Role of neck dissection in metastatic squamous cell carcinoma to the parotid gland.

OBJECTIVE: To investigate the rate of occult neck disease in patients with metastatic squamous cell carcinoma to the parotid gland following parotidectomy and neck dissection. METHODS: A consecutive series of patients treated between 2000 and 2014 for metastatic squamous cell carcinoma to the parotid were analysed. Patients were included if they had no clinical or radiological evidence of neck disease. Pathology of parotidectomy and neck dissection specimens was reviewed. Other variables analysed included patient immune status, surgery type, complications, use of positron emission tomography

Complications of neck dissection at a tertiary level hospital: study of 30 cases.

Metastatic dissemination into lymph nodes of neck occurs frequently in head neck cancers which down grade the patient's curability and survival. Neck dissection is a curable option for its management. To evaluate the complications following different types of neck dissection. This cross sectional study was conducted among patients undergone different types of neck dissection due to cervical nodal metastasis and attended follow up during July 2010 to June 2011 in Department of ENTs and Head-Neck surgery Dhaka Medical College Hospital, Dhaka, Bangladesh. Among 30 selected patient, 23 were male (

Influence of prophylactic neck dissection on rate of retreatment for papillary thyroid carcinoma.

BACKGROUND: Prophylactic neck dissection (PND) for papillary thyroid cancer is controversial. The objective of this study was to analyze the influence of PND on the rate of retreatment. METHODS: In this retrospective case-control study, papillary thyroid carcinomas >10 mm without ultrasonographic evidence of nodal disease (cN0) were treated with total thyroidectomy (TT) or TT with bilateral central compartment PND. All received postoperative radioactive iodine ((131)I) and were followed for at least 1 year. We compared the rate of retreatment (surgery or (131)I). RESULTS: Altogether, 246 pat

Spontaneous cervicocephalic arterial dissection with headache and neck pain as the only symptom.

BACKGROUND AND OBJECTIVE: Cervicocephalic arterial dissection can cause both ischemic stroke and hemorrhagic stroke. However, spontaneous cervicocephalic arterial dissection presenting only with headache and neck pain has rarely been reported. The clinical features of patients with spontaneous cervicocephalic arterial dissection presenting only with headache and neck pain were investigated. METHODS: The subjects were seven patients with spontaneous cervicocephalic arterial dissection with headache and neck pain alone who were admitted to our hospital during the past 3 years. The clinical feat

[Is it necessary to perform neck dissections as a staged procedure in laser supraglottic laryngectomy?].

INTRODUCTION AND OBJECTIVES: Endoscopic CO2 laser supraglottic laryngectomy has similar oncological results to the classical open approach. Treatment of the neck is of paramount importance in these tumours and it is usually performed as a staged procedure. The aim of this work is to ascertain if it is safe to perform the neck dissections at the same time as the laser supraglottic laryngectomy. METHODS: Twenty-four patients with supraglottic epidermoid carcinoma, who underwent laser CO2 supraglottic laryngectomy and bilateral neck dissection, were studied. In 12 patients the neck dissections w

[Level choice of the neck dissection in clinical N0 patients with supraglottic cancer].

OBJECTIVE: To study the characteristics of the cervical lymph node metastasis in negative node with clinically supraglottic cancer patients and decide which level should be dissected reasonably. METHODS: 57 modified neck dissection (MND) cases in the cN0 with supraglottic cancer were performed. Each lymph node in the 57 (63 sides) MND samples was studied pathologically in order to define the level of the lymph node metastasis. RESULTS: 1,877 lymph nodes were obtained in these MND samples; an average of 29.8 lymph nodes was obtained in one side. Among them 43 lymph nodes were metastasis nodes

Prognostic value of histologic findings in neck dissections for squamous cell carcinoma.

BACKGROUND: Cervical node involvement is the most significant prognostic factor in head and neck squamous cell carcinoma. When histologic findings show node invasion, the number of positive nodes and the presence of extracapsular spread are commonly accepted as prognostic factors. OBJECTIVE: To confirm the findings of recent reports that there is no significant difference in outcome associated with extracapsular spread. SETTING: Referral center. DESIGN: Retrospective study. PATIENTS: Three hundred thirty-seven patients under-going 487 neck dissections for carcinoma of the oral cavity, orop

Headache and neck pain in spontaneous internal carotid and vertebral artery dissections.

We studied the characteristics of headaches in 161 consecutive symptomatic patients with spontaneous dissections of the internal carotid artery (n = 135) or the vertebral artery (n = 26). For patients with internal carotid artery dissection (ICAD), the mean age was 47 years and for those with vertebral artery dissection (VAD), 40.7 years. A history of migraine was present in 18% of the ICAD group and in 23% of the VAD group. Headache was reported by 68% of the patients with ICAD and by 69% of those with VAD, and, when present, it was the initial manifestation in 47% of those with ICAD and in 3

Application of posterior neck dissection in treating malignant melanoma of the posterior scalp.

Utilization of a posterior neck dissection in the treatment of nine patients with malignant melanoma of the posterior scalp is presented. Seven of nine patients presented with metastatic disease present in the posterior neck after initial treatment elsewhere. Four of the seven patients developed regional disease 4 months to 4 years following initial diagnosis. Variables dictating surgical treatment included the Clark's level, Breslow depth of invasion of original lesion, evidence of metastatic disease, and type of melanoma. The surgical technique of the posterior neck dissection is graphically

The Interscalene Triangle: A Cadaveric Dissection of a Critical Neurovascular Corridor at the Root of the Neck.

