Lower airway dysfunction in patients with chronic rhinosinusitis with nasal polyps (CRSwNP): associations with asthma, ACQ5, and inflammatory markers.

OBJECTIVE: CRSwNP is typically a type 2 inflammatory condition of the upper airways often associated with asthma. However, emerging evidence suggests that lower airway dysfunction may also occur in CRSwNP patients without a formal asthma diagnosis. This study aimed to explore the associations between lower airway symptoms (ACQ5), lung function, airway hyperresponsiveness (AHR), and inflammatory markers in patients with CRSwNP. METHODS: In this cross-sectional study, 72 patients with CRSwNP were systematically evaluated using spirometry, impulse oscillometry (IOS), methacholine and mannitol ch

Occupational rhinitis.

Work-related rhinitis, which includes work-exacerbated rhinitis and occupational rhinoconjunctivitis (OR), is two to three times more common than occupational asthma. High molecular weight proteins and low molecular weight chemicals have been implicated as causes of OR. The diagnosis of work-related rhinitis is established based on occupational history and documentation of immunoglobulin E (IgE) mediated sensitization to the causative agent if possible. Management of work-related rhinitis is similar to that of other causes of rhinitis and includes elimination or reduction of exposure to causat

[Comparison of nasal histamine and allergen provocations in patients with allergic rhinitis].

OBJECTIVES: We evaluated the effects of histamine and allergen provocations in the nasal mucosa. PATIENTS AND METHODS: Nasal provocations were performed with extracts of histamine, Dermatophagoides pteronyssinus (Dp) and mixed grass pollen (Mg) in patients with perennial and seasonal allergic rhinitis and in healthy subjects. Nasal airway resistance (NAR), the amount of secretions, and the number of sneezes were recorded and evaluated. RESULTS: Regarding NAR, histamine provocation resulted in a significant difference compared to Dp provocation, but the difference was not significant when com

Anaphylaxis to calcitonin.

BACKGROUND: The calcitonin is an hormone produced by the thyroid gland C cells. The salmon calcitonin is used in some osteomuscular diseases. There are few references of allergic reaction to this hormone. We introduce a case of a sixty years old woman with several previous episodes of rhinitis, conjunctivitis and perspiration immediately after the administration of salmon calcitonin with nasal spray or intramuscular administration (Calsynar). There were some temporal periods of good tolerance between these episodes. METHODS: Skin prick test (SPT), nasal and intramuscular challenge test with c

Intralymphatic immunotherapy improves grass pollen allergic rhinoconjunctivitis: A 3-year randomized placebo-controlled trial.

BACKGROUND: Allergic rhinoconjunctivitis is a global health problem. Different allergen immunotherapy regimes are marketed but have low adherence because they are expensive, complex, and time-consuming. New allergen immunotherapy forms are needed. OBJECTIVE: In a 3-year follow-up double-blind randomized placebo-controlled trial, we aimed to investigate the effect of intralymphatic allergen immunotherapy (ILIT). METHODS: Patients with grass pollen rhinoconjunctivitis were treated with 3 ILIT injections and an ILIT booster 1 year later, 3 ILIT injections and a placebo booster, or 3 placebo inj

The Role of Nasal Cytology in the Phenotyping and Monitoring of Chronic Rhinitis in Children.

BACKGROUND: Local allergic rhinitis (LAR) is a rhinitis subtype characterised by an IgE-mediated response in the nasal mucosa. Although the nasal provocation test (NPT) is the diagnostic gold standard, it is impractical in many centres. Consequently, patients are often misclassified as having non-allergic rhinitis, delaying appropriate treatment. This study evaluated the role of nasal cytology in the classifying and monitoring paediatric allergic rhinitis (AR), distinguishing probable LAR (pLAR), and guiding treatment. METHODS: This retrospective study analysed data from 255 patients diagnose

Important roles of tachykinins in the development of allergic nasal hyperresponsiveness in guinea-pigs.

BACKGROUND: Although it has been suggested that the use of tachykinin receptor antagonists might prove to be an effective treatment for allergic rhinitis (AR), they are not used clinically. Therefore, we decided to examine the effects of tachykinin receptor antagonists on AR symptoms in an appropriate experimental model. OBJECTIVE: To evaluate newly developed tachykinin receptor antagonists in a Japanese cedar pollen-induced AR model and to determine their effect on allergen-induced sneezing, nasal blockage, and nasal hyperresponsiveness (NHR). METHODS: Sensitized guinea-pigs were challenged

[Is celecoxib a safe alternative for the Fernand Widal syndrome?].

The Fernand Widal syndrome is a set of associations between asthma, nasal polyposis and aspirin sensitivity. Selective cyclo-oxygenase 2 (COX 2) inhibitors are recognized as being a therapeutic alternative in cases needing analgesic or anti-inflammatory treatment. In a retrospective study, we have compiled data concerning oral provocation tests (OPT) undertaken with celecoxib, one of most the selective COX 2 inhibitors, in eight patients with the Fernand Widal syndrome. They were compared with twenty-seven control patients with sensitivity to aspirin or non-steroidal anti-inflammatories, manif

Trichophyton asthma: sensitisation of bronchi and upper airways to dermatophyte antigen.

12 adult patients with perennial asthma and chronic skin infection were found to have immediate hypersensitivity to Trichophyton spp. 10 patients were tested by bronchial provocation and gave immediate bronchial reactions to an extract of T tonsurans. Double-blind, placebo-controlled nasal challenge of 8 patients demonstrated that the upper airways of these patients were also sensitive to this dermatophyte antigen. In addition to perennial asthma most of the patients had persistent eosinophilia and chronic rhinosinusitis. The results suggest that absorption of fungal antigen can give rise to I

Montelukast 10 mg improves nasal function and nasal response to aspirin in ASA-sensitive asthmatics: a controlled study vs placebo.

