignment of the nails in the hands of 100 normal adults with the interphalangeal joints extended and the metacarpophalangeal (MCP) joints at 0 degrees and 90 degrees. All fingers were naturally supinated (i.e. rotated towards the thumb), the index and little being the most supinated. When examining individual hands in MCP joint extension, only 17% of hands had all nails parallel. With the MCP joints flexed, this improved to 56%. A total of 61 patients with a proximal humeral fracture was treated between January 1996 and March 1998 by closed reduction and fracture fixation with intramedullary Prévot (or Nancy) nails. A treatment protocol is demonstrated, consisting of an osteotomy, either proximal or distal, of the bone defect with subsequent segmental transport via an anteromedially (tibia) or laterally (femur) mounted AO external fixation over an unreamed interlocking nail (monorail system). Twenty patients were treated by this method with indications as follows: 13 had a segmental bone defect of the tibia, 3 of the femur. Three patients showed post-traumatic and postinfectious leg-length discrepancies and one was treated for hypertrophic non-union of the femur. Defect distance varied between 5 and 18.5

Two men (non smokers) suffer from unilateral Raynaud's phenomenon, as well as homolateral splinter hemorrhages of the nails. One of the patients has digital artery obstruction, confirmed angiographically. The other patient, in whom a thoracic outlet syndrome was diagnosed, suffers of cold-induced vascular spasm.
