Clinical presentation and treatment of transfusion-associated babesiosis in premature infants.

We review here 7 cases of neonatal transfusion-associated babesiosis at a NICU in the northeast United States. Transfusion from 2 infected u … ases. The risk of Babesia microti infection is not eliminated through current blood-bank practices. Neonatologists in endemic areas should have a high index of suspicion for babesiosis in premature infants exposed to blood transfusions.

Post-discharge nutrition of preterm infants.

The ever-increasing survival of premature and extremely low birth weight infants has posed a great challenge to neonatologists and nutritionists. While nutrient requirements are still being defined, there is a great need to continue to improve our strategies for providing effective nutrition for these infants. Similarly, postdischarge nutritional issues need to be addressed since catch-up growth does not always occur. In addition to fetal origins of adult diseases, especially cardiovascular disorders, diabetes and obesity, new data are emerging that postnatal growth alterations also affect the…

[Vein of Galen malformation in a newborn infant with clinical symptoms of a cardiologic emergency].

We report on the management of one term newborn infant with a malformation of the Vena Galeni. The baby suffered from cyanosis and intractab … nge-gated pulsed Doppler velocimeter should be available in newborn intensive care units. Otherwise neonatologists are forced to transport these infants to pediatric heart centers because of inability to detect such an extracardiac origin of heart and circulatory failure.

Short Bowel Syndrome in an Infant.

Short bowel syndrome (SBS) is a malabsorptive state that may occur either after surgical bowel resection or as the result of congenital bowel anomalies. SBS can incur significant morbidity and mortality including intestinal failure, cholestasis, sepsis, and death. For patients with SBS, management involves a multidisciplinary approach that begins with neonatology, pediatric surgery, nutritionists, pharmacists, and nurses in the NICU and also includes the transition to an intestinal rehabilitation program. The aim of this review is to provide the neonatologist with an overview of the common cau

Role of Transnasal Humidified Rapid-Insufflation Ventilatory Exchange Therapy in the Management of a Twin Pregnancy with H1N1 Infection in Early Acute Respiratory Distress Syndrome.

Pregnancy with H1N1 infection presenting with early acute respiratory distress syndrome (ARDS) is a challenging situation, where the life of both mother and fetus are jeopardized. Morbidity and mortality in such a clinical situation are not uncommon; it may result in hypoxemic acute respiratory failure in a pregnant patient, leading to mechanical ventilation and poorer outcomes in neonates due to prematurity. An interdisciplinary approach involving obstetricians, respiratory physicians, neonatologist, and anesthesiologist is mandatory for a good outcome. This case report highlights the managem

Apneic crises: a clue for MECP2 testing in severe neonatal hypotonia-respiratory failure.

Males with methyl-CpG-binding protein 2 (MECP2) mutations may present with neonatal encephalopathy. We report on an infant with a MECP2 mutation who exhibited complex constellation of symptoms, including severe hypotonia, respiratory failure, and apneic episodes. In the neonatal period these symptoms are common to other disorders, including Ondine syndrome. Our observation confirms that the triad of severe hypotonia, apneic episodes, and respiratory failure may be caused by MECP2 mutations. Neonatologist and neuropediatricians must be alert to the presence of these symptoms to exclude this rar

[Foetal medicine].

This paper deals with present possibilities in the field of diagnosis and prognosis of foetal malformations and developmental disturbances. Differential diagnosis of congenital malformations, including intrauterine growth retardation (IUGR), polyhydramnios and foetal hydrops is discussed on the basis of probable or suspected etiology. Indications for in-utero treatment, applicable only in a very limited and carefully selected number of cases, are considered. In the majority of severe cases of congenital malformations diagnosed prenatally, the only factors considered are: the optimal time and w

Epignathus: clinical and sonographic observations of two cases.

The epignathus or mature pluritissued teratoma of the mid-line of the palate is a benign tumor for which the neonatal prognosis is related to the obstruction of the superior airway. This paper concerns observations of two cases, which developed in different ways. Echography enables diagnosis of this type of malformation which classically combines a heterogeneous polylobed mass, polyhydramnios and the absence of other morphological anomalies. It enables an accurate differential diagnosis to be made by eliminating the other masses of the cephalic region. Echographic diagnosis also allows a ratio

Glycogen Storage Disease Type I With Hypercalcemia in an Infant: A Case Report.