The interscalene triangle is a key cervicothoracic gateway bounded by the anterior and middle scalene muscles and the first rib. It transmits the roots and trunks of the brachial plexus (C5-T1) and the subclavian artery (SA), while the subclavian vein (SV) courses anterior to the anterior scalene muscle and the phrenic nerve (PN) descends along its anterior surface. Cadaveric dissection highlights these critical topographic relationships, providing an anatomical basis for understanding thoracic outlet syndrome, optimizing ultrasound-guided regional anesthesia, and minimizing the risk of iatrog

Lymphedema Severity and Quality of Life as a Function of Selective Neck Dissection Technique.

OBJECTIVE: Combined lymphatic and venous insufficiency has been described as a consequence of lymph node dissections, but never in the context of selective neck dissections (SND). The objective of this retrospective cohort study was to compare two methods of SND: one (PEF), which preserves the facial vein (FV) and external jugular vein (EJV), and one (REF), which resects the FV and often the EJV. METHODS: Head and neck lymphedema was assessed with a modified external lymphedema-fibrosis scale and with the revised Patterson Scale (internal lymphedema). The FACT-HN questionnaire was used to ass

A case of complete remission of Hodgkin lymphoma confirmed histopathologically by neck dissection.

BACKGROUND: Hodgkin lymphoma (HL) is diagnosed definitively by biopsy, and treatment is based on stage. Owing to the nature of the disease, post-treatment efficacy is determined mainly by fluorodeoxyglucose-positron emission tomography/computed tomography, and the efficacy of treatment is not confirmed by histopathology. We report a case of tongue cancer after treatment for HL, in which a post-treatment lymph node with complete remission was histopathologically confirmed by neck dissection. CASE: The patient was a 74-year-old man who was referred to our hospital for cancer on the right side o

Use of neuromuscular blockade for neck dissection and association with iatrogenic nerve injury.

BACKGROUND: Cranial nerve injury is an uncommon but significant complication of neck dissection. We examined the association between the use of intraoperative neuromuscular blockade and iatrogenic cranial nerve injury during neck dissection. METHODS: This was a single-center, retrospective, electronic health record review. Study inclusion criteria stipulated patients > 18 years who had ≥ 2 neck lymphatic levels dissected for malignancy under general anesthesia with a surgery date between 2008 - 2018. Use of neuromuscular blockade during neck dissection was the primary independent variable. Th

Is it essential to remove the submandibular gland in neck dissection in tongue cancer patients?

INTRODUCTION: Submandibular sialadenectomy is a part of the classic technique of neck dissection for oral cavity cancers. However, its removal is associated with a reduction in the salivary outflow in many patients, as well as, some uncommon complications such as lingual and hypoglossal nerve injuries. Assessment of the necessity of such maneuvers should be addressed. PATIENTS AND METHODS: The data of 105 patients with tongue cancer who underwent neck dissection in the Oncology Center, Mansoura University from January 2008 to March 2019 were reviewed and analyzed retrospectively. RESULTS: In

Dissecting Thoracic Aortic Hematoma Masquerading as Blunt Force Injury of the Neck.

Discoloration of the skin of the anterior and lateral neck may raise suspicion for blunt force injury, particularly cervical compression, in an unwitnessed death. We present a case of an elderly woman with an unwitnessed death at home which highlights an external examination finding of blue/purple discoloration of the skin of the neck and links this finding with those from internal examination at autopsy. Pertinent negatives include absence of conjunctival and mucosal petechiae, absence of cutaneous abrasions of the neck, and absence of contusions of the anterior neck musculature. This case il

Metastases to the lingual nodes in tongue cancer: a pitfall in a conventional neck dissection.

Some classical textbooks of anatomy provided a detailed description of the lingual nodes, which are small inconstant lymph nodes in the floor-of-mouth and the upper neck. The clinical importance of these lymph nodes in cancer therapy, however, has been underestimated so far. We previously reported an extremely poor prognosis of oral tongue cancer patients who had lesions at the root of the lingual artery and assumed that metastases in occult lingual nodes might be responsible for such lesions. This case report clearly demonstrates the distinctive draining course of the lateral lingual nodes, w

Indication of elective contralateral neck dissection in squamous cell carcinoma of the hypopharynx.

UNLABELLED: Lymph node metastases (LNM) are common in hypophariyngeal carcinomas; the neck dissection is an important therapeutic approach. AIM: to analyze the incidence and distribution of LNM and failures in treating the contralateral neck. METHODS: a retrospective study of 174 patients with hypopharyngeal cancer treated from 1978 to 2003. The distribution of LNM and regional recurrences were evaluated. RESULTS: 44% of the cases were false negatives and 4.9% were false positives. Among the 48 patients who underwent bilateral ND, 29 had bilateral metastases and one had contralateral metast

[Internal jugular vein thrombosis following functional and selective neck dissections].

OBJECTIVES: Internal jugular vein (IJV) thrombosis is a rare complication of functional and selective neck dissections. It increases morbidity and may seldom be fatal. We investigated the frequency of IJV thrombosis and its relationship with the dissection technique. PATIENTS AND METHODS: We evaluated 52 functional and selective neck dissections performed in 34 male patients (mean age 57 years; range 34 to 76 years) with head and neck cancer. Dissections were mainly performed by sharp dissection (n=27) or cautery (n=25). The patients were examined by Doppler ultrasonography with respect to IJ