UNLABELLED: Aspirin-induced asthma (AIA) is a clinical syndrome characterized by acute airway reaction to aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDS). The most recent etiological hypothesises is that an overexpression of the enzyme LTC(4) synthase occurs in AIA, with the consequent production of sulfidopeptide leukotrienes (LTs). AIM: Aim of the present study was to assess the effect of Montelukast, a selective cys-LT receptor antagonist, on nasal function, nasal reactivity to ASA and blood markers of eosinophilic inflammation in mild-to-moderate AIA. MATERIAL AND METHOD:

Allergic rhinitis is a risk factor for traffic safety.

BACKGROUND: Allergic rhinitis (AR) affects up to 30% of the adult population and symptomatic patients continue to engage in daily life activities, including car driving. Previous studies have shown that AR can impair cognitive functions, especially during longer-lasting tasks. Other reports suggest negative effects on psychomotor functions such as driving, but no clear evidence has been presented yet. OBJECTIVES: Primary objective was to determine the effect of AR per se on actual driving performance and compare it with the effects of treated AR. METHODS: Nineteen patients with documented AR

Responses to ragweed-pollen nasal challenge before and after immunotherapy.

To evaluate whether immunotherapy reduces mediator release after nasal challenge, we followed previously untreated patients with ragweed hay fever through 2 years (three seasons) of treatment. Eleven adult patients started immunotherapy after a season of symptom diaries and graded pretreatment challenges with 0.03, 0.3, 3.3, and 16.5 mg of ragweed pollen. Repeat challenges were performed when the treatment dosage reached 0.6, 12.4, and 24.8 micrograms of Amb a I (antigen E) equivalents per injection. After the 0.6 micrograms dose, there was little change, but after the larger doses, there was

Role of Predatory Mites in Persistent Nonoccupational Allergic Rhinitis.

Mites can sensitize and induce atopic disease in predisposed individuals and are an important deteriorating factor in patients with allergic rhinitis, asthma, and atopic dermatitis. Although Pyroglyphidae mites have been extensively studied, very scarce reports are available on Cheyletidae spp. especially regarding human respiratory pathology. The main objective of the present study is to investigate the clinical role of this predator mite (Cheyletus eruditus) as a respiratory antigen in a selected sensitized human population. Fifty-two adult patients were recruited from the outpatient allergy

Effects of sublingual immunotherapy in patients sensitised to Ambrosia. An open controlled study.

BACKGROUND: Allergy to Ambrosia is a disease of growing importance in Europe. Injective and non-injective immunotherapy have been recognised as safe and effective but no evidence is currently available for sublingual immunotherapy (SLIT) in patients sensitised to Ambrosia. This study was planned to assess the effects and the safety of SLIT in patients clinically sensitised to Ambrosia. METHODS: 19 patients clinically sensitised to Ambrosia and treated with SLIT were compared to 14 patients treated only with drugs. Diary cards with symptoms and drug consumption were filled-in by patients durin

Appraisal of the validity of histamine-induced wheal and flare to predict the clinical efficacy of antihistamines.

Antihistaminic drugs have been used successfully for many years in the treatment of allergic diseases. Second-generation antihistamines have fewer sedating side effects than first-generation agents, and the number of newer drugs available for clinical use is growing. Various methods have been used to assess antihistaminic activity, the most popular of which is the epicutaneous histamine-induced wheal and flare. This test relies on the ability of epicutaneously injected histamine to bring about the wheal and flare, a neurovascular response that involves reflex vasodilation (flare) and local swe

Local allergic rhinitis in Asia: rethinking the allergy landscape.

PURPOSE OF REVIEW: Local allergic rhinitis (LAR) is increasingly recognized as a distinct phenotype of chronic rhinitis, yet its prevalence and characteristics in Asia remain underexplored. Given the variability in reported LAR prevalence across regions, this review re-evaluates the allergy landscape in Asia, considering environmental and immunological factors, as well as study methodology that may contribute to differences from Western populations. RECENT FINDINGS: There were more Asian studies which reported lower prevalence (0-20%), in contrast to higher rates in Europe, particularly Medit

Evidence of maxillary sinus inflammation in seasonal allergic rhinitis.

OBJECTIVE: Allergic rhinitis has been frequently associated with both acute and chronic sinusitis. Previous studies have shown an influx of eosinophils into the maxillary sinus after nasal challenge with allergen. The objective of this study was to determine, in humans, if the development of seasonal allergic inflammation, secondary to natural allergen exposure, leads to similar inflammation within the maxillary sinus. STUDY DESIGN: Prospective, longitudinal study. Setting. Academic medical center and research laboratory. SUBJECTS AND METHODS: Eighteen subjects were evaluated in and out of t

Clinical trial design, nasal allergen challenge models, and considerations of relevance to pediatrics, nasal polyposis, and different classes of medication.

Clinical trials in allergic rhinitis present several specific difficulties. In seasonal pollen-related disease, there are variations between subjects in the extent of pollen sensitization, individual variations in exposure to pollen even within a set area because of lifestyle differences, and variations between different areas in pollen counts and weather patterns. Thus, large patient numbers are needed in multicenter trials to account for such variations when the standard endpoint is symptom reporting. Furthermore, a pollen season may be relatively short (eg, lasting 6-8 weeks), and the polle