Glycogen storage disease type I (GSDI) is an uncommon condition resulting from a deficiency or absence of glucose-6-phosphatase, a key enzym … , and calcium. The combination of hypertriglyceridemia, hypoglycemia, and hepatomegaly should alert neonatologists and pediatricians to consider GSDI in the diagnosis. Hypercalcemia arose as an unknown problem in GSDI patients and should be considered during acute attacks.

A Preterm Infant with Multiple Anomalies Diagnosed with Atypical CHARGE Syndrome after a Novel CHD7 Variant Confirmed Using Whole-Genome Sequencing.

CHARGE syndrome has a clinically broad spectrum of phenotypes, including partial or atypical type. CHD7 mutation is related to CHARGE syndrome that shows various phenotypes according to the CHD7 variant. Developments in genetic analysis techniques, such as next-generation sequencing (NGS), are helping both diagnosis and treatment of diseases. We report the case of a preterm infant diagnosed with atypical CHARGE who has a novel and de novo CHD7 variant that was identified using whole-genome sequencing (WGS). Neonatologists tend to be reluctant to diagnose infants with multiple malformations bec…

Hypertension in the Neonatal Period: An Update.

There has been a growing interest among neonatologists and paediatricians regarding identification and evaluation of hypertension in the neonatal period. Despite the emergent normative data on blood pressure values in term and preterm neonates over the last two decades, there is still controversy regarding correct definition and classification of hypertension. This article will discuss the current definitions, available normative data and etiology of neonatal hypertension. There is paucity of records in terms of efficacy of antihypertensive drugs in this specific population and management is u…

Heterologous assisted reproduction and kernicterus: the unlucky coincidence reveals an ethical dilemma.

Secrecy and anonymity related to heterologous assisted reproduction may hide basic newborn data to neonatologists. Secrecy and anonymity are discussed in view of their possible consequences on relational dynamics and on developmental psychology. Nevertheless, they can also involve the offspring's genetic status regarding inheritable diseases. International guidelines have been recently published on this topic. Because no guidelines are 'ideal' unfortunate and possibly dramatic consequences can occur. We aimed to embark on a debate about this matter starting with a real clinical experience. In …

Foetal surgery for spina bifida.

In utero repair of open spina bifida or myelomeningocele (MMC) is now performed in selected patients and presents an additional alternative to obstetricians and neonatologists counselling expectant mothers carrying a foetus with MMC. Early foetal intervention may improve neurologic outcomes and reduce the hindbrain herniation associated with the Arnold-Chiari II malformation in open spina bifida. These changes may improve long-term neurologic function and limit requirements for shunt placements and other surgical interventions. Further research is needed to better understand the pathophysiolog…

Bilious vomiting in the newborn: rapid diagnosis of intestinal obstruction.

Bilious vomiting in newborns is an urgent condition that requires the immediate involvement of a team of pediatric surgeons and neonatologists for perioperative management. However, initial detection, evaluation and treatment are often performed by nurses, family physicians and general pediatricians. Bilious vomiting, with or without abdominal distention, is an initial sign of intestinal obstruction in newborns. A naso- or orogastric tube should be placed immediately to decompress the stomach. Physical examination should be followed by plain abdominal films. Dilated bowel loops and air-fluid l…

Sudden infant death with periventricular leukomalacia.

Periventricular leukomalacia (PVL) is a form of cerebral infarction occurring in neonates, particularly in low-weight and premature infants. PVL is well-known to neonatologists, but generally considered nonfatal. Many infants with PVL die in the hospital with multiple medical problems. Those infants with PVL who survive because of intensive care will have serious motor and sensory deficits, but these problems are rarely recognized before one year of age. When infants with PVL die at home, death seems sudden and unexpected. However, it is important to distinguish death caused by PVL from the Su…
